Summary: Adjuvant radiotherapy after resection of distal cholangiocarcinoma was associated with a survival benefit in patients, even in patients with margin-negative or node-negative resections. Adjuvant radiotherapy should be considered routinely irrespective of margin and nodal status after resection of distal cholangiocarcinoma.

Summary: This study demonstrates that hepatic arterial infusion chemotherapy (HAIC) is a safe and effective treatment for unresectable intrahepatic cholangiocarcinoma (iCCA), providing better intrahepatic tumor control compared to systemic chemotherapy (SC). Patients with a single tumor are more likely to benefit from HAIC.

Summary: Intrahepatic cholangiocarcinoma (iCCA), the most frequent subtype of cholangiocarcinoma (CCA), has seen an increased incidence globally. This study aimed to assess the impact of fibrosis on survival in iCCA patients undergoing palliative chemotherapy. The results showed that fibrosis did not significantly affect overall survival (OS) or progression-free survival (PFS) in iCCA patients. However, patients with cirrhosis and iCCA receiving palliative chemotherapy had lower OS rates.

Summary: The effectiveness of adjuvant gemcitabine plus cisplatin (GemCis) therapy was evaluated in a homogeneous group of high-risk patients with resected, lymph node-positive extrahepatic cholangiocarcinoma. The study found that adjuvant GemCis did not improve survival outcomes compared with capecitabine in this patient population.

Summary: The study suggests that adjuvant chemoradiotherapy may benefit patients with perihilar cholangiocarcinoma. For R1 resection patients and stage III-IVA patients, adjuvant chemotherapy or chemoradiotherapy is recommended for improved overall survival. Further prospective studies are needed to validate these findings.

Summary: This study found that neoadjuvant chemotherapy can improve long-term survival in patients undergoing resection for cholangiocarcinoma, especially in those with tumor within the resection range and negative lymph nodes. However, there was no difference in outcomes between neoadjuvant chemotherapy and adjuvant chemotherapy.

Summary: This study aimed to investigate the role of adjuvant chemotherapy after radical resection in patients with early-stage intrahepatic cholangiocarcinoma (ICC). The results showed that adjuvant chemotherapy can prolong overall survival (OS) in these patients, and preoperative elevation of CA 19-9 levels and nerve invasion are independent prognostic factors for poor survival outcomes. All chemotherapy regimens used in the study are safe.

Summary: This study retrospectively analyzed clinical data of patients with cholangiocarcinoma who received treatment and found that the prognosis of advanced cholangiocarcinoma is determined by macroscopic tumor characteristics rather than gene mutations. High microsatellite instability may be a favorable predictor of response to immunotherapy for cholangiocarcinoma.

Summary: Photochemical internalization (PCI) is a novel technology that enhances the local therapeutic effect of cancer drugs by using a specially designed photosensitizing molecule. The main cytotoxic effect of PCI is the disruption of endosomal membranes to release drug molecules, resulting in delivery of chemotherapy drugs. PCI with gemcitabine showed promising results in patients with unresectable perihilar cholangiocarcinoma, with good disease control, response rates, and survival outcomes.
