Summary: This article discusses the immune evasion and resistance mechanisms in head and neck squamous cell carcinomas, with a focus on the role of PD-L1 and PD-1 inhibitors in this disease. It also mentions research on predicting patient benefit from immunotherapy and studying resistance mechanisms, along with novel strategies to overcome resistance.

Summary: EGFR is overexpressed in head and neck squamous cell carcinoma and anti-EGFR strategies have shown some clinical benefit but often lead to resistance. Mutations in various domains of the EGFR gene can impact drug binding, radiation response, and overall survival in HNSCC patients. Understanding the EGFR mutational landscape and its effects on treatment resistance can help stratify patients for targeted therapies and maximize therapeutic benefits.

Summary: A systematic review of 15 studies comparing PD-L1 expression before and after conventional therapy in head and neck squamous cell cancer revealed heterogeneous results. However, a quantitative analysis showed a trend towards an increase in PD-L1 expression in tumor cells after platinum-based therapy. Further studies are needed for more robust data.

Summary: Studies suggest a underestimation of PD-L1 expression in biopsies compared to resection specimens, with higher positivity in metastatic lymph nodes. The therapy has conflicting effects on PDL1 expression. The most recent patient material related to the natural history of tumor may be the most reliable for evaluating PD-L1 expression.

Summary: Taxanes, especially docetaxel, are widely used in combination therapy for head and neck squamous cell carcinoma (HNSCC) and have shown advantages in improving patient outcomes. Taxanes not only function as antimitotics, but also impair various oncogenic signaling pathways. However, the development of drug resistance remains a major obstacle to treatment response.

Summary: IL-6 plays a critical role in regulating xCT gene in the carcinogenesis of head and neck squamous cell carcinoma, and its upregulation is associated with ferroptosis resistance. High expression of xCT is correlated with poor prognosis.

Summary: This study investigates the spatial association between the signatures of ferroptosis and inflammation/immune activation in head and neck squamous cell carcinoma (HNSCC). The study finds a stronger association between ferroptosis signature and inflammation/immune activation in HPV-negative HNSCC compared to HPV-positive ones. It is shown that ferroptotic stress induces PD-L1 expression through reactive oxygen species (ROS)-elicited NF-kappa B signaling pathway and calcium influx. Pre-treatment of murine HNSCC with ferroptosis inducers enhances the sensitivity of tumors to anti-PD-L1 antibody treatment.

Summary: In this study, a prognostic multilncRNA signature based on ferroptosis-related differentially expressed lncRNAs in HNSCC was constructed and found to impact the prognosis of HNSCC, outperforming traditional clinicopathological features in predicting HNSCC prognosis. The signature was associated with poor prognosis of HNSCC, with a higher AUC value. GSEA identified immune and tumor-related pathways affected by the signature, while TCGA revealed significant differences in T cell functions and immune checkpoint expression between low-risk and high-risk groups.

Summary: This study reveals the high expression level of ferroptosis-related factor ATG5 in HNSCC and NPC patients, which is correlated with poor prognosis and the expression of immune checkpoints. HNSCC patients with high expression of ATG5 showed poor response to immune checkpoint blockade therapy. Furthermore, ATG5 is associated with the G2M checkpoint pathway, indicating the potential value of G2M inhibitors in HNSCC/NPC treatment.
