4-IHC classification of breast cancer subtypes in a large cohort of a clinical cancer registry: use in clinical routine for therapeutic decisions and its effect on survival

The aim of the present study was to evaluate to what extent the combination of standard histopathological parameters determines the biology of breast cancer and the effect on therapy and prognosis. The Clinical Cancer Registry Regensburg (Bavaria, Germany) included n = 4,480 female patients with primary, non-metastatic (M0) invasive breast cancer diagnosed between 2000 and 2012. Immuno-histochemical analyses, i.e., estrogen receptor (ER), progesterone receptor (PR), HER2, and Ki-67 (4-IHC), defined the tumor biological subtypes Luminal A, Luminal B, HER2-like, and Basal-like. Subtype-related differences in therapies and overall survival (OS) were analyzed using multivariable statistical methods. 4344 patients (97.0 %) could be classified into the four common tumor biological subtypes. The two most frequent entities were Luminal A (48.4 %), Luminal B (24.8 %), HER2-like (17.8 %), and Basal-like subtype (9.0 %). A multivariable Cox regression model showed that the best 7-year OS was seen in Luminal A patients and that OS of Luminal B and HER2-like patients was comparable (HR = 1.59, P < 0.001 versus HR = 1.51, P = 0.03). Lowest OS was seen in patients with Basal-like tumors (HR = 2.18, P < 0.001). In conclusion, the classification of tumor biological subtypes by the ER, PR, HER2, and Ki-67 biomarkers is practical in routine clinical work. Providing that quality assurance of these markers is ensured, this classification is useful for making therapy decisions in the routine clinical management of breast cancer patients.

Effects of percutaneous coronary intervention on dyspnea in stable coronary artery disease

Summary: This study assessed the impact of percutaneous coronary intervention (PCI) on dyspnea, quality of life, and angina pectoris in patients with stable coronary artery disease (CAD). The results showed that PCI effectively reduced the symptomatic burden of angina pectoris and dyspnea. However, patients with concomitant dyspnea had worse physical limitation scores, angina frequency, and quality of life after PCI.

Expression of pH-Sensitive TRPC4 in Common Skin Tumors

Summary: TRPCs are important in tumor biology, particularly in Ca2+ homeostasis in cancer cells. TRPC4, a pH-sensitive protein, may function as a signaling molecule in the altered tumor microenvironment due to the reversed pH-gradient in solid tumors. The study investigated TRPC4 expression profiles in common skin cancers, revealing that SCC, NCN, and MM show positive TRPC4-expression, while BCC only does so in about half of the analyzed samples. This finding suggests TRPC4 as an immunohistochemical marker to distinguish SCC and BCC and warrants further research on its role in tumor progression and metastasis.

Analysis of long-term mortality after total body irradiation-based and melphalan-based chemotherapy conditioning for acute myeloid leukemia

Summary: This study compared the outcomes of allogeneic hematopoietic stem cell transplantation (allo-HSCT) patients with acute myeloid leukemia who received total body irradiation (TBI)-based conditioning or non-TBI-based conditioning. The results showed that both TBI and non-TBI conditioning had similar effects on non-relapse mortality, relapse incidence, and overall survival. The study suggests that both conditioning methods are equally effective and well-tolerated for eligible AML patients undergoing their first allo-HSCT.

Nanosilver/DCOIT-containing surface coating effectively and constantly reduces microbial load in emergency room surfaces

Summary: This study investigated the efficacy of a newly developed antimicrobial coating containing nanosilver in long-term reduction of bacterial load on hospital surfaces. The results showed that the nanosilver-containing coating significantly reduced bacterial contamination and load. This study demonstrates the effectiveness of nanosilver antimicrobial coating in reducing bacterial colonization on frequently touched surfaces in a clinical setting.

Effects of Individualised High Positive End-Expiratory Pressure and Crystalloid Administration on Postoperative Pulmonary Function in Patients Undergoing Robotic-Assisted Radical Prostatectomy: A Prospective Randomised Single-Blinded Pilot Study

Summary: The aim of this study was to evaluate whether crystalloid administration and individual management of positive end-expiratory pressure (PEEP) improve peri- and post-operative pulmonary function in patients undergoing robotic-assisted laparoscopic prostatectomy (RALP). The results showed that individualized high PEEP levels improved intraoperative blood oxygenation and resulted in more lung-protective ventilation. Furthermore, postoperative pulmonary function was improved for up to 48 h after surgery. Restrictive crystalloid infusion seemed to have no effect on peri- and post-operative oxygenation and pulmonary function.

Isokinetic knee muscle strength comparison after enhanced recovery after surgery (ERAS) versus conventional setup in total knee arthroplasty (TKA): a single blinded prospective randomized study

Summary: The study aimed to compare the isokinetic quadriceps muscle strength after cemented TKA with enhanced recovery after surgery (ERAS) and conventional setup. The ERAS group showed significantly better knee flexion isokinetic muscle strength 5 days postoperatively, but no significant difference was observed between the two groups after 4 weeks. In conclusion, TKA with ERAS improves isokinetic outcomes and patient satisfaction, and isokinetic muscle strength measurement can help modify expectations and improve patient satisfaction.

Summary: This study investigated the diagnostic value of speckle tracking echocardiography in patients with acute myocarditis and normal left ventricular ejection fraction (LVEF). The results showed that patients with acute myocarditis had impaired left ventricular and left atrial function compared to healthy controls, despite preserved LVEF. This suggests that ventricular diastolic dysfunction and elevated LV filling pressures may be present in these patients.
