Summary: This study validated the applicability of the 7th and 8th editions of the AJCC staging system for IPMN-associated PDAC. The 8th edition introduced T1a sub-staging and a new distinction between N1 and N2, both of which showed mixed results. The findings support the use of the AJCC 8th edition in the IPMN-associated PDAC population, but suggest that certain cutoffs may need to be revisited.

Heart transplantation for pediatric foreign nationals in the United States

Summary: This study compared the survival outcomes, waitlist mortality, and waitlist days of heart transplantation between pediatric foreign nationals and pediatric United States citizens. The study found that heart transplantation for pediatric foreign nationals, mainly indicated for cardiomyopathy, had a significantly higher transplant rate and better survival outcomes compared to pediatric United States citizens. This difference in survival outcomes may be attributed to the differing diagnoses for which transplantation was performed.

The impact of geographic location versus center practice on center volume in liver transplantation after the acuity circle policy

Summary: This study evaluated the impact of the acuity circles policy on liver transplant volumes. The results showed a significant increase in liver transplants for patients with MELD scores >= 29 and shorter wait times. Additionally, it was found that transplant centers in high-population states increased their transplant volume, while centers in nonhigh-population states utilized a higher proportion of marginal and circulatory death donors.

Evaluating the outcomes of donor-recipient age differences in young adults undergoing liver transplantation

Summary: The current liver allocation system may disadvantage younger adult recipients by not considering the age difference between donors and recipients. This study aimed to investigate the long-term prognostic influence of the donor-recipient age difference in young adult recipients. The results showed that a larger age difference in younger recipients was associated with inferior long-term survival, highlighting the importance of considering age in liver transplantation.

Summary: This study compared the long-term survival outcomes of liver transplantation (LT) with surgical resection and non-curative intent treatment (non-CIT) for patients with very early intrahepatic cholangiocarcinoma (ICC) and cirrhosis. The results showed that the 5-year overall survival rate of LT patients was similar to those who underwent resection, but better than patients treated with non-CIT. Therefore, LT may be a treatment option for patients with early stage ICC who are unsuitable candidates for resection.

Summary: This study aimed to identify characteristics of liver transplantation for elderly patients and assess the therapeutic benefit derived from liver transplantation over liver resection. The results showed that elderly patients who underwent liver transplantation had similar 5-year survival rates compared to non-elderly recipients. Liver transplantation was associated with a lower risk of death compared to liver resection.
