Summary: This study investigated the effectiveness of the fourth BNT162b2 vaccine dose in reducing the rate and severity of COVID-19 caused by the Omicron variant in renal transplant recipients. The results showed that the fourth dose of the vaccine can lower the infection rate and minimize the severity of the disease, despite the reduced immune response to the variant.

Summary: This retrospective study aimed to characterize antibody responses induced by a third dose of mRNA COVID-19 vaccine in kidney transplant recipients and CLL patients. Results showed a moderate increase in SARS-CoV-2 anti-spike IgG levels after the third dose, but the impact on prevalence of anti-spike IgG & GE; 30 BAU/mL was limited in these immunocompromised patients.

Summary: This study aimed to predict acute rejection in kidney transplant recipients using a set of integrated gene signatures. While the gene signatures showed good predictive values within their own cohorts, the predictability decreased when tested in other independent datasets. Integration of gene signature sets with high specificity scores may improve the prediction performance of these markers.

Summary: The immune response to two doses of SARS-CoV-2 mRNA vaccine is limited among kidney transplant recipients. This study aimed to evaluate the humoral and cellular response to a third dose of the BNT162b2 vaccine. The results showed that the third dose improved the immune response among kidney transplant recipients, although 30% of them remained seronegative. Temporary reduction of immunosuppression before vaccination was found to improve the antibody response.

Survivors of COVID-19 and those vaccinated on the waiting list prior to transplantation exhibit a robust immune response to mRNA vaccines.

Summary: This study found that the long-term humoral response after the second dose of SARS-CoV-2 mRNA vaccine in Japanese KTRs was poor. In comparison with high-dose, low-dose mycophenolic acid was related to an appropriate humoral response. Five months is too long to wait for a third dose after the second dose of the vaccine in KTRs. In this cohort, there was no statistical difference in humoral response to the BNT162b2 and mRNA-1,273 vaccines. Additional large observational studies and meta-analyses are needed to clarify the factors related to an appropriate humoral immune response to COVID-19 vaccination.

Summary: Kidney transplant recipients (KTRs) show poor response to SARS-CoV-2 mRNA vaccination, with a significant number remaining seronegative and lacking neutralizing antibodies. Third doses of mRNA vaccine in KTRs did not improve the response and showed low levels of spike-specific CD8+ T cells. B cell dysfunction and ineffective T cell help may contribute to the lack of neutralizing response. Developing more effective vaccine strategies for KTRs is crucial.

Summary: This study evaluated the humoral immune response and safety of the fourth dose of COVID-19 vaccine in solid organ transplant recipients through a systematic review and meta-analysis. The results showed that approximately 56-92% of recipients developed a humoral immune response after the third dose, and approximately 76-95% developed a response after the fourth dose. All doses of the vaccine were associated with relatively mild adverse effects and did not require hospitalization.

Summary: This large single-center study aimed to identify predictive factors for the serological response to the mRNA-1273 vaccine in kidney transplant recipients (KTRs) and developed a score for optimizing personalized vaccination strategies. The study found that immunosuppressive drugs, graft function, age, interval from transplantation, body mass index, and sex were associated with vaccine response in KTRs. The study suggested considering a fourth vaccine dose and/or pre-exposure prophylaxis with monoclonal antibodies for KTRs who remain unprotected.
