Simulation-based VATS resection of the five lung lobes: a technical skills test

Assessing VATS competence based on simulated lobectomies of all five lung lobes

Summary: The study aimed to determine the number of procedures and expert raters necessary to provide a reliable assessment of competence in Video-Assisted Thoracoscopic Surgery (VATS) lobectomy. The results showed that a minimum of two procedures and four raters or three procedures and three raters were needed to ensure the overall reliability of 0.8. In addition, a credible pass/fail level was established using the contrasting groups' standard setting method.

Summary: This study aims to determine the difference in outcome between surgery (video-assisted thoracoscopic surgery) and non-surgical treatment (thoracic ultrasound-guided drainage and intrapleural therapy) in patients with complex parapneumonic effusion and pleural empyema. The primary endpoint is length of hospital stay, and secondary endpoints include mortality, need for additional interventions, analgesia consumption, and quality of life.

Summary: The aim of this study was to develop a reliable composite score based on simulator metrics to assess competency in virtual reality video-assisted thoracoscopic surgery lobectomy, and to explore the benefits of combining it with expert rater assessments. The results showed that combining simulator metrics with expert rater scores can increase reliability and be used for assessing surgical trainees.

The findings suggest that the program may compromise nurses' professional identity and responsibilities, leading to overlooking patients' life situations and values. Continuing specialized thoracic nursing after discharge is crucial to provide comprehensive care and maintaining a caring attitude.

Summary: Lung neuroendocrine neoplasms (NEN) are a heterogeneous group of tumors different from common lung cancers in terms of pathology, clinical behavior, and prognosis. Recent advances and new methods in diagnosis and treatment of lung-NEN patients are summarized in these Nordic guidelines. These guidelines are designed to be useful for clinicians in their daily practice and reflect the current state of the art. Small cell lung carcinoma (SCLC) is not covered in these guidelines.

Summary: Pulmonary nodules are commonly encountered by respiratory physicians, radiologists, and thoracic surgeons. A collaborative effort by the European Association of Cardio-Thoracic Surgery and the European Society of Thoracic Surgeons aims to provide a comprehensive review of the management of pure ground-glass opacities and part-solid nodules. The literature suggests an increasing detection of early-stage lung cancer, emphasizing the importance of surgical resection and the need for guidelines. Multidisciplinary decision-making is recommended, considering radiologic characteristics, lesion evolution, patient fitness, and co-morbidities.

Continuous perioperative heart rate variability monitoring in video-assisted thoracoscopic surgery lobectomy-a pilot study

Summary: Heart rate variability (HRV) is a measure of cardiac autonomic modulation and is potentially related to hypotension, postoperative atrial fibrillation, and orthostatic intolerance. However, there is a lack of knowledge on which specific time points and indices to measure. To improve future study design, there is a need for procedure-specific studies in an enhanced recovery after surgery (ERAS) video-assisted thoracic surgery (VATS) lobectomy setting, and for continuous measurement of perioperative HRV.

Consensus on technical procedures for simulation-based training in thoracic surgery: an international needs assessment

Summary: A Delphi survey was conducted to identify and prioritize technical procedures for simulation-based training in the thoracic surgical curriculum. The survey involved 34 key opinion leaders from 14 countries and resulted in the selection of 17 procedures for training.

Early and late readmissions after enhanced recovery thoracoscopic lobectomy

Summary: This study aimed to describe the incidence and reasons for early and late readmissions after enhanced recovery video-assisted thoracoscopic surgery lobectomy. The results showed that pulmonary complications and adjuvant chemotherapy were the predominant reasons for readmissions.
