Gender Differences in Diabetic Kidney Disease: Focus on Hormonal, Genetic and Clinical Factors

Gender differences have been found in the incidence, phenotypes, and clinical manifestations of diabetic kidney disease (DKD), with hormonal factors, especially estrogen loss, playing a significant role in explaining these differences. Additional research on the impact of sex chromosomes and gene-sex interactions with susceptibility genes for DKD is ongoing.
Diabetic kidney disease (DKD) is one of the most serious complications of both type 1 (T1DM) and type 2 diabetes mellitus (T2DM). Current guidelines recommend a personalized approach in order to reduce the burden of DM and its complications. Recognizing sex and gender- differences in medicine is considered one of the first steps toward personalized medicine, but the gender issue in DM has been scarcely explored so far. Gender differences have been reported in the incidence and the prevalence of DKD, in its phenotypes and clinical manifestations, as well as in several risk factors, with a different impact in the two genders. Hormonal factors, especially estrogen loss, play a significant role in explaining these differences. Additionally, the impact of sex chromosomes as well as the influence of gene-sex interactions with several susceptibility genes for DKD have been investigated. In spite of the increasing evidence that sex and gender should be included in the evaluation of DKD, several open issues remain uncovered, including the potentially different effects of newly recommended drugs, such as SGLT2i and GLP1Ras. This narrative review explored current evidence on sex/gender differences in DKD, taking into account hormonal, genetic and clinical factors.

Pimozide and pancreatic cancer in diabetic chorea: a case report

Summary: This case report describes a 70-year-old female patient with diabetic chorea, who benefitted from pimozide treatment but later died of pancreatic cancer. The patient initially presented with symptoms of choreic movements and high blood sugar, and despite various treatments, only pimozide proved effective in managing her symptoms. The diagnosis of pancreatic cancer several months after the onset of chorea suggests a potential causal link between the two conditions.

Summary: This study investigated the prevalence of obesity and severe obesity, clinical correlates, and potential sex differences in a large cohort of T1D subjects in Italy. The results showed that obesity was similar between men and women, but severe obesity was more prevalent among women. Obesity was associated with a higher burden of cardiovascular disease risk factors and complications, and a lower quality of care, with no major sex differences.

Summary: Genome-wide association analyses have identified 30 risk loci for IgA nephropathy, a progressive form of kidney disease. Functional annotations of potential causal genes suggest inflammatory signaling pathways and cytokine ligand-receptor pairs as potential new drug targets. These findings provide valuable insights into the genetic basis of IgA nephropathy and offer potential avenues for developing therapeutic interventions.

Summary: This study aimed to evaluate the occurrence and severity of liver fibrosis in type 2 diabetes outpatients without a known history of liver disease, and to identify predictive factors. The results showed that obesity, high triglycerides, worse glycemic control, and high creatinine levels were independent predictors of liver fibrosis in type 2 diabetes patients.

Summary: Immune-oncology has become a new therapeutic weapon in the past decade, revolutionizing the natural history of many cancers. Immune checkpoint inhibitors have changed outcomes for many patients, but can also cause inflammatory side-effects known as immune-related adverse events (irAEs). Renal toxicity from immune checkpoint inhibitors is rare, but early diagnosis and treatment are crucial to avoid toxicities and hospitalizations. Enhancing immune responses is a new challenge in anticancer therapies, and immune-related adverse events are common. This review provides a practical approach to the multidisciplinary management of cancer patients with renal toxicity during treatment with immune checkpoint inhibitors.

Summary: In recent years, there has been increasing reports on gender differences in the epidemiology, pathophysiological mechanisms and clinical features of chronic liver diseases associated with type 2 diabetes (T2D). This sexual dimorphism is influenced by a complex interaction between sex- and gender-related factors. However, the impact of sex and gender on the management of T2D subjects with liver disease is still unclear. Therefore, it is important to consider sex-related differences in pharmacology in order to improve drug safety and optimize medical therapy in both men and women with T2D.

Summary: Hyperphosphatemia is a common complication in advanced chronic kidney disease and can lead to cardiovascular morbidity and mortality. This review focuses on the management of phosphatemia in uremic patients receiving peritoneal dialysis. The control of serum phosphate in peritoneal dialysis patients is influenced by various factors such as patient transporter characteristics, peritoneal dialysis modalities, prescriptions, and diet. There is a need for more clinical trials specifically designed for peritoneal dialysis patients to evaluate the effects of multiple therapeutic approaches on serum phosphate control and clinical outcomes.
