Summary: Premature ASCVD patients often have multiple traditional cardiovascular risk factors, as well as genetic predisposition or unique non-traditional features such as substance abuse and chronic inflammatory conditions. Identification and management of at-risk individuals pose a great challenge for clinicians in this younger patient cohort.

Do We Really Need Aspirin Loading for STEMI?

Summary: Aspirin loading is recommended for patients with ST elevation myocardial infarction (STEMI) as soon as possible after presentation, but its effects on the therapeutic effects of other medications have not been studied. This review suggests that aspirin loading may attenuate the protective effects of statins, ticagrelor, morphine, and postconditioning in reducing myocardial infarct size. Further research is needed to investigate the effects of aspirin loading on these medications and to compare aspirin loading with alternative antiplatelet regimens in STEMI patients.

Cerebral Embolic Protection during Transcatheter Aortic Valve Implantation: Updated Systematic Review and Meta-Analysis

Summary: The use of cerebral protection devices (CPD) during transcatheter aortic valve implantation (TAVI) reduces the risk of stroke and improves outcomes. A systematic review and meta-analysis showed that CPD was associated with a significant reduction in major adverse cardiovascular events, mortality, and stroke in patients undergoing TAVI. The use of CPD also resulted in lower MRI volume per lesion. However, there was no significant difference in acute kidney injury, bleeding, or vascular complications. It should be noted that the reduction in mortality was mainly driven by observational studies. (Curr Probl Cardiol 2023;48:101127.)

Predictors of In-Hospital Mortality in Patients With End-Stage Renal Disease Undergoing Transcatheter Aortic Valve Replacement: A Nationwide Inpatient Sample Database Analysis

Summary: This study aims to identify predictors of mortality in patients with end-stage renal disease (ESRD) undergoing transcatheter aortic valve replacement (TAVR). The study found that baseline hypertension, liver disease, third-degree heart block, periprocedural pneumonia, cardiogenic shock, and ST-elevation myocardial infarction (STEMI) significantly increased in-hospital mortality in ESRD patients undergoing TAVR.

Summary: The incidence of readmissions after ST-elevation myocardial infarction (STEMI) in young patients is increasing, mainly due to the prevalence of cardiometabolic risk factors. Cardiovascular etiologies are the most common cause of readmission, and the development of cardiogenic shock and acute renal failure during the index admission is associated with significantly higher rates of readmission.

Summary: Compared to White patients, Other-races/ethnicities patients with T2MI have higher in-hospital mortality and longer length of stay. Hispanic and Other-races/ethnicities patients have higher hospital costs compared to White patients. All racial/ethnicity groups are less likely to be discharged to a facility compared to White patients.

Contemporary trends in the incidence of spontaneous coronary artery dissection (SCAD) - ethnic and household income disparities

Summary: This study investigated the overall incidence of spontaneous coronary artery dissection (SCAD) and its trends based on race, household income, and U.S. census regions. The incidence of SCAD is rising in all regions, with predominantly female patients. White population and higher-income groups have higher incidence rates. The highest incidence is in the West region, followed by the Northeast, Midwest, and South. There are significant differences in incidence rates among racial groups, household income quintiles, and U.S. census regions.

Summary: Radiation exposure is a significant occupational hazard for interventional cardiologists and cardiac catheterisation laboratory staff. The use of lead caps can effectively reduce radiation exposure to the head and should be widely employed in the catheterisation laboratory as a mainstay personal protective equipment.
