Analysis of risk factors for donation after circulatory death kidney transplantation in Japan

In Japan, outcomes of donation after circulatory death kidney transplantations showed similar patient survival and graft survival rates compared to donation after brain death kidney transplantations. However, older donors and prolonged cold ischemia time were identified as risk factors for graft loss in the donation after circulatory death group. Proper evaluation of donors and careful preparation for transplant surgery are essential for good transplant outcomes.
Background In Japan, donations after circulatory death kidney transplantation are widely performed due to legislation delays. The number of donations after brain death kidney transplantations is increasing, but the target remains unmet. We reviewed the outcomes of donation after circulatory death in Japan. Methods We analyzed 2923 deceased kidney transplantations (2239: donation after circulatory death (DCD), 684: donation after brain death (DBD)) performed in Japan from 2000 to 2019. The outcomes of the DCD and DBD groups were compared. We examined the risk factors for graft loss in the DCD group. Results The 5-year patient survival and death-censored graft survival rates of the DCD group, obtained by propensity score matching, were 93.6% and 95.2%, respectively, which were equivalent to 94.2% and 93.8%, respectively, obtained in the DBD group. Older donors (>= 50 years) and prolonged cold ischemia time (>= 12 h) were risk factors for graft loss; in the presence of these, graft survival was lower in the DCD group. Conclusions Older donors and prolonged cold ischemia time reduced graft survival in the DCD group. Proper evaluation of donors and careful preparation for transplant surgery are, therefore, essential to ensure good transplant outcomes.

Outcomes of pancreas transplantation in older versus younger recipients: a comparative analysis

Summary: The feasibility of pancreas transplantation (PT) in older recipients remains controversial, as studies have shown that older recipients have significantly lower patient survival rates and are more prone to death from infection, multiple organ failure, and cardiac/cerebrovascular events compared to younger recipients. A longer duration of diabetes was identified as an independent risk factor for 1-year mortality in older recipients. Therefore, caution should be exercised when considering PT for older recipients with a long history of diabetes.

Analysis of Factors Associated with Charcot Neuroarthropathy following Pancreatic Transplantation

Summary: Charcot neuroarthropathy (CN) is a progressive complication of diabetes mellitus that can occur in patients who have undergone pancreatic transplantation. In this study, factors associated with CN in pancreatic transplant patients were investigated. Analysis of data from 61 patients revealed that six developed CN, with cumulative incidence rates increasing over time. Severe sensory neuropathy and absence of sural nerve waveforms were observed in patients with CN. The administration of high-dose corticosteroids significantly increased the incidence of CN in patients without sural nerve waveforms. Appropriate therapeutic intervention is necessary due to the association between corticosteroid administration and bone metabolism.
