Summary: Patients with absent contractility (AC) have more esophageal symptoms and require more intense gastrointestinal follow-up than controls. Only a minority of patients with AC have underlying rheumatologic disease. Those without rheumatologic disease at baseline did not subsequently develop one, suggesting that a rheumatologic evaluation is likely unnecessary. The clinical course of AC in patients with rheumatologic disease and those without appears to be similar.

Summary: Parkinson's disease is a neurodegenerative disorder characterized by motor and non-motor dysfunction, with dysphagia being a common yet often underdiagnosed symptom. A multinational group of experts conducted a systematic review and consensus conference to provide guidance on the screening, diagnosis, and impact of dysphagia on the prognosis and quality of life of PD patients. The consensus statements offer a framework for timely detection and accurate diagnosis of dysphagia in PD.

Summary: The study validated the usefulness of the modified Videofluoroscopic Dysphagia Scale (mVDS) in determining feeding method and predicting dysphagia prognosis. Among the parameters of mVDS, aspiration showed the highest reliability, while triggering pharyngeal swallow and pyriformis residue had the lowest. The ICC of the total mVDS score was 0.876, significantly correlated with feeding method chosen and presence of aspiration pneumonia.

Summary: This study investigated characteristics of dysphagia aortica (DA) by comparing videofluoroscopic swallowing study (VFSS) findings among postoperative dysphagia aortica (PDA) patients, dysphagia after brainstem infarction (DBI) patients, dysphagia after anterior cervical discectomy and fusion (DACDF) patients, and subjective swallowing difficulty (SSD) patients. The study found that PDA patients had higher FDS scores on thin liquids than SSD patients but lower than DBI patients, while their scores on thick liquids were lower than DBI or DACDF patients. Additionally, PDA patients had longer esophageal transit time compared to other groups.

Dysphagia was associated with laryngeal penetration and aspiration. The reconstruction method has a significant impact on swallowing, with a focus on minimizing dysphagia.

Summary: The peristaltic reflex is a fundamental behavior of the gastrointestinal tract, involving the activation of neurons to produce propulsive contractions. This study found that there are mechanisms other than cholinergic neurons involved in the generation and propagation of colonic migrating motor complexes (CMMCs), and that poststimulus excitations in interstitial cells of Cajal (ICC) play a role in this process. These findings provide alternative insights into the mechanisms of CMMCs in the colon.

Summary: This study investigated the clinical features and management strategy of patients with delayed perforation after gastric endoscopic submucosal dissection (ESD). Among 11,531 patients who underwent ESD, delayed perforation occurred in 0.13% of cases and was more common in older patients. Patients with delayed perforation presented with severe abdominal pain and were diagnosed through chest X-rays or CT scans. Non-surgical treatments, including endoscopic closure, might be effective for perforations smaller than 1 cm.
