Maintaining oxygen delivery is crucial to prevent intestinal ischemia in critical ill patients

Background Intestinal ischemia is a common complication with obscure pathophysiology in critically ill patients. Since insufficient delivery of oxygen is discussed, we investigated the influence of oxygen delivery, hemoglobin, arterial oxygen saturation, cardiac index and the systemic vascular resistance index on the development of intestinal ischemia. Furthermore, we evaluated the predictive power of elevated lactate levels for the diagnosis of intestinal ischemia. Methods In a retrospective case-control study data (mean oxygen delivery, minimum oxygen delivery, systemic vascular resistance index) of critical ill patients from 02/2009-07/2017 were analyzed using a proportional hazard model. General model fit and linearity were tested by likelihood ratio tests. The components of oxygen delivery (hemoglobin, arterial oxygen saturation and cardiac index) were individually tested in models. Results 59 out of 874 patients developed intestinal ischemia. A mean oxygen delivery less than 250ml/min/m(2) (LRT vs. null model: p = 0.018; LRT for non-linearity: p = 0.012) as well as a minimum oxygen delivery less than 400ml/min/m(2) (LRT vs null model: p = 0.016; LRT for linearity: p = 0.019) were associated with increased risk of the development of intestinal ischemia. We found no significant influence of hemoglobin, arterial oxygen saturation, cardiac index or systemic vascular resistance index. Receiver operating characteristics analysis for elevated lactate levels, pH, CO2 and central venous saturation was poor with an area under the receiver operating characteristic of 0.5324, 0.52, 0.6017 and 0.6786. Conclusion There was a significant correlation for mean and minimum oxygen delivery with the incidence of intestinal ischemia for values below 250ml/min/m(2) respectively 400ml/min/m(2). Neither hemoglobin, arterial oxygen saturation, cardiac index, systemic vascular resistance index nor elevated lactate levels could be identified as individual risk factors.

This study collected ground truth labels for sepsis to address the issue of defining sepsis. The findings showed that SIRS ranked higher than sepsis in assessing disease severity, and the accuracy of Sepsis-1/2 and Sepsis-3 in detecting sepsis onset was questioned.

Summary: Data on sepsis in patients with a subarachnoid hemorrhage are limited. This study evaluated the impact of different sepsis criteria on the outcome in a cohort of SAH patients. The findings suggest that the modified Sepsis-3 criteria and alternative sepsis criteria may provide more accurate diagnosis of sepsis in SAH patients and are associated with prognosis.

Individualized Positive End-expiratory Pressure Titration Strategies in Superobese Patients Undergoing Laparoscopic Surgery: Prospective and Nonrandomized Crossover Study

Summary: This study compared the end-expiratory transpulmonary pressure between fixed and individualized positive end-expiratory pressure (PEEP) strategies in superobese patients and analyzed their effects on respiratory mechanics, end-expiratory lung volume, gas exchange, and hemodynamic parameters. The study found that individualized PEEP strategy resulted in lower end-expiratory transpulmonary pressure while improving respiratory mechanics, lung volume, and oxygenation. Therefore, individualized PEEP strategy may be a feasible approach for respiratory support in superobese patients undergoing laparoscopic surgery.

The Outcome Relevance of Pre-ECMO Liver Impairment in Adults with Acute Respiratory Distress Syndrome

Summary: We investigated the association between pre-ECMO liver impairment, V-VA ECMO support, and mortality in patients undergoing ECMO treatment. We found that a higher pre-ECMO MELD score independently predicts mortality, and V-VA ECMO is associated with a higher mortality risk in patients with a pre-ECMO MELD score >16. These findings suggest the importance of assessing liver function and considering the type of ECMO support in predicting and managing mortality risk.

Bacterial Co- or Superinfection in Patients Treated in Intensive Care Unit with COVID-19- and Influenza-Associated Pneumonia

Summary: Viral pneumonia frequently leads to bacterial co- or superinfection, which has negative effects on patients' outcomes. However, the incidence and impact of co- or superinfection might vary depending on the type of viral pneumonia. In this retrospective study, we found that the incidence of bacterial co- and superinfection did not differ significantly between COVID-19 pneumonia and influenza pneumonia patients. Furthermore, co- or superinfection was not an independent predictor of mortality in patients with high disease severity. However, there was a significantly higher probability of superinfection in COVID-19 pneumonia patients compared to influenza pneumonia patients.

Summary: Results from an extended observation of a randomized trial on COVID-19 convalescent plasma treatment showed a trend towards better outcomes in the CCP group compared to the control group, with significantly improved long-term survival seen in patients receiving higher levels of neutralizing antibodies. The study also found no significant difference in the increase of neutralizing antibodies after vaccination between the two groups.

Summary: The study demonstrates that prone positioning combined with appropriate PEEP levels can increase transpulmonary pressures, improve oxygenation, and stabilize hemodynamics in patients with moderate to severe ARDS. Using a lower PEEP strategy (PEEPARDSNetwork) in conjunction with prone positioning may be an effective lung protective ventilation strategy for these patients.
