Summary: This study aimed to assess the clinical efficacy and safety of laparoscopic myomectomy for uterine fibroids. The findings showed that compared to open surgery, laparoscopic myomectomy reduced intraoperative bleeding, shortened operation time and hospital stay, and lowered the incidence of postoperative complications. It also resulted in better clinical outcomes.

Summary: This study analyzed the operative and maternal-fetal outcomes of laparotomic myomectomy in 28 pregnant women with symptomatic uterine fibroids. The procedure was performed between 14 and 16 weeks of pregnancy. The incidence of intraoperative complications was 14.3%, and postoperative complications were observed in 32.1% of cases. Most women (85.7%) delivered at full term, with more than half (54.2%) having vaginal deliveries and normal fetal weights.

Summary: This study compared the cost-effectiveness of different treatment approaches for uterine fibroids (UFs). The results showed that myomectomy was the most cost-effective strategy, while hysterectomy was more expensive and less effective. Furthermore, the study found an increased risk of coronary artery disease associated with hysterectomy.

Summary: The study compared focused ultrasound simulations and treatment results on uterine fibroids, finding that simulations show promise in treatment planning, especially in terms of spatial accuracy. However, patient-specific tissue parameters, particularly the absorption coefficient, are important for predicting temperature rise accurately during sonication.

Summary: This study compared myomectomy with uterine artery embolization (UAE) as treatment options for women who wish to avoid hysterectomy. The results showed that myomectomy resulted in better quality of life at 4 years compared with UAE, but both treatments had similar effects on menstrual bleeding and ovarian reserve.

Summary: The study found that women with abdominal scars had lower non-perfused volume (NPV) during MRgFUS treatment compared to women without scars. However, no serious adverse events were reported in both groups. Linear regression models indicated that fibroids' volume, stopping the treatment due to severe pain, and the presence of abdominal scars had a statistically negative effect on NPV, while the number of sonications had a statistically positive effect on NPV.

Summary: This study examined the rate and risk factors of re-intervention for uterine fibroids (UFs) patients undergoing high-intensity focused ultrasound (HIFU) treatment. The results showed that younger age, T2-weighted magnetic resonance imaging (T2WI) intensity of fibroids, and preoperative anemic status were important factors associated with re-intervention.

Summary: For premenopausal women with symptomatic uterine fibroids, myomectomy is a cost-effective option for treatment. UAE was associated with higher costs and lower QALYs compared to myomectomy, indicating that myomectomy is the preferred choice for these patients.

Summary: This review provides an in-depth summary of the effects on fertility and reproductive outcomes following myomectomy and uterine artery embolization (UAE) for the treatment of symptomatic fibroids. The results show that myomectomy has superior reproductive outcomes compared to UAE, making it the preferred option for women who desire future pregnancy.

Summary: All treatment modalities showed significant improvements in health-related quality of life and symptom severity reduction at 1-year posttreatment. However, abdominal myomectomy, laparoscopic myomectomy, and uterine artery embolization demonstrated a gradual decline in symptom improvement and health-related quality of life by the third year after the procedure.

Summary: This study found that anxiety/depression symptoms are common among women receiving treatment for symptomatic uterine fibroids, but with treatment, there was a significant improvement in symptom severity, and most women experienced a reduction in anxiety/depression symptoms. Summary: Current gaps in knowledge hinder the full implementation of patient-reported outcomes in routine post-operative care. This study evaluated symptoms in patients undergoing open laparotomy using MD Anderson Symptom Inventory (MDASI-PeriOp-GYN). The findings indicate that abdominal bloating, abdominal cramping, and urinary urgency are associated with increased risk of post-operative complications. These results support the integration of patient-reported outcomes assessment in patient-centered post-operative care.

Summary: This study compared the pregnancy outcomes of high-intensity focused ultrasound ablation (HIFU), myomectomy, and uterine artery embolization (UAE) for fertility-sparing patients with uterine fibroids and investigated the possible mechanism of improving pregnancy by HIFU. The results showed that myomectomy had a higher pregnancy rate than HIFU and UAE, while the miscarriage rate of HIFU was similar to myomectomy and UAE. Subgroup analysis suggested that ultrasound-guided HIFU (USgHIFU) may have more ideal pregnancy outcomes than magnetic resonance imaging-guided HIFU. In conclusion, HIFU, especially USgHIFU, may serve as an alternative fertility-sparing modality for patients with uterine fibroids over 40 years old and improve pregnancy rates by elevating uterine blood flow impedance.

The findings showed a significant association between ACE I/D polymorphism and PCOS risk, especially in Caucasian populations. Interestingly, the ACE I/D polymorphism was only correlated with insulin levels and HOMA-IR in Asian populations.
