Summary: Among adolescent SUD patients, MDMA use in addition to cannabis use is associated with APS, while cannabis use is not related to APS. Cannabis may increase the risk for psychosis after a longer period of use and in combination with other risk factors, such as trauma history. Clinicians should screen for APS among SUD patients using MDMA and cannabis in order to adapt treatment plans of SUDs.

Summary: Psychotic disorders are unevenly distributed by race in the United States. Hispanic and Black respondents reported higher rates of psychotic experiences, with hallucinations more commonly reported by the Hispanic respondents. These racial disparities in psychotic experiences are explained by socioenvironmental risk factors such as income, education, urban/rural living, discrimination, and trauma exposure.

Summary: Based on the assumption of a universal neurodevelopmental model of psychosis, the diagnosis and treatment of psychosis in minors usually follow those in adults. However, recent studies have shown that developmental aspects may play a crucial role in the prevalence and appraisal of relevant symptoms in minors, including genetic and other risk factors. Therefore, caution is advised in overpathologizing potentially transient and clinically irrelevant symptoms in the diagnosis and treatment of psychotic disorders in minors. More studies on developmental aspects are urgently needed.

Summary: Sleep disturbances are common in individuals at ultra high-risk of psychosis and have been found to play a causal role in the occurrence of psychotic symptoms. Few studies have systematically investigated sleep disturbances in this population. The results suggest that high-risk individuals have difficulties waking up and feel exhausted upon awakening, indicating a delayed sleep phase.

Summary: This study examined the relationship between neural measures of performance monitoring and executive function, symptoms, and functioning in individuals with psychotic disorders. The results showed that reduced error-related negativity and error positivity were associated with impaired executive function, negative symptom severity, and poor real-world functioning in the clinical cohort. Multiple potential pathways were identified, suggesting a transdiagnostic model of psychotic disorders where poor performance monitoring contributes to impaired executive function, negative symptoms, and poor functioning.

Summary: The study found that the diagnostic inter-rater reliability of the DSM-5-APS among child/adolescent mental health practitioners was consistent with the results from the DSM-5 field trials. Almost three in four practitioners endorsed psychopharmacological intervention as a treatment option for the DSM-5-APS case. Therefore, there is a need to raise awareness regarding the CHR-P paradigm and its treatment, as well as the development of solid guidelines for implementation in clinical practice.

Summary: Pontocerebellar hypoplasias (PCH) are a group of genetically heterogeneous disorders, with CASK being the major causative gene in Italy. The most common genetic causes include CASK, TSEN54, and EXOSC3, with a clear association observed between CASK and specific phenotypic clusters. Variability in phenotypes associated with the most common genetic causes of PCH is wider than previously thought, with marked overlap between CASK and TSEN54-associated disorders.

Summary: Joubert syndrome (JS) is a recessively inherited ciliopathy characterized by specific cerebellar and brainstem malformation. Clinical signs include hypotonia, developmental delay, and ocular motor apraxia, and genetic diagnosis is usually based on next-generation sequencing targeted gene panels. However, the accuracy of diagnostic referral based on clinical and imaging assessment is crucial, as misdiagnosis or missed diagnosis may occur. Whole-exome sequencing (WES) may be more advantageous in cases where diagnostic suspicion is not straightforward.

Summary: Heterozygous truncating or splice-site SUFU variants cause a novel neurodevelopmental syndrome encompassing COMA and mild JS, likely representing overlapping entities. These variants can be de novo or inherited from a healthy parent, making it the first cause of JS with dominant inheritance and reduced penetrance. Awareness of this condition will increase the diagnostic yield of JS genetic testing and allow appropriate counseling about prognosis, medical monitoring, and recurrence risk.

Summary: The aim of this study was to investigate the characteristics and prognosis of elderly patients with new-onset focal epilepsy and the role of leukoaraiosis. The results showed that patients with unknown cause of epilepsy had a higher frequency of seizures with impaired awareness and focal to bilateral tonic-clonic seizures during sleep compared to those with structural cause of epilepsy. Patients with unknown cause of epilepsy also showed a preferential temporal lobe localization of interictal epileptiform discharges and increased activation during deep non-rapid eye movement sleep. The overall prognosis was good, with higher seizure freedom rates in patients with unknown cause of epilepsy compared to those with structural cause of epilepsy. However, the presence of leukoaraiosis worsened the prognosis.

Summary: Cerebral Visual Impairment (CVI) has become a leading cause of visual impairment in children in developed countries. This study aims to describe the clinical and neuropsychological profiles of children with CVI and investigate the impact of visual functions on their neuropsychological performance. The results suggest that oculomotor dysfunction and visual acuity may significantly affect cognitive visual and other neuropsychological abilities in children with CVI. Comprehensive assessment of both visual and neuropsychological functions is important for diagnosis and habilitation strategies for children with CVI.

Identifying the key personality traits involved in headache disorders is crucial for developing psychological interventions.
