Extensive research has been published, showing the usefulness of angiogenic markers in both diagnosis and subsequent prediction and management of preeclampsia and placenta-related disorders. Recent evidence provides a helpful cut off for the Elecsys ratio sFlt-1 to PlGF, that predicts preeclampsia development in women with sign and symptoms, before its clinical onset in the short term. In Europe, no accordance exists for the use of such kind of test in clinical practice; only German guidelines have recently taken it into account, as a diagnostic aid for preeclampsia, in conjunction with other clinical findings. This panel of Italian experts recently met, in order to review the literature and to promote the evaluation of the clinical utility of Flt-1/PlGF ratio at the Italian country level, as regards: prediction of preeclampsia during the first trimester, prediction or exclusion of new onset or recurrence in patients with risk factors for preeclampsia, triage of patients suffering from gestational hypertension, evaluation of disease severity, prediction of adverse maternal and fetal outcomes. Role of Inflammaging on the Reproductive Function and Pregnancy

Summary: Inflammation and cellular senescence play a role in various physiological processes in females, including ovulation, menstruation, placental homeostasis, and delivery. Factors such as lifestyles, nutrition, environmental insults, and pregestational inflammatory diseases can lead to chronic excessive inflammation and inflammaging, resulting in decreased fertility, poor pregnancy outcomes, impaired placental function, fetal development, and increased future health risks in offspring. Maladaptation to pregnancy and obstetric diseases can contribute to maternal inflammaging, which can further accelerate aging processes and the development of chronic diseases. Maternal inflammaging can also impact future adult health through transgenerational effects. Therefore, effective interventions should be implemented to reduce modifiable factors contributing to the inflammaging process and improve public health.

Predictors of Postpartum Depression among Italian Women: A Longitudinal Study

Summary: Postpartum depression is a common issue for mothers worldwide and is associated with anxiety disorders and parenting stress. This study aimed to describe the psychological status of mothers up to 12 months postpartum and identify predictors of depressive symptoms at 12 months postpartum. The findings highlight the importance of stable programs for mothers in the first year postpartum, and ensuring a positive childbirth experience to reduce potential long-term consequences.

Epigenetic Profiling in the Saliva of Obese Pregnant Women

Summary: This study found that the epigenetic profile in the saliva of obese pregnant women is markedly different from that of normal-weight pregnant women, suggesting the presence of impaired mechanisms and damaged pathways. The characterization of this profile can serve as a promising tool for the identification of obesity-related altered mechanisms and the development of biomarkers for early diagnosis and treatment of pregnancy-adverse conditions.

Summary: This study found a higher prevalence of placental maternal vascular malperfusion in HDP-FGR and early FGR groups, which were associated with abnormal feto-placental Doppler parameters. In contrast, HDP-AGA fetus and late FGR presented more heterogeneous placental lesions not severe enough to cause feto-placental Doppler anomalies. These findings suggest potential preventive nutritional approaches for pregnant women with predisposing factors for HDP-AGA fetuses and late FGR.

Glycation-Driven Inflammation: COVID-19 Severity in Pregnant Women and Perinatal Outcomes

Summary: This study investigated the impact of SARS-CoV-2 infection on pregnant women and its relationship with overweight, obesity, and gestational diabetes. Comprehensive assessment of the implementation of maternal immunization programs in Italy: A national survey across maternity care units

Summary: This study assessed the characteristics and integration of immunization services into prenatal care provided by maternity care units (MCUs) in Italy. The findings showed that most MCUs provided vaccine information, but only 22% had an onsite vaccination clinic. Less than half of the MCUs offered vaccines during prenatal visits, and 75% of them required women to set up an extra appointment for vaccination. Additionally, only 20% of MCUs managed to record vaccines in their own medical records. Institutional barriers and lack of certain vaccine-related services were identified as the main reasons for missed vaccination opportunities. Embedding maternal immunization programs within prenatal care services could improve vaccine coverage.

Maternal Sepsis in Italy: A Prospective, Population-Based Cohort and Nested Case-Control Study

Summary: Italy conducted a population-based study on maternal sepsis occurring before or after childbirth. The study found an incidence rate of 5.5 per 10,000 maternities. Genital, respiratory, and urinary tract infections were the predominant sources of infection, mainly caused by E. coli and polymicrobial infections. It is important to raise awareness of maternal sepsis and its risk factors and management.

Summary: This study aimed to evaluate the sFlt-1/PlGF ratio in pregnancies complicated by COVID-19 and gestational diabetes (GD) and found that patients with both GD and SARS-CoV-2 infection had a higher sFlt-1/PlGF ratio, along with a higher incidence of obesity and hypertensive disorders of pregnancy.

Umbilical vein blood flow: State-of-the-art

Summary: Placental blood supply to the fetus can be measured by evaluating the umbilical vein blood flow. Despite its potential application in healthcare, the umbilical vein blood flow volume is still used only in research setting. One of the reasons is a concern regarding its reproducibility, partly due to technology issues. Nowadays, technology improvements make this evaluation accurate and reproducible. The aim of this review is to refresh basic elements of the physiology of umbilical vein blood flow and its analysis. Its evaluation in normal and abnormal fetal growth is also discussed.

Umbilical Vein Blood Flow in Uncomplicated Pregnancies: Systematic Review of Available Reference Charts and Comparison with a New Cohort

Summary: The objectives of this study were to review the available umbilical vein blood flow volume (UV-Q) reference ranges in uncomplicated pregnancies and compare them with UV-Q values obtained from a local cohort. Literature in English was reviewed according to PRISMA guidelines. Original articles were grouped based on UV sampling sites and formulae used to compute UV-Q. The UV-Q values were derived from formulae or tables presented by authors. A prospective observational study of uncomplicated singleton pregnancies was conducted to compare UV-Q with the systematic review results. Overall, there was a substantial heterogeneity in reported UV-Q central values, but values overlapped when using the same sampling methodology and formulae. The study suggests that UV-Q assessment is accurate and reproducible when adhering to the same methodology, encouraging further investigation on clinical applications.

Characterization of Maternal Circulating MicroRNAs in Obese Pregnancies and Gestational Diabetes Mellitus

Summary: Maternal obesity and gestational diabetes mellitus (GDM) are associated with adverse maternal and foetal outcomes, and involve epigenetic alterations contributing to the pathogenesis of metabolic diseases. This pilot study analyzed plasma microRNAs in obese pregnant women with/without GDM, revealing differentially expressed microRNAs related to oxidative stress, inflammation, nutrients, and hormones metabolism. These findings provide insight into the complex epigenetic network regulating metabolic pathways in MO and GDM, but further investigations are needed.

Immune Response and Transplacental Antibody Transfer in Pregnant Women after COVID-19 Vaccination

Summary: COVID-19 infection is associated with increased risk of pregnancy complications, highlighting the importance of vaccination during pregnancy. This study evaluated the humoral and cell-mediated responses to SARS-CoV-2 immunization in maternal and neonatal plasma. The results showed that most vaccinated pregnant women produced positive anti-S antibodies and had effective placental transfer, but the evaluation of IFN-γ production was inconclusive.

Summary: This study aimed to evaluate the long-term cardiological outcomes of prenatal RVOTAs in monochorionic diamniotic twin pregnancies complicated by TTTS and treated with FLS. The majority of these pregnancies still had CHD at long-term follow-up. The risk of major CHD increased with prenatal evidence of PS and decreased with the gestational age at the time of TTTS and with the prenatal normalization of blood flow across the pulmonary valve.

Performance of fetal ultrasound and magnetic resonance imaging in predicting birthweight according to the test-to-delivery interval: A cohort study

Summary: The objective of this study was to assess the influence of the test-to-delivery interval on the performance of ultrasound and MRI in predicting birthweight. The results showed that MRI performed best in predicting birthweight greater than gestational age if delivery occurred within two weeks of the examination, with a slight decrease thereafter, while ultrasound performance decreased drastically over time.

Induction of labor versus expectant management in patients with idiopathic polyhydramnios

Summary: This retrospective cohort study aimed to evaluate whether induction of labor is associated with a lower risk of cesarean section in patients with isolated polyhydramnios. The study found that planned induction was associated with a lower rate of cesarean delivery compared to expectant management, but the difference was not statistically significant. Additionally, no differences were observed in maternal or fetal secondary outcomes.

A systematic review on idiopathic intracranial hypertension comorbid with polycystic ovarian syndrome and its consequences

Summary: This systematic review examined the comorbidity of idiopathic intracranial hypertension (IIH) and polycystic ovarian syndrome (PCOS) and their impact on metabolism, hormone levels, and reproduction. The findings showed a high prevalence of comorbidity between the two conditions, which may have significant clinical implications for patient management.
