Behcet's Syndrome is a variable vessel vasculitis that can affect any organ, with main features including oral and genital ulcers, skin lesions, ocular inflammation and arthritis. In Europe, BS appears to have six clinical manifestation phenotypes, such as mucocutaneous, arthritis, vascular, ocular, CNS involvement and gastrointestinal involvement.
Behcet's Syndrome (BS) is a variable vessel vasculitis according to the Chapel Hill Consensus Nomenclature (1) and may thus affect any organ, including major and minor arterial and venous vessels to a varying degree and with varying frequency. Although the main features of BS are recurrent oral and genital aphthous ulcers, cutaneous lesions, ocular inflammation and arthritis-major vessel and life-or organ threatening involvement of internal organs and the central and peripheral nervous system occur. In general, BS in Europe appears to form six phenotypes of clinical manifestations (2), which are (1) mucocutaneous only, (2) predominant arthritis/articular involvement, (3) vascular phenotype, (4) ocular manifestations, which are most likely associated with CNS manifestations and HLA-B51, (5) dominant parenchymal CNS manifestations (being associated with the ocular ones), and (6) gastrointestinal involvement. Mucocutaneous manifestations are present in almost all patients/all phenotypes. In the following review, we summarize the current knowledge concerning vascular, neurologic, gastrointestinal and musculoskeletal manifestations of the disease.

Differential diagnosis of a vasculitic syndrome of the lower limb

Nikolas Ruffer, Martin Krusche, Marie-Therese Holzer, Christian Hagel, Ina Koetter

Summary: Vasculitides can present with diverse clinical signs involving skin, peripheral nerves, and musculature. mPAN is characterized by elevated inflammatory markers, vasculitic skin lesions, active polyneuropathy, and lower limb myalgia. Magnetic resonance imaging and muscle biopsy provide crucial diagnostic information for patients with vasculitic syndromes.

Summary: Cocaine mixed with levamisole can cause osteocartilaginous destruction mimicking granulomatosis with polyangiitis, and can also lead to systemic vasculitis. Early diagnosis and drug cessation are crucial for clinical improvement.

Summary: The DU-VASC model was developed and validated to assist in decision-making for the management of digital ulcers in patients with systemic sclerosis. The model showed that PI treatment was the most important predictor associated with reduced occurrence of digital ulcers.

Summary: This study reveals that male patients with Behcet's disease have a higher genetic risk compared to female patients, primarily within the HLA region. These findings suggest that genetic factors might contribute to differences in disease presentation between men and women with Behcet's disease.

Summary: The prevalence and characteristics of SSc-associated interstitial lung disease (SSc-ILD) vary between geographical regions. This study aimed to explore the differences in prevalence, phenotype, treatment, and prognosis in patients with SSc-ILD from different regions in the EUSTAR database. The results showed significant variations in clinical characteristics, survival rates, and treatment approaches among different regions.

Association of the different forms of uveitis with inflammatory rheumatic diseases and their treatment

Nicole Stuebiger, Sanaz Farrokhi, Yannik Gkanatsas, Christoph Deuter, Ina Koetter

Summary: The etiology of uveitis varies greatly worldwide, with noninfectious causes being more common in industrial nations. In Germany, 44% of uveitis cases are due to systemic diseases, with spondylarthritis, vasculitis, and sarcoidosis being the most common associated conditions in rheumatology. Collaboration between ophthalmologists and rheumatologists is crucial in differentiating uveitis subtypes and determining the optimal treatment options.

Summary: This study utilized the EUSTAR database to assess the safety and effectiveness of Tocilizumab in a real-life setting. Although the results did not show significant effectiveness, the consistency of direction in all predefined endpoints generates a hypothesis for potential effectiveness in a broader SSc population.

Summary: This study aimed to assess the safety and efficacy of HSCT therapy for patients with SSc and compare them with non-HSCT patients. The results showed that HSCT therapy can improve the condition and reduce mortality of SSc patients to some extent. However, when comparing HSCT cohorts with control groups, the distribution of risk factors for mortality should be considered.

New-onset dermatomyositis following SARS-CoV-2 infection and vaccination: a case-based review

Marie-Therese Holzer, Martin Krusche, Nikolas Ruffer, Heinrich Haberstock, Marlene Stephan, Tobias B. Huber, Ina Koetter

Summary: In this study, an increase in new-onset dermatomyositis after SARS-CoV-2 infection or vaccination was observed. The findings suggest that SARS-CoV-2 infection and vaccination may trigger interferon-pathway, leading to the production of dermatomyositis-specific autoantibodies.
