Summary: This review provides an overview of the epidemiology, pathogenesis, clinical manifestations, diagnosis, treatment, and prognosis of eosinophilic granulomatosis with polyangiitis (EGPA). EGPA is a rare small to medium vessel necrotizing vasculitis that is often misdiagnosed due to its evolving nature. The unique features of EGPA include asthma and eosinophilia. The disease commonly presents with polyneuropathies and heart involvement, which are often severe and require aggressive immunosuppressive therapy. The treatment approach depends on the severity of the disease, with more severe cases requiring aggressive therapies such as cyclophosphamide. Asthma management often requires prolonged treatment with corticosteroids due to the high rate of disease relapses. Improved understanding of the heterogeneity of the disease is necessary for the development of better therapies.

The study findings were endorsed by ACR and EULAR.

Summary: This study aimed to develop an OMERACT ultrasonography score for monitoring disease activity in giant cell arteritis (GCA) and evaluate its metric properties. The study followed the OMERACT Instrument Selection Algorithm and conducted Delphi rounds and online reliability exercise. The results showed that the OGUS had good reliability and sensitivity in assessing disease activity in GCA and could be used in clinical trials.

Summary: This study aimed to identify risk factors associated with severe infections in patients with antineutrophil cytoplasm antibody-associated vasculitis (AAV). The use of low-dose TMP/SMX was found to be associated with a reduced risk of severe infections in patients treated with either rituximab (RTX) or cyclophosphamide (CYC)/azathioprine (AZA). Reduced B cell subpopulations at the start of treatment may be a useful indicator of reduced immunocompetence.

Summary: The objective of this study was to analyze the prevalence and characteristics of subclinical giant cell arteritis (GCA) in patients with polymyalgia rheumatica (PMR). The researchers used ultrasound to examine multiple arteries of PMR patients and found some of them had subclinical GCA. Patients with subclinical GCA were older, had longer morning stiffness duration, and more frequently reported hip pain compared to those without subclinical GCA. The optimal screening strategy to detect vasculitis in PMR needs further investigation.

Summary: A matched study was performed to compare the periprosthetic fracture rates after TKR and UKR. The fracture rate after UKR was approximately twice as high as that after TKR within the first 3 months postoperatively. However, over the first 10 years, the cumulative fracture rate after TKR was almost twice as high as that after UKR. Fracture rates after both UKR and TKR were higher in women, patients aged 75 or older, and patients with normal weight.

Summary: Anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides are a group of diseases characterized by necrotizing inflammation of small vessels. Different subgroups of AAV, including GPA, MPA, and EGPA, are defined based on clinical features. Renal involvement is common in AAV, especially in MPA. The prognosis of untreated AAV is poor, but with appropriate immunosuppressive therapy, the 5-year renal survival rate can reach 70-75%. Challenges in the management of AAV include improving disease activity measures, determining optimal therapy duration, and exploring targeted therapies with fewer adverse effects.

Summary: This study aimed to investigate the effects of immunosuppressive doses on the results of the QuantiFERON-TB Gold Plus (QFT-Plus) test in patients with Rheumatoid Arthritis (RA), as well as the impact of the TB2 tube in the QFT-Plus test. The results showed that higher doses of immunosuppressive drugs in RA patients led to a lower positive rate in the QFT-Plus test, and the addition of the TB2 tube improved the test sensitivity.
