Machine Learning Models Prognosticate Functional Outcomes Better than Clinical Scores in Spontaneous Intracerebral Haemorrhage

This study compared the use of deep neural networks (DNN) and support vector machines (SVM) with clinical prognostic scores in predicting mortality and poor functional outcome in spontaneous intracerebral haemorrhage (SICH). The results showed that the SVM model performed significantly better than the clinical prognostic scores in predicting poor functional outcome at 90 days.
Objective: This study aims to develop and compare the use of deep neural networks (DNN) and support vector machines (SVM) to clinical prognostic scores for prognosticating 30-day mortality and 90-day poor functional outcome (PFO) in spontaneous intracerebral haemorrhage (SICH). Materials and methods: We conducted a retrospective cohort study of 297 SICH patients between December 2014 and May 2016. Clinical data was collected from electronic medical records using standardized data collection forms. The machine learning workflow included imputation of missing data, dimensionality reduction, imbalanced-class correction, and evaluation using cross-validation and comparison of accuracy against clinical prognostic scores. Results: 32 (11%) patients had 30-day mortality while 177 (63%) patients had 90-day PFO. For prognosticating 30-day mortality, the class-balanced accuracies for DNN (0.875; 95% CI 0.800-0.950; McNemar's p-value 1.000) and SVM (0.848; 95% CI 0.767-0.930; McNemar's p-value 0.791) were comparable to that of the original ICH score (0.833; 95% CI 0.748-0.918). The c-statistics for DNN (0.895; DeLong's p value 0.715), and SVM (0.900; DeLong's p-value 0.619), though greater than that of the original ICH score (0.862), were not significantly different. For prognosticating 90-day PFO, the class-balanced accuracies for DNN (0.853; 95% CI 0.772-0.934; McNemar's p-value 0.003) and SVM (0.860; 95% CI 0.781-0.939; McNemar's p-value 0.004) were better than that of the ICH-Grading Scale (0.706; 95% CI 0.600-0.812). The c-statistic for SVM (0.883; DeLong's p-value 0.022) was significantly greater than that of the ICH-Grading Scale (0.778), while the c-statistic for DNN was 0.864 (DeLong's p-value 0.055). Conclusion: We showed that the SVM model performs significantly better than clinical prognostic scores in predicting 90-day PFO in SICH.

Summary: This study aimed to identify the risk factors for a 30-day postoperative surgical site hematoma requiring evacuation after surgical resection of brain metastases. The results showed that younger age, higher BMI, and blood type AB were significantly associated with a higher risk of developing a 30-day postoperative hematoma. Additional care in intraoperative hemostasis and postoperative monitoring may be indicated among patients who have these risk factors.

Analysis of 345 cases revealed a mortality rate of 40.8%, with a higher prevalence of severe-to-critical COVID-19 and non-aneurysmal SAH among COVID-SAH deaths. Further studies are warranted to explore the association between COVID-19 and SAH and the long-term effects of COVID-19.

Summary: This study aimed to validate the associations of plaque inflammation on FDG-PET and the symptomatic carotid atheroma inflammation lumen-stenosis (SCAIL) score with recurrent ischemic events. A single-center prospective cohort study recruited patients with recent TIA or ischemic stroke, carotid artery stenosis, and not considered for early revascularization. Measurements of plaque inflammation and stenosis severity were taken, and the results showed that FDG-PET demonstrated better predictive abilities than the SCAIL score in risk stratification for patients. The study concluded that further research is needed to evaluate the benefits of carotid revascularization and/or anti-inflammatory therapy based on plaque inflammation detected on FDG-PET.

Summary: This study aimed to evaluate the outcomes of surgical intervention compared to medical therapy in the treatment of malignant posterior circulation infarcts (MPCI). A systematic review of 31 studies with 723 patients showed that surgical therapy led to significant differences in mortality and functional outcomes in patients with severe disease. However, limited evidence exists regarding preventative neurosurgery and the superiority of one surgical modality over another.

Effect of blood type on mortality among patients with brain metastases

The International Pediatric Stroke Study (IPSS) risk factors and Embolic Stroke of Undetermined Source Potential Embolic Sources (ESUS-PES) construct may better reflect the heterogeneous risk factors and etiologies in young stroke patients.

Cognition, Emotional States and Health-Related Quality of Life in Awake Craniotomy for Glioma: A Case Series

Summary: This case series study investigated the perioperative cognitive function and emotional states in a multilingual Asian population of glioma patients who underwent awake craniotomy. The study found that higher preoperative cognitive scores were associated with better follow-up HRQoL, while preoperative depressive and stress symptomatology were associated with worse follow-up HRQoL. Mild preoperative anxiety was associated with better follow-up HRQoL.

Safety and efficacy of endovascular thrombectomy for primary and secondary MeVO

Summary: This study aimed to evaluate the efficacy and safety of endovascular treatment (EVT) in primary and EVT-induced secondary medium-vessel occlusions (MeVO). The results showed that primary MeVO had poorer outcomes in terms of complications, EVT efficacy, and the proportion of patients with a modified Rankin Scale (mRS) of 0-2 at 3 months compared to exclusive large-vessel occlusion (LVO). However, EVT-induced secondary MeVO showed similar safety and efficacy outcomes as exclusive LVO and had better outcomes in terms of mRS 0-2 compared to primary MeVO.

One year clinical outcome of dual anti-platelet therapy with the Novel Ticagrelor plus Aspirin versus Clopidogrel plus Aspirin for Endovascular Intervention of patients with Intracranial Aneurysm: A meta-analysis

Summary: Compared with clopidogrel, DAPT with ticagrelor plus aspirin was associated with significantly lower thromboembolic events without an increase in hemorrhagic events in the treatment of intracranial aneurysms, according to the one-year clinical outcome analysis. However, larger upcoming trials are needed to confirm the effectiveness and safety of ticagrelor-associated DAPT.

An examination of geographic access to outpatient stroke rehabilitation services in Tennessee, a stroke belt state

Summary: This study assessed the geographic accessibility of stroke rehabilitation facilities in Tennessee. The study found that rural counties had significantly longer travel distances and times compared to urban counties. Counties with high and medium stroke prevalence also had higher travel times and distances compared to low stroke prevalence counties. Therefore, addressing transportation barriers and implementing tele-rehabilitation are crucial for improving access to stroke aftercare in these areas.

Role of serum alkaline phosphatase as a potential biomarker of severity and prognosis in intracerebral hemorrhage

The results showed that serum ALP levels in ICH patients were significantly elevated and correlated with hematoma volume and Glasgow coma scale (GCS) score. High serum ALP levels may serve as a potential predictor for the severity and unfavorable prognosis of ICH.
