With the increasing numbers of survivors of cancer in young people, future fertility and ovarian function are important considerations that should be discussed before treatment commences. Some young people, by nature of the treatment they will receive, are at high risk of premature ovarian insufficiency and infertility. For them, ovarian tissue cryopreservation (OTC) is one approach to fertility preservation that remains both invasive and for young patients experimental. There are important ethical and consent issues that need to be explored and accepted before OTC can be considered established in children with cancer. In this review we have discussed a framework for patient selection which has been shown to be effective in identifying those patients at high risk of premature ovarian insufficiency and who can be offered OTC safely. Performance of plasma kisspeptin as a biomarker for miscarriage improves with gestational age during the first trimester

Summary: This study compared the performance of Kisspeptin and beta hCG as biomarkers for miscarriage in the first trimester, finding that Kisspeptin showed better performance, especially in later gestational weeks. The conclusion suggests that Kisspeptin is a promising biomarker for miscarriage, and combining it with beta hCG improves prediction accuracy.

Summary: Our study aimed to evaluate the changes in circulating kisspeptin levels in women with antenatal complications. The results showed that third-trimester kisspeptin levels were higher in women with hypertensive disorders of pregnancy (HDP) but lower in those with fetal growth restriction (FGR). We found that the odds of HDP increased by 30% and the odds of FGR decreased by 28% for every 1 nmol/L increase in plasma kisspeptin.

Summary: This study investigated the value of anti-mullerian hormone (AMH) as a serum biomarker for predicting loss of ovarian function after chemotherapy in premenopausal women with early breast cancer (eBC). The results showed that AMH can accurately diagnose ovarian dysfunction after chemotherapy, and early analysis of AMH can accurately identify women who will not recover ovarian function.

Family size and duration of fertility in female cancer survivors: a population-based analysis

Summary: This study assessed family size and timescale for achieving pregnancy in women who remain fertile after cancer. The data showed a reduced chance of live birth after cancer, and women who subsequently conceived had a higher mean age at live birth and a smaller family size. However, the period of time after cancer diagnosis across which pregnancies occurred was similar or increased.

Survival after breast cancer in women with a subsequent live birth: Influence of age at diagnosis and interval to subsequent pregnancy

Summary: This study found that having a live birth after breast cancer does not have a negative impact on overall survival, especially for younger women who have not been pregnant before. The interaction between subsequent childbirth and survival increases with decreasing age. Women with different ER statuses and those who have received chemotherapy have similar survival rates after subsequent childbirth.

Summary: In vitro culture alters the composition and architecture of the human ovarian cortical extracellular matrix (ECM). The ECM varies in different layers of the ovarian cortex and undergoes remodeling during culture. Mechanical loosening during tissue preparation for culture leads to undefined modifications in the ECM network and alters cell-cell contact, resulting in spontaneous follicle activation.

A predictive model of the effect of therapeutic radiation on the human ovary

Summary: Radiation to the female pelvis as part of cancer treatment can lead to Premature Ovarian Insufficiency (POI) in young women. A model that predicts the age at which POI occurs for a known dose at a known age can aid in counseling patients about their fertility risk. The authors developed an algorithm that combines an updated model of ovarian reserve decline with the estimated median lethal dose for the human ovary to generate personalized predictions of premature ovarian insufficiency. The algorithm is available as an online calculator and can inform discussions about survivor fertility.

Long-term follow-up to assess criteria for ovarian tissue cryopreservation for fertility preservation in young women and girls with cancer

Summary: The Edinburgh Selection Criteria accurately identify female cancer patients under the age of 18 who are at risk of premature ovarian insufficiency (POI) and can be candidates for ovarian tissue cryopreservation (OTC). Long-term follow-up is necessary to ensure the appropriate use of OTC, as it is not without risk.

Summary: This narrative review summarized existing literature on reproductive function in young Hodgkin lymphoma survivors. The results showed that female survivors had a high incidence of premature ovarian insufficiency, while male survivors often experienced post-treatment azoospermia. Higher doses of chemotherapeutic agents and pelvic radiotherapy were associated with impaired reproductive ability.
