Summary: Obsessive-compulsive disorder affects many people and causes a lot of suffering. Diagnosis is often delayed and treatments often do not follow evidence-based guidelines. Specialty standards have been developed to emphasize the importance of integrating pharmacological treatment with clear diagnosis and evidence-based cognitive behavioral therapy.

Summary: Almost 25% of schizophrenia patients have comorbid obsessive-compulsive symptoms (OCS), which can complicate treatment and affect therapy response. The neurobiological underpinnings of this comorbidity are still unclear, but understanding them is clinically relevant.

Summary: This study aimed to evaluate the efficacy and safety of agomelatine augmentation of sertraline in the treatment of patients with moderate to severe OCD. The results showed that while agomelatine did not improve OCD symptoms, the combination treatment was safe and effective compared to placebo.

Summary: Acute angle closure (AAC) is a rare but serious ophthalmological condition, often associated with the use of selective serotonin reuptake inhibitors (SSRIs). AAC in individuals receiving SSRIs is mainly caused by pupillary block mechanism, but may also result from other mechanisms such as uveal effusion.

Summary: The systematic review and meta-analysis found that SSRIs and SNRIs as adjuncts for postoperative pain management significantly reduced postoperative pain, opioid consumption, and improved patient satisfaction without increasing adverse events compared with placebo. SSRIs tend to be less effective than SNRIs, but the effects of SSRIs remain inconclusive due to limited evidence.

Summary: Obsessive-compulsive symptoms (OCS) commonly occur in patients treated with clozapine and can significantly affect subjective wellbeing. This study found that OCS severity had a negative impact on wellbeing scores, independent of depressive and psychotic symptoms. However, OCS did not impair general functioning. Obsessional thinking and hoarding behavior, rather than compulsions, were specifically associated with decreased wellbeing, possibly due to the nature of the compulsions.

Summary: This study aimed to summarize the optimal dose and dose-response relationship of SRIs in the treatment of OCD. The findings indicated that the optimal dose for efficacy was around 40mg fluoxetine equivalent, while tolerability decreased with increased doses. There was no significant correlation between acceptability and doses of SRIs.

Summary: This network meta-analysis evaluates and compares the effects of different pharmacological agents as augmentation agents for serotonin reuptake inhibitors (SRIs) in obsessive-compulsive disorder (OCD). The results show that memantine is the most effective augmentation agent for SRIs in OCD, followed by lamotrigine, ondansetron, and granisetron, with moderate certainty of evidence. Symptom reduction is greater in SRI-refractory OCD patients compared to non-refractory OCD.

Summary: The study showed that augmentation of SRIs with olanzapine or aripiprazole significantly improved symptoms in resistant OCD patients, while L-methyl folate was not effective. This suggests that olanzapine and aripiprazole could be promising options for treatment-resistant OCD, while L-methyl folate may not be suitable for this condition.

Summary: This study aimed to investigate the clinical risk factors, phenomenology, and impact of clozapine-induced obsessive-compulsive symptoms (OCS) in patients with schizophrenia. It found that a significant proportion of patients receiving clozapine treatment developed OCS, and the severity of OCS was associated with clozapine and norclozapine plasma levels. There were differences in the types of compulsions between clozapine-induced OCS group and those with a prior history of OCS.

Summary: The rapid spread of COVID-19 requires urgent drug treatments, especially in vulnerable individuals. Prophylactic vaccines and neutralizing monoclonal antibodies may not be fully effective against emerging variants. Selective serotonin reuptake inhibitors (SSRIs) and other antidepressants have emerged as potential treatments, but should be used with caution.

Summary: This study aimed to investigate the association between serum lipid levels, neutrophil/lymphocyte ratio (NLR), and violent suicide attempts in patients with mental disorders. The results showed that patients who used a violent suicide method had significantly lower levels of total cholesterol, LDL, VLDL, and NLR compared to those who used a nonviolent method.

Summary: The aim of this study was to examine the factors associated with duration of untreated illness (DUI) in borderline personality disorder (BPD). A chart review of 152 patients followed up by community psychiatry services in Milan and Monza, Italy was conducted. Pearson correlation and analyses of variances were used to analyze the association between DUI and socioeconomic and clinical variables. The results showed that a longer DUI was associated with several variables, but these associations were not maintained in the regression analyses. Therefore, DUI does not appear to be significantly associated with specific clinical aspects of BPD or the course and outcome of the disorder. Larger studies are needed to confirm these findings.

Summary: The aim of this study was to summarize available data on the link between air pollution exposure and the onset and severity of psychiatric disorders in pregnant women during the perinatal period. Nine studies were included in the analysis, with most focusing on the association between air pollutant exposure and postpartum depression (PPD) onset. The results showed a significant association between PM2.5 and NO2 exposure and PPD onset, as well as an association between PBDE blood levels and more severe depressive symptoms. One study also observed a link between exposure to PM2.5, PM10 during pregnancy and stressful symptoms.

Summary: Generalized Anxiety Disorder (GAD) and Panic Disorder (PD) share similar neurobiological mechanisms and clinical features, leading to potential misdiagnosis and delayed treatment. This study aimed to assess the relationship between clinical and socio-demographic markers and GAD/PD diagnosis. Out of 290 outpatients identified with PD or GAD in mental health services in Italy, post-onset psychiatric and medical comorbidities were associated with GAD, while treatment with selective serotonin reuptake inhibitors (SSRIs) was associated with PD. GAD patients had a worse prognosis due to higher comorbidity rates, relapses, and poorer treatment compliance. The association between GAD and psychodynamic psychotherapy was a unique finding of this study.

Summary: The aim of this systematic review was to examine the impact of the COVID-19 pandemic on binge eating disorder, including new onset and clinical changes. Twelve studies with 4326 participants were included, and the findings suggest an increase in BED diagnoses and worsening of symptoms during the pandemic. However, the quality of the included studies was weak to moderate, and there was high heterogeneity among the studies, indicating a need for further research to validate these findings and develop preventive strategies and personalized treatments.

Summary: Social anxiety disorder (SAD) is a common psychiatric condition that requires prompt treatment to prevent psychiatric comorbidity and impairment in social and occupational functioning. This study systematically reviewed the literature on potential biomarkers for SAD. While some preliminary evidence suggests the role of certain genes and neuroimaging findings in SAD, the results overall remain inconsistent and need further investigation on larger samples to identify more specific biomarkers.

Summary: TRD patients were older, had longer illness duration, were more often treated with psychiatric poly-therapy, had more severe depressive symptoms, had lower bilirubin levels, and had more history of suicide attempts and antidepressant treatments. Candidate biomarkers such as bilirubin levels and NLR were associated with TRD, indicating a predisposition to suicidal behaviors.
