Summary: This study provides evidence on the natural history of non-functioning pituitary microadenomas (micro-NFPAs) and suggests that the probability of micro-NFPA growth is low, and the development of new hypopituitarism is rare. Delaying the first follow-up MRI to 3 years and avoiding hormonal re-evaluation in the absence of tumor growth or clinical manifestations is a safe approach for micro-NFPA surveillance.

Summary: Objective data on pituitary function before and after transsphenoidal surgery in nonfunctioning pituitary adenomas were inconsistent. This study found that adrenocorticotrophic hormone deficiency increased significantly at 1 year postoperatively, while other axis function changes were not significant. Therefore, continuous individual evaluations are needed during longer follow-up of patients operated for NFPA.

Summary: Crooke cell adenomas (CCAs) are rare, potentially aggressive pituitary adenomas. After reviewing 59 cases of functioning corticotroph adenomas and literature on CCAs, it was found that CCAs have a higher prevalence among functioning ACTH adenomas, most being macroadenomas with high levels of adrenocorticotropic hormone. Postoperative outcomes were excellent in our series, but some cases in the literature were refractory to standard treatments.

Analysis of 73 NFPA tumor samples revealed a correlation between immune/stromal scores and cavernous sinus invasion, as well as a significant association between high immune score group and poor recurrence-free survival. The study identified critical genes involved in the tumor microenvironment, suggesting new therapeutic targets for NFPA.

Summary: The aim of this study was to assess the factors influencing the occurrence of delayed hyponatremia after transsphenoidal surgery (TSS) in patients with a non-functional pituitary adenoma (NFPA). The results showed that delayed postoperative hyponatremia was independently associated with larger craniocaudal dimension, preoperative hyperprolactinemia, and larger preoperative pituitary stalk deviation angle.

Summary: Current clinical guidelines may not be accurate for predicting the laterality of adrenal lesions in young patients with primary aldosteronism (PA) based solely on imaging studies. Initial guidelines were based on one study, which included only 6 patients under the age of 35, and since then four additional studies have been published that show discordance between imaging and adrenal venous sampling (AVS) results in patients under 35. These findings challenge the current guidelines and suggest the need for further evaluation.

Summary: This study describes the clinical presentation and treatment outcomes in a nationwide cohort of patients with giant prolactinomas. The results show that dopamine agonists effectively reduced prolactin levels and tumor size, but some patients required additional treatments. The response to dopamine agonists after 1 year can help identify patients who may need more careful monitoring and additional treatment in some cases.

Summary: Patients with adrenal insufficiency have a similar incidence of COVID-19 infection as the general population, but they have a higher risk of hospitalization, intensive care admission, and death. After adjusting for socioeconomic factors and comorbidities, the hazard ratios for hospitalization, intensive care admission, and death were significantly increased in patients with adrenal insufficiency.

Summary: The aim of this study was to establish a valid national cohort of patients diagnosed with acromegaly by combining data from the National Patient Register and the Swedish Pituitary Register. After applying an algorithm and reviewing medical records, a total of 1034 acromegaly cases were confirmed in Sweden, resulting in a higher estimated incidence rate.

Summary: The study found that the content and language of inpatient records mostly focused on symptoms and medical interventions, with little inclusion of patients' preferences, wishes, needs, and shared decision-making in person-centered documentation. In discharge letters, some physicians attempted to write in a person-centered manner, using plain language and narratives, but others still used medical terms and abbreviations reflective of their discourse community. The established norms for documentation in healthcare pose a barrier to achieving person-centered documentation in clinical practice.
