Mortality in children with positive SARS-CoV-2 polymerase chain reaction test: Lessons learned from a tertiary referral hospital in Indonesia

A study in Indonesia revealed a high mortality rate in paediatric patients with positive SARS-CoV-2 polymerase chain reaction test, with older patients and those with severe and chronic conditions being at higher risk. Acute respiratory distress syndrome and septic shock were the most common causes of death in fatal COVID-19 cases.
Background: The incidence of coronavirus disease 2019 (COVID-19) is still increasing rapidly, but little is known about the prevalence and characteristics of fatal cases in children in Indonesia. This study aimed to describe the characteristics of children with COVID-19 with fatal outcomes in a tertiary referral hospital in Indonesia. Methods: This cross-sectional study used data collected from the medical records of patients with COVID-19 admitted to Dr. Cipto Mangunkusumo Hospital, Jakarta, Indonesia from March to October 2020. Results: During the study period, 490 patients were admitted and diagnosed with suspected and probable COVID-19. Of these patients, 50 (10.2%) were confirmed to have COVID-19, and 20 (40%) had a fatal outcome. The fatality rate was higher in patients aged >= 10 years, categorized with severe disease upon admission, PaO2/FiO(2) ratio <= 300 mmHg and chronic underlying diseases. The most common clinical manifestations were generalized symptoms, while acute respiratory distress syndrome (8/20) and septic shock (7/20) were the two most common causes of death. Increased procalcitonin, D-dimer, lactate dehydrogenase and presepsin levels were found in all fatal cases. One patient met the criteria of multisystem inflammatory syndrome in children. Conclusion: Our work highlights the high mortality rate in paediatric patients with positive SARS-CoV-2 polymerase chain reaction test. These findings might be related to or co-incided with COVID-19 infection. Further studies are needed to improve understanding of the role of severe acute respiratory syndrome coronavirus-2 in elaborating the mechanisms leading to death in children with comorbidities. Summary: This study aimed to describe the characteristics of patients with MIS-C in a tertiary referral hospital in Indonesia. The study found that the number of MIS-C cases in Indonesia is relatively low. Most patients required mechanical ventilation and intubation, and inflammatory markers and coagulation factor parameters were elevated. Survivors had a shorter time to MIS-C diagnosis, longer hospital stays, and a different treatment plan compared to non-survivors.

Warning Signs in Predicting Severe Pediatric Dengue Infection

Summary: This study aimed to describe the prevalence of vitamin D deficiency in Indonesian children and adolescents. Through meta-analysis, the prevalence of hypovitaminosis D in Indonesia was calculated as 33%, with higher rates in females. Immediate strategies for detecting and treating vitamin D deficiency in Indonesian children and adolescents are recommended.

Serum pentraxin-3 in diagnosing acute appendicitis in patients with acute abdominal pain: A systematic review and meta-analysis of diagnostic test accuracy

Summary: This study examined the diagnostic accuracy of serum Pentraxin-3 in diagnosing acute appendicitis in patients with acute abdominal pain. The results showed that serum Pentraxin-3 had a sensitivity of 90.3% and a specificity of 91.2%. It can be used as a confirmation test for acute appendicitis but not to exclude it.

Summary: Due to high entry barriers, implementing the NBS program in countries, especially those just starting, can be daunting. This review discusses Indonesia's barriers to NBS implementation and their future implications. Relevant literature and grey literature were searched for information. The barriers Indonesia faces in implementing NBS include lack of prevalence data, ethical issues, infrastructure, cost-benefit analysis, logistical issues, government support, patient issues, lack of commitments, and a shortage of healthcare workers and training. Government support, proper infrastructure, and a single-payer system are necessary to accelerate NBS programs in Indonesia.

Pediatric sepsis profile in a tertiary-care hospital in Indonesia: a 4-year retrospective study

Summary: This study aimed to explore the factors contributing to mortality and its management among pediatric sepsis patients at a single center in Indonesia. The results showed that the mortality rate of pediatric sepsis was 76.1% among 176 records with outcome identified. Mortality was significantly associated with septic shock at triage, number of organ failure, intensive care unit admission, inotropic use, septic shock and severe sepsis during hospitalization. Death within the first 24 hours occurred in 41.8% of subjects, mostly due to septic shock.

Emerging Multiple Parasite Reinfection as Neglected Tropical Disease: A Case Series

Summary: The article highlights the endemic problem of soil-transmitted helminth infection in Indonesia, which can lead to reinfection despite personalized therapy and mass treatment. The case series demonstrates the association between multiple helminth infections, chronic bloody diarrhea, and stunted growth in children. Pediatricians are alerted to consider untreated helminthic infections as part of the differential diagnosis in children with gastrointestinal bleeding. Environmental surveillance in high-risk areas is recommended to prevent reinfection and growth alteration. Early routine fecal analysis is advised in endemic areas. Correct diagnosis, prompt treatment, and source control are crucial in preventing long-term complications.
