Melatonin supplementation decreases oxidative stress, and when used concomitantly with vancomycin, it is associated with a significant reduction in vancomycin-related AKI. Further large prospective studies are warranted to validate these findings.
Vancomycin is associated with nephrotoxicity, and the mechanism may in part be related to oxidative stress. In vitro and preclinical studies suggest that melatonin supplementation decreases oxidative stress. The objective of this study was to evaluate concomitant use of melatonin and vancomycin and the incidence of acute kidney injury (AKI). We performed a retrospective cohort study at a large community medical center. All consecutive patients admitted to the medical center between January 2016 and September 2020 who received vancomycin therapy alone or concomitantly with melatonin as part of ordinary care were considered for inclusion. The primary endpoint was the development of AKI, defined as an absolute increase in serum creatinine of >= 0.3 mg/dl or a >50% increase in serum creatinine. All data were analyzed using descriptive statistics. A multivariable logistic regression was constructed to account for potential confounding variables. We identified a total of 303 adult patients meeting inclusion and exclusion criteria treated with vancomycin, 101 of which received melatonin concomitantly. Overall baseline characteristics were similar between the two groups except for the incidence of bacteremia/sepsis. After controlling for the vancomycin area under the curve, baseline creatinine clearance, and intensive care unit admission in a multivariable logistic regression analysis, melatonin use was associated with a 63% decrease in AKI (odds ratio [OK 0.37; 95% confidence interval [CI], 0.14 to 0.96; P = 0.041). Melatonin use was associated with a significant reduction in vancomycin-related AKI. Although this was a retrospective study with a small sample size, given the magnitude of the difference seen, further large prospective studies are warranted.

Acid Suppression Medications During Hospitalization as a Risk Factor for Recurrence of Clostridioides difficile Infection: Systematic Review and Meta-analysis

Palna Mehta, Ronald G. Nahass, Luigi Brunetti

Summary: A systematic review and meta-analysis found that the use of acid-suppression medications (ASMs) during hospitalization was associated with a higher risk of recurrent Clostridioides difficile infection (CDI), especially with proton pump inhibitor (PPI) use. Further studies are needed to clarify the iatrogenic effects of ASMs, and unnecessary PPI use should be discontinued.

Treatment of Acute Gout Flares in the Emergency Department: Prescribing Patterns and Revisit Rates

Luigi Brunetti, Janaki Vekaria, Peter E. Lipsky, Naomi Schlesinger

Summary: This study aimed to investigate pharmacotherapy use related to gout treatment in U.S. emergency department patients, finding that roughly 30% of patients did not receive anti-inflammatory medication upon discharge and opioids were frequently overused in gout management in the ED. There is an opportunity for further education of healthcare providers regarding gout treatment.

Classification and Assessment of Medication Risk in the Elderly (CARE): Use of a Medication Risk Score to Inform Patients' Readmission Likelihood after Hospital Discharge

Summary: The study introduced a novel medication risk score (MRS) to evaluate medication regimens of older adults and found that only a small percentage of patients met the criteria, indicating potential for future use on high-risk patients. The MRS was associated with an increased risk of readmission for patients discharged home, highlighting its potential utility in identifying patients at high risk of medication-related iatrogenesis.

Summary: This retrospective cohort study evaluated the safety and efficacy of different doses of cefazolin for the treatment of infections in obese patients. The results showed that high-dose cefazolin was safe and well tolerated in patients with high body weight. However, larger studies are needed to confirm the benefit of this dosing strategy.

Summary: This meta-analysis compares the impact of AUC-guided dosing strategies with trough-based monitoring on vancomycin-induced nephrotoxicity. The results show that AUC-guided dosing strategies are associated with a significantly lower incidence of kidney injury compared to trough-guided strategies.

Summary: The effect of obesity on IgG pharmacokinetics was investigated in a rat model and clinical evidence for an association between body composition and IVIG pharmacokinetic parameters in humans was collected. The pharmacokinetics of human IgG was significantly different between obese and lean rats. In the clinical study, total serum IgG concentration and subtype half-life negatively correlated with BMI and fat mass.

Summary: This review evaluates emerging therapies for peanut allergy (PA) and their potential role in treating this condition. Oral, epicutaneous, and sublingual immunotherapies have shown promising efficacy, with Palforzia (AR101) and Viaskin Peanut already approved or expected to be approved. The use of adjuvant therapies with immunotherapy has demonstrated improved efficacy and safety. However, the high rates of adverse reactions associated with immunotherapy indicate a need for further research.
