Summary: This narrative review provides an overview of the application of endoscopic ultrasound-guided biliary drainage (EUS-BD) for the treatment of malignant biliary obstruction, with a focus on EUS-guided gallbladder drainage (EUS-GBD) using lumen apposing metal stent (LAMS). EUS-BD has shown high rates of technical and clinical success, with fewer adverse events compared to percutaneous trans-hepatic biliary drainage (PTBD). It is currently considered as the preferred alternative technique for biliary drainage in patients with distal malignant biliary obstruction (DMBO) after failed endoscopic retrograde cholangiopancreatography (ERCP).

Summary: Acute cholecystitis is a common disease in clinical practice. Laparoscopic cholecystectomy is the standard treatment, but it may be too risky for certain patients. Non-operative treatments such as percutaneous gallbladder drainage, endoscopic transpapillary gallbladder drainage, and EUS-guided drainage can be effective alternatives. These options should be considered in a stepwise approach after an accurate evaluation in a multidisciplinary discussion.

Summary: This study used national inpatient data from 2007 to 2017 to analyze trends in hospitalizations and outcomes of colonic volvulus (CV) in the United States. The results showed an increase in CV-related hospitalizations, but a decrease in inpatient mortality. Among the CV-related hospitalizations, those treated with endoscopic intervention had lower mortality, shorter length of stay, and lower healthcare charges compared to those treated with surgery.

Summary: This study aimed to evaluate the safety and efficacy of a novel hemostatic peptide gel, PuraStat, in preventing and controlling bleeding during EUS-guided drainage of WOPN. The results showed a technical success rate of 100% with PuraStat, and it effectively prevented bleeding in most cases and managed active bleeding in a few cases without any reported adverse events.

In recent years, bariatric endoscopy has emerged as an alternative to surgery, with procedures such as intragastric balloons and endoscopic gastroplasty showing effectiveness in inducing weight loss. Other novel endoscopic systems aim to improve both weight loss and metabolic conditions. Revisional endoscopic therapies are performed to address weight regain after bariatric surgery, with techniques like APC-TORe and Re-EndoSleeve showing promise. This review explores the efficacy and safety of primary and revisional endoscopic bariatric therapies and their potential application in clinical practice.
