Summary: This meta-analysis found that mastectomy with immediate reconstruction does not affect postoperative survival rates in breast cancer patients who have completed neoadjuvant chemotherapy. However, it may slightly increase the risk of short-term surgical complications.

Summary: This study compared the outcomes of submuscular versus prepectoral implant placement in conservative mastectomy with immediate prosthetic breast reconstruction. The results showed that prepectoral implant placement is a safe and effective alternative with shorter operative time and fewer complications related to manipulation of the pectoralis major muscle.

Summary: This study conducted a meta-analysis of previously published studies and found that neoadjuvant chemotherapy (NACT) does not significantly impact the complication rate after breast reconstruction in breast cancer patients. In terms of minor complications, single-stage direct-to-implant (DTI) appears to be a safer procedure than two-staged tissue expander/implant (TE/I).

Summary: The oncological safety of different types and timings of postmastectomy breast reconstruction (PMBR) remains controversial. This study found that delayed autologous PMBR leads to similar (loco)regional recurrence rates as immediate autologous PMBR. Current evidence suggests that oncological concerns should not be a reason to withhold patients from certain reconstructive timings or techniques, and patients should be offered all technically qualified reconstructive options equally.

Summary: Immediate breast reconstruction (IBR) following mastectomy is a safe option for patients with locally advanced breast cancer, with higher overall survival compared to mastectomy alone. However, it does not have significant impact on disease-specific survival, disease-free survival, local recurrence, adjuvant therapy use, or reoperation rates.

Summary: This study evaluated the factors predicting locoregional recurrence (LRR) in breast cancer patients who underwent neoadjuvant chemotherapy (NACT) and immediate breast reconstruction (IBR). Patients with post-NACT Ki67 >= 10%, high tumor grade, and presence of lymphovascular invasion (LVI) were found to have an increased risk of developing LRR. High LRR rates were observed in patients with all three associated risk factors compared to those with none or fewer factors.

Summary: This article introduces the definition and impact of lymphedema, and proposes a treatment method called ILR, which can reduce the risk of lymphedema through prophylactic lymphovenous anastomosis. Lymphedema has significant effects on the quality of life and morbidity of patients, and ILR may be an effective prophylactic treatment method. Long-term studies are needed in the future to demonstrate the efficacy of ILR.

Summary: The use of immediate breast reconstruction (IBR) after mastectomy has been increasing in all age groups. This study evaluated the age-specific trend of IBR and its impact on work organization at an Italian Breast Unit. The results showed that rates of mastectomy + IBR increased over the years, especially among older women. Furthermore, mastectomy + IBR resulted in longer operative times and hospital stay compared to mastectomy alone.

Summary: A systematic review and meta-analysis of breast reconstruction after mastectomy showed similar event-free survival (EFS) and breast cancer-specific survival (BCSS) rates compared to mastectomy alone, but better overall survival (OS). Breast reconstruction is a feasible and safe treatment option for breast cancer patients.

Summary: This systematic review compared patient-reported outcomes (PROs) and complication rates after nipple-sparing mastectomy (NSM) and skin-sparing mastectomy (SSM). The study found that NSM resulted in higher scores in sexual and psychosocial well-being, with no differences in overall complication rates between the two groups. Therefore, the decision for NSM or SSM should be made on an individual basis, considering factors such as patient preferences and safety.

Summary: This study systematically reviewed the characteristics of perforators in deep inferior epigastric perforator (DIEP) flap breast reconstruction and found that a single dominant perforator and medial row perforators may be associated with a higher risk of fat necrosis. Adopting a standardized perforator selection algorithm could help surgeons make operative decisions and improve postoperative outcomes.

Summary: This study compared the treatments and time intervals between treatments of breast cancer patients diagnosed during and before the COVID-19 pandemic, and the number of treatments started during and before the pandemic. The results showed that patients diagnosed during the transition period were less likely to receive neo-adjuvant chemotherapy for stage I, stage II, and hormone receptor+/HER2- tumors compared to 2018/2019. The time between diagnosis and first treatment decreased for patients diagnosed during the lockdown period. Additionally, there was an increase in the number of neo-adjuvant endocrine therapies and surgeries starting in week 14 of 2020, while the number of adjuvant chemotherapies decreased in week 15 and increased in week 22.

Summary: This article introduces the Areola study, which aims to assess the risks of breast implant illness (BII) and autoimmune diseases in Dutch breast cancer survivors with silicone breast implants. The study design includes a retrospective cohort of breast cancer survivors who underwent implant-based reconstruction surgery between 2000 and 2015 in six major hospitals in the Netherlands. A comparison group of breast cancer survivors without breast implants and a group of women who received breast augmentation surgery will also be included. The study will collect data through a web-based questionnaire and link it with population-based databases to evaluate the prevalence, incidence, and risk factors of BII and autoimmune disorders.

Summary: This study investigated the techniques and opinions of Dutch plastic surgeons and oncological breast surgeons regarding the preservation of pectoral fascia (PF) during mastectomy. The results showed that approximately one in five plastic surgeons and breast surgeons routinely preserve the PF, and even more surgeons preserve the PF in specific cases. There were significant differences in surgical techniques and opinions regarding PF preservation among the surgeons. Future studies should clarify the impact of PF preservation on oncological safety, complication rates, postoperative pain, cosmetic outcomes, and patient satisfaction.

Summary: This study aimed to explore the trends in emotional functioning and psychosocial wellbeing of breast cancer survivors, as well as the demographic and treatment characteristics that may influence these trends. The findings showed a decline in psychosocial wellbeing and a steady improvement in emotional functioning over time. Women who underwent breast reconstruction experienced a more significant improvement in emotional functioning, while women with no partner or children showed a marginal decline in psychosocial wellbeing.

Summary: This study examined the factors influencing plastic surgeons' choice of breast implants and found substantial variation in the weighing of these factors among different surgeons. It also revealed that the choice of breast implants is not solely based on scientific evidence, but brand reputation plays a significant role. Therefore, it is crucial for patients to have comprehensive knowledge about all aspects of breast implants during shared decision making.
