Summary: Older adults evaluated by a community paramedic for urgent needs had more primary care visits, shorter wait times for urgent evaluations, similar acute care use, and lower 30-day readmission rates compared to patients evaluated by a physician home visit. Physician productivity also significantly increased when utilizing the community paramedic model. This study suggests that older adults may benefit from combining emergency medical services and primary care resources for urgent evaluations.

Summary: This study aimed to compare the efficacy and adverse effects of intravenous meperidine and inhaled nitrous oxide for intrapartum analgesia in multiparous patients. The results showed that there was no difference in pain intensity between the two groups 20-30 minutes after administration of the analgesics. Additionally, there were no significant differences in the need for additional analgesia, labor length, delivery mode, maternal and neonatal adverse effects, and patient satisfaction between the groups. Therefore, meperidine and nitrous oxide have similar efficacy and safety in intrapartum analgesia for multiparous patients.

Summary: The IMTAD study compared the reliability, accuracy, and safety of three localization markers (clip + iodine seed, magnetic seed, carbon suspension) for axillary staging in breast cancer patients after neoadjuvant chemotherapy. The study found that there was no significant difference in complication rates during marker implantation and detection between the different markers, but the clip + iodine seed had a higher rate of dislodgement. The study confirmed that the false-negativity rate of targeted lymph nodes was lower than that of sentinel lymph nodes, indicating that targeted lymph nodes are a better marker for axillary lymph node status after neoadjuvant chemotherapy.

The results showed that surgeon-administered transversus abdominis plane block with bupivacaine was not found to be superior to the placebo intervention in reducing postoperative opioid requirements or improving other postoperative outcomes for patients undergoing a midline laparotomy.

Summary: This study aimed to evaluate the effect of adding dexmedetomidine to periarticular infiltration analgesia (PIA) for pain management after total knee arthroplasty (TKA). The results showed that patients in the dexmedetomidine group had lower visual analog scale scores at 6 hours after surgery, and the time until the administration of first rescue analgesia was delayed and opioid consumption was lower in the dexmedetomidine group. Adding dexmedetomidine as an adjuvant to PIA could potentiate and prolong the analgesic effect without increasing the risk of adverse events.

Summary: Effective pain management during the perioperative period is crucial for improving patient experience and satisfaction following surgery. This study compared the use of intravenous and intraperitoneal infusions of lidocaine for postoperative pain management in patients undergoing laparoscopic colon resection. The results showed that the intraperitoneal group had significantly lower opioid consumption compared to the intravenous group. This suggests that targeting the peritoneal cavity with local anesthetic administration is an important component of a multimodal pain strategy following colectomy.

Summary: The efficacy and safety of intranasal etripamil 70 mg nasal spray for conversion of atrioventricular-nodal-dependent paroxysmal supraventricular tachycardia to sinus rhythm within 30 minutes have been evaluated in this study. The results showed that intranasal etripamil was effective in rapidly converting atrioventricular-nodal-dependent paroxysmal supraventricular tachycardia to sinus rhythm and was well tolerated.

Summary: This study examines the use of NEXUS and CSR by EMS and tele-EMS physicians for spinal clearance in trauma patients. The results show that both groups rarely document complete NEXUS or CSR criteria. The decision to immobilize or not was appropriately implemented in both groups, but tele-EMS physicians performed immobilization without indication more often. Tele-EMS physicians showed better guideline adherence in the choice of immobilization device.
