Summary: Early surgical treatment could be an alternative to biological therapy for patients with ileocecal Crohn's disease. The study compares outcomes of ileocecal resection for luminal and complicated CD, finding that surgical procedures were longer and had lower laparoscopic approach rates for the complicated CD group.

The results showed differentially methylated positions and differentially variable positions in patients with CD recurrence. The study also validated the previously reported CD-associated methylome.

Summary: This study aimed to evaluate whether pre-operative nutritional status could predict post-operative complications in patients with Crohn's disease (CD) and whether pre-operative enteral nutrition (EN) can prevent post-operative complications. The study found that pre-operative nutritional support significantly affected post-operative outcomes, and enteral nutrition improved pre-operative nutritional status and reduced the rate of post-operative adverse events.

Summary: Bacterial vaginosis (BV) is a condition where the normal protective Lactobacilli are replaced by high quantities of facultative anaerobes, leading to gynecologic and obstetric post-operative complications. BV is an important cause of obstetric and gynecological adverse sequelae and it increases the risk of sexually transmitted infections. BV is often asymptomatic, can resolve spontaneously, and often relapses with or without treatment.

Summary: This study aimed to investigate risk factors for pre-operative anemia in gynecologic cancer patients and describe outcomes among patients undergoing surgery by a gynecologic oncologist. The results showed that 23.1% of patients undergoing surgery by a gynecologic oncologist had pre-operative anemia, with the highest rate observed in ovarian cancer patients (39.7%). Patients with advanced-stage cancer had a higher risk of anemia compared to those with early-stage disease. Pre-operative anemia was associated with increased odds of peri-operative complications.

Summary: Crohn's disease is increasing globally and changing toward more colonic and inflammatory behavior. While surgery was previously performed mainly for ileal/ileocaecal location and stricturing behavior, there is now an increasing complexity among patients at the time of surgery with a higher number of patients having abdominal perforating disease. To improve care for patients with penetrating abdominal CD, multidisciplinary team discussions are essential, along with proper surgical planning and evaluation of patients prior to surgery.

Summary: Exacerbation or new onset orthostatic hypotension in perioperative patients may lead to post-operative complications, especially in frail and older patients. This study found that patients with pre-operative orthostatic hypotension were more likely to experience falls, dizziness, and unsteadiness post-operatively. Further research into modifiable perioperative risk factors associated with orthostatic hypotension is necessary to improve patient outcomes.

Summary: This study investigated the various factors that contribute to the development of postoperative complications in patients who undergo surgery for complicated Crohn's disease. The results showed that patient age, smoking status, steroid use, perianal disease, and presence of a stoma were significantly associated with the occurrence of complications. Close monitoring of patients with Crohn's disease who undergo surgery for the presence of complications is recommended.

Summary: Many patients with Crohn's disease require surgical intervention over time despite aggressive medical therapy, commonly leading to disease recurrence. Ileo-colonoscopy and the Rutgeerts score are used for diagnosis and monitoring, but their limitations can be addressed by using wireless capsule endoscopy as a potentially more accurate, less invasive, and less costly alternative.

Summary: The study found that local excision after neoadjuvant therapy may provide comparable survival benefits to radical surgery for selected patients with clinical stage T2N0 rectal cancer. However, the evidence is still limited to provide solid recommendations, and a personalized therapeutic approach taking into account tumor and patient-related factors should be considered.

Summary: This study evaluated the diagnostic accuracy of handheld bowel sonography (HHBS) compared with magnetic resonance enterography (MRE) for Crohn's disease (CD) diagnosis. The results showed that both HHBS and MRE had high sensitivity and specificity for CD diagnosis, but MRE was superior in defining the extent, location, and complications of the disease.

Summary: Although local excision after neoadjuvant treatment has shown promising results in selected patients, radical surgery is still necessary in cases of unfavorable pathology. Patients with pathological incomplete response have a higher risk of late-onset rectal bleeding and positive resection margins, but long-term recurrence rates and survival outcomes are comparable.
