Summary: This study aimed to assess the effects of one-year dienogest therapy on the clinical picture, pain patterns, psycho-emotional status, and quality-of-life indicators in adolescents with endometriosis after surgical treatment. Results showed that dienogest therapy significantly alleviated clinical symptoms, improved quality of life, reduced pain symptoms, and showed good tolerability and compliance.

Summary: Chronic pelvic pain is commonly associated with additional pathologies such as adenomyosis and endometriosis. A retrospective study found that 40% of patients with associated pathologies did not experience relief from pelvic pain at the early follow-up and 32% at the 12-month follow-up after laparoscopic niche repair. Therefore, careful patient selection is important for uterine scar repair in patients with concomitant adenomyosis and endometriosis.

Summary: The study aimed to quantify postoperative opioid use after laparoscopic surgery for endometriosis or pelvic pain, and identify patient characteristics associated with greater postoperative opioid requirements. Results showed that most patients had a lower postoperative opioid requirement than prescribed, suggesting surgeons can prescribe fewer postoperative narcotics in this population. Patients with a previous surgery for pelvic pain, self-reported chronic pelvic pain syndrome, anxiety, and depression may represent a subset of patients with increased postoperative opioid requirements.

Summary: This review synthesized systematic reviews and meta-analyses published in the last 5 years to explore the association between endometriosis and adenomyosis with reproductive and obstetric outcomes. The findings suggest that women with endometriosis and adenomyosis have reduced pregnancy and live birth rates, increased miscarriage rates, and higher risks of various adverse obstetric outcomes. Women with deep infiltrating endometriosis and severe adenomyosis should be considered at high obstetric risk and referred to tertiary care centers for safe management during pregnancy and delivery. It is uncertain whether the same applies to pregnant women with minimal endometriosis and adenomyosis forms. Emerging evidence suggests that some adverse outcomes observed in women with endometriosis are associated with coexisting adenomyosis.

Summary: According to epidemiological data, the incidence of endometriosis increases rapidly around the age of 25. The delay in diagnosis is usually between 5 and 8 years in adult women and over 10 years in adolescents. Ovulation and menstruation, which are inflammatory events, may contribute to the early development of endometriosis and adenomyosis. Therefore, secondary prevention targeting severely symptomatic adolescents is crucial for reducing suffering, limiting lesion progression, and preserving future reproductive potential.
