Vitamin D and Its Role in the Lipid Metabolism and the Development of Atherosclerosis

Vitamin D plays a crucial role in cardiovascular health by influencing lipid levels, vascular tone, and atherosclerosis development. Deficiency of this vitamin increases cardiovascular risk, while supplementation has the potential to lower the risk factors.
Vitamin D, a crucial hormone in the homeostasis and metabolism of calcium bone, has lately been found to produce effects on other physiological and pathological processes genomically and non-genomically, including the cardiovascular system. While lower baseline vitamin D levels have been correlated with atherogenic blood lipid profiles, 25(OH)D supplementation influences the levels of serum lipids in that it lowers the levels of total cholesterol, triglycerides, and LDL-cholesterol and increases the levels of HDL-cholesterol, all of which are known risk factors for cardiovascular disease. Vitamin D is also involved in the development of atherosclerosis at the site of the blood vessels. Deficiency of this vitamin has been found to increase adhesion molecules or endothelial activation and, at the same time, supplementation is linked to the lowering presence of adhesion surrogates. Vitamin D can also influence the vascular tone by increasing endothelial nitric oxide production, as seen in supplementation studies. Deficiency can lead, at the same time, to oxidative stress and an increase in inflammation as well as the expression of particular immune cells that play a pivotal role in the development of atherosclerosis in the intima of the blood vessels, i.e., monocytes and macrophages. Vitamin D is also involved in atherogenesis through inhibition of vascular smooth muscle cell proliferation. Furthermore, vitamin D deficiency is consistently associated with cardiovascular events, such as myocardial infarction, STEMI, NSTEMI, unstable angina, ischemic stroke, cardiovascular death, and increased mortality after acute stroke. Conversely, vitamin D supplementation does not seem to produce beneficial effects in cohorts with intermediate baseline vitamin D levels.

The Role of Hypomagnesemia in Cardiac Arrhythmias: A Clinical Perspective

Summary: The importance of magnesium in maintaining normal heart rhythm is still controversial. Magnesium supplementation has been shown to reduce the recurrence of ventricular arrhythmias and improve ventricular response. It also has beneficial effects on the occurrence of polymorphic ventricular tachycardia and supraventricular tachycardia. However, the use of magnesium in the treatment of atrial fibrillation and after cardiac surgery remains controversial.

Clinical Utility of Boston-CTS and Six-Item CTS Questionnaires in Carpal Tunnel Syndrome Associated with Diabetic Polyneuropathy

Summary: Diabetic polyneuropathy (DPN) is a common complication of diabetes, while carpal tunnel syndrome (CTS) is a common neuropathy characterized by compression of the median nerve at the wrist. This study aimed to compare the Boston Carpal Tunnel Syndrome Questionnaire (BCTQ) and the six-item CTS symptoms scale (CTS-6) with nerve conduction studies (NCS) for detecting CTS in patients with DPN. The results showed that both the BCTQ and CTS-6 had significant differences in clinical evaluation between patients with and without CTS. Patients with DPN had higher chances of detecting CTS using these questionnaires.

Metabolomics in acromegaly: a systematic review

Summary: Despite advances in medical and surgical techniques, the therapeutic response to acromegaly still varies among patients, making personalized medicine necessary. Metabolomics can uncover the molecular mechanisms behind this response variability and identify new pathways for therapeutic management. A systematic review of metabolomic studies on acromegaly revealed that choline levels were negatively correlated with somatostatin receptors type 2 expression and positively correlated with MRI T2 signal and Ki-67 index in growth hormone-secreting pituitary adenomas (PAs). Metabolomic profiling also highlighted alterations in glucose metabolism, linoleic acid, sphingolipids, glycerophospholipids, arginine/proline, and taurine/hypotaurine. Matrix-assisted laser desorption/ionization coupled with mass spectrometry imaging accurately distinguished PAs from healthy pituitary tissue.

Antibacterial and Phytochemical Screening of Artemisia Species

Summary: This study aimed to investigate the antioxidant and antibacterial activities of leaf and stem ethanolic extracts of Artemisia absinthium L. and Artemisia annua L. The results showed that the leaf extract of Artemisia annua L. had the highest total phenolic content and the strongest antioxidant capacity, while both Artemisia annua L. and Artemisia absinthium L. leaf extracts exhibited low minimum inhibitory concentration and minimum bactericidal concentration against various bacteria.

Summary: Congenital heart defects (CHD) are the most common congenital abnormality, with a global birth prevalence of 9.41 per 1000 live births. The etiology of CHDs is complex and still poorly understood, with environmental factors accounting for about 10% of cases. Transcription factors and signaling molecules are potential genetic markers for CHDs. This review provides an overview of the current knowledge on genetic mechanisms and associations between transcription factors, signaling molecules, and congenital heart defects.

Comprehensive imaging in patients with suspected pulmonary arterial hypertension

Summary: Currently, various imaging techniques play a crucial role in the comprehensive evaluation of patients with suspected pulmonary hypertension (PH), providing important diagnostic information and facilitating early detection of the disease. Transthoracic echocardiography is the primary method for assessing right ventricle pressure overload and dysfunction, while right heart catheterisation is considered the gold standard assessment. Other imaging modalities such as chest radiography, CT, and CT pulmonary angiography are equally important for evaluating lung parenchyma and pulmonary arteries. Advanced techniques like dual-energy CT and cardiac MRI provide accurate diagnostic information for specific subtypes of PH.

Usefulness of the Edmonton Frail Scale in Assessing the Impact of Heart Failure on Frailty

Summary: The aim of this prospective study was to assess the incidence of physical frailty in patients with HF and the role of physical exercise in improving physical performance. The study found that 20.56% of patients enrolled in the study had mild and moderate frailty, and 2.83% had severe frailty. The prevalence of mild and moderate frailty was significantly higher in the HF group compared to the non-HF group. Exercise-based rehabilitation was found to improve physical performance and increase independence in daily activities and functional performance in HF patients.

Summary: This study aimed to investigate the influence of gender on post-angioplasty prognosis in patients with peripheral arterial disease (PAD). The results showed no significant difference in the incidence rates of major adverse limb events (MALE) between men and women undergoing peripheral angioplasty. The ankle-brachial index (ABI) was found to be an independent predictor for MALE.

Summary: Previous research has established normal limits for children's heart rates based on age. However, children of the same age can have significantly different heart rates depending on their weight, height, and body size. Our study aimed to create Z scores for heart rate based on various body size factors and compare them to the normal age-related heart rate limits.
