Mortality in incident dementia - results from the German Study on Aging, Cognition, and Dementia in Primary Care Patients

Abstract

Objective: Dementia is known to increase mortality, but the relative loss of life years and contributing factors are not well established. Thus, we aimed to investigate mortality in incident dementia from disease onset. Method: Data were derived from the prospective longitudinal German AgeCoDe study. We used proportional hazards models to assess the impact of sociodemographic and health characteristics on mortality after dementia onset, Kaplan-Meier method for median survival times. Results: Of 3214 subjects at risk, 523 (16.3%) developed incident dementia during a 9-year follow-up period. Median survival time after onset was 3.2 years (95% CI = 2.8-3.7) at a mean age of 85.0 (SD = 4.0) years (>= 2.6 life years lost compared with the general German population). Survival was shorter in older age, males other dementias than Alzheimer's, and in the absence of subjective memory complaints (SMC). Conclusion: Our findings emphasize that dementia substantially shortens life expectancy. Future studies should further investigate the potential impact of SMC on mortality in dementia.

Risk factors for subjective cognitive decline: the CABLE study

Summary: The study identified risk factors for subjective cognitive decline (SCD) in individuals without objective cognitive impairment. Factors such as older age, thyroid diseases, minimal anxiety symptoms, and day time dysfunction were stable risk factors, while female sex, anemia, lack of physical exercises, and living alone were suggestive risk factors for SCD. The prevalence of SCD increased with the clustering of more risk factors in individuals.

Creativity across the lifespan: changes with age and with dementia

Summary: The aim of this study was to investigate the influence of age and cognitive impairment on creativity. The results showed that older individuals scored lower in certain aspects of creativity, particularly in abstract reasoning. Additionally, cognitively impaired older adults had lower scores in creativity compared to cognitively healthy older adults and younger individuals. These findings suggest that while creativity remains relatively stable in older age, abstract reasoning skills may be affected by aging.

Epigenome-wide analysis identifies methylome profiles linked to obsessive-compulsive disorder, disease severity, and treatment response

Summary: Obsessive-compulsive disorder (OCD) is a common mental disorder affecting around 2-3% of the population. This study investigated the epigenetic factors associated with OCD by analyzing DNA methylation patterns in 185 OCD patients and 199 controls. The researchers identified differentially methylated CpG positions and validated their findings using a subset of 12 CpGs. These CpGs were found to be related to genes associated with the sweet-compulsive brain hypothesis, suggesting a potential role of aberrant dopaminergic transmission in OCD pathogenesis.

Social isolation in the oldest-old: determinants and the differential role of family and friends

Summary: This study aimed to explore the association between sociodemographic and health-related factors and social isolation in relation to family and friends among the oldest-old. The findings revealed that factors such as gender, age, functional activities of daily living, depression severity, and cognitive functioning had unequal effects on social isolation. Therefore, the social context should be considered more strongly in the detection and prevention of social isolation to maintain mental and physical health.
