The COVID-19 pandemic has significantly impacted gastroenterology patients and healthcare systems, forcing them to adapt to new measures to ensure continuous medical care. Evaluating the impact of the pandemic on this field is crucial for improving service quality and anticipating future practices and policies.
Background The COVID-19 pandemic has created unprecedented challenges in all fields of society with social, economic, and health-related consequences worldwide. In this context, gastroenterology patients and healthcare systems and professionals have seen their routines changed and were forced to adapt, adopting measures to minimize the risk of infection while guaranteeing continuous medical care to chronic patients. Objective At this point, it is important to evaluate the impact of the pandemic on this field to further improve the quality of the services provided in this context. Methods/Results/Conclusion We performed a literature review that summarizes the main aspects to consider in gastroenterology, during the pandemic crisis, and includes a deep discussion on the main changes affecting gastroenterology patients and healthcare systems, anticipating the pandemic recovery scenario with future practices and policies.

Metabolic dysfunction-associated fatty liver disease and excessive alcohol consumption are both independent risk factors for mortality

Laurens A. van Kleef, Robert J. de Knegt, Willem Pieter Brouwer

Summary: The study aimed to investigate the mortality risk of MAFLD in relation to excessive alcohol consumption and its potential interactions. The results showed that MAFLD increases mortality risk independent of excessive alcohol consumption. Furthermore, participants with both MAFLD and excessive alcohol consumption have the highest mortality risk.

End-of-treatment HBsAg, HBcrAg and HBV RNA predict the risk of off-treatment ALT fiares in chronic hepatitis B patients

Sylvia M. Brakenhoff, Margo J. H. van Campenhout, Annemiek A. van der Eijk, Willem P. Brouwer, Florian van Bommel, Bettina E. Hansen, Thomas Berg, Harry L. A. Janssen, Robert A. de Man, Milan J. Sonneveld

Summary: The study aimed to investigate the relationship between off-treatment ALT flares and virological outcomes, as well as to explore whether serum levels of novel biomarkers at the end of treatment can predict flares. The results showed that ALT flares were associated with decreased sustained response rates, and higher levels of HBsAg, HBV RNA, and HBcrAg were associated with an increased risk of flares.

Summary: This study evaluated the effectiveness of biologic agents in Crohn's disease complicated with internal fistulizing disease. It found that biologic therapy is beneficial for approximately three-quarters of patients, achieving a fistula closure rate of 24%, but around one-third of patients still require surgery. Patient- and lesion-related factors can identify patients who will obtain more benefit from these drugs.

Malnutrition with Low Muscle Mass Is Common after Weaning off Home Parenteral Nutrition for Chronic Intestinal Failure

Lucas Wauters, Solene Dermine, Brune de Dreuille, Joanna Bettolo, Coralie Hutinet, Ashiq Mohamed, Emilie Lecoq, Lore Billiauws, Alexandre Nuzzo, Olivier Corcos, Francisca Joly

Summary: This study investigated the differences in outcomes after weaning off intravenous support in adult patients with chronic intestinal failure. Nutritional and functional markers were assessed, and it was found that malnutrition and reduced muscle mass were common in these patients. Treatment with teduglutide was effective in preventing these complications.

Summary: This systematic review and meta-analysis found that the clinical response and remission rates in Crohn's disease patients treated with biological agents have remained stable over the years. The publication year did not significantly influence these outcomes, except for a decreased effect on clinical remission in maintenance studies.

How many biomarker measurements are needed to predict prognosis in Crohn's disease patients under infliximab?-A prospective study

Fernando Magro, Maria Manuela Estevinho, Gaia Catalano, Marta Patita, Bruno Arroja, Paula Lago, Isadora Rosa, Helena Tavares de Sousa, Paula Ministro, Irina Mocanu, Ana Vieira, Joana Castela, Joana Moleiro, Joana Roseira, Eugenia Cancela, Paula Sousa, Francisco Portela, Luis Correia, Paula Moreira, Mafalda Santiago, Sandra Dias, Joana Afonso, Silvio Danese, Laurent Peyrin-Biroulet, Claudia Camila Dias

Summary: This study aimed to evaluate the performance of readily available biomarkers and develop risk matrices for predicting Crohn's disease (CD) progression. The results showed that the presence of anemia, highly elevated CRP, and FC in at least one visit were significant predictors. Combining these biomarkers in risk matrices had good ability to predict disease progression.

Adherence to Vaccines in Adult Patients with Immune-Mediated Inflammatory Diseases: A Two-Year Prospective Portuguese Cohort Study

Candida Abreu, Antonio Martins, Fernando Silva, Gabriela Canelas, Lucia Ribeiro, Stefano Pinto, Antonio Sarmento, Fernando Magro

Summary: Patients with immune-mediated inflammatory diseases treated with immunomodulatory therapy are at higher risk of infections, but adherence to vaccination is suboptimal. In this study, less than 5% of patients had all their vaccines up-to-date, and a majority did not fully adhere to the recommended vaccination schedule.
