Pediatric asthma: The REGAP consensus

Asthma is a common chronic disease in childhood that requires timely recognition and treatment. Maintenance treatment and patient education are crucial for disease control, and if control is inadequate, reevaluation of diagnosis or addition of other drugs may be necessary.
Asthma is one of the main chronic diseases in childhood, due to its high prevalence and its social and health costs. This document is a summary of a consensus guideline approved by 6 Spanish pediatric societies related to asthma and endorsed by the Spanish Pediatric Association. Asthma is easily identifiable by clinical criteria in most patients. Spirometry and other tests are helpful for diagnosis, especially in atypical cases. Asthma exacerbation is a frequent manifestation of the disease and must be identified and treated promptly. When asthma symptoms are frequent and the quality of life is affected, maintenance treatment must be instituted to achieve control of the disease. Low-dose inhaled corticosteroids are effective and safe for long-term use. Education of the patient with asthma is essential for good control. The main reason for poor asthma control is non-compliance with treatment, either due to its erratic and insufficient administration, or due to poor application technique of inhaled drugs. If control is not obtained despite adequate treatment, the diagnosis must be reconsidered, as well as the factors or comorbidities that make control difficult. Other drugs can be added to avoid high doses of inhaled corticosteroids, notably montelukast or long-acting beta 2 adrenergic agonists. Severe or difficult-to-control asthma, which does not respond to the usual treatments, should be managed in specialized units. (C) 2021 Asociacion Espanola de Pediatria. Drug reexposure in children with severe mucocutaneous reactions

Summary: Severe cutaneous adverse reactions (SCARs) frequently occur in pediatric patients with acute illnesses. Drug provocation test (DPT) is usually contraindicated in these cases, but it may be considered after careful evaluation and sequential study. This study confirms ibuprofen tolerance in three pediatric patients with SCAR.

Transition of Adolescents With Severe Asthma From Pediatric to Adult Care in Spain: The STAR Consensus

Summary: The study aimed to assess the consensus among a multidisciplinary expert panel on the transition of adolescents with severe asthma from pediatric to adult care. A 61-item survey was conducted using a 2-round Delphi process, involving 98 experts. Consensus was reached for 42 items (70%). The experts identified the main goal during the transition as promoting autonomy in managing severe asthma and prescribed treatments, and agreed on the importance of developing an individualized plan and identifying factors associated with the home environment.

Successful removal of distal persistent foreign body airway with CO2 cryotherapy in a child

Summary: We report a case of delayed diagnosis of foreign body (FB) aspiration in a 9-year-old boy. The FB was successfully extracted after unsuccessful attempts with flexible forceps, by using nitrous oxide cryotherapy to remove the associated inflamed granulation tissue. Early diagnosis is crucial in minimizing granulation tissue development.

Positive drug provocation with beta-lactam antibiotics in children: A single test may not be enough

Summary: This study retrospectively analyzed presumedly positive or inconclusive drug provocation tests conducted in pediatric allergy clinics and evaluated the need for repeat tests and their outcomes. The study found that the positive predictive value of drug provocation tests may be lower than expected, and therefore, a second confirmatory test is recommended within a few weeks or months.

Analysis of response of severe eosinophilic asthmatic patients to benralizumab

Summary: This study describes the super-responders to benralizumab in a series of 79 patients with severe eosinophilic asthma and compares them to non super-responders. The results show that almost two-thirds of the patients were super-responders and 36% had a complete response. Super-responders were associated with older age, higher eosinophils count, nasal polyposis as comorbidity, and less severe disease at baseline.

Quality of life in children with diseases causing central airway obstruction: Reliability and validity of the TAPQOL questionnaire

The results showed that the TAPQOL questionnaire is a valid tool for measuring the health-related quality of life in patients with CAO.
