Neuroendocrine Carcinomas with Atypical Proliferation Index and Clinical Behavior: A Systematic Review

NECs are heterogeneous tumors that can be further divided into two prognostically distinct categories based on the Ki67 LI < 55%. Those with lower Ki67 LI have a better prognosis and should be treated and monitored differently from those with higher Ki67 LI.
Simple Summary Neuroendocrine carcinomas (NECs) are generally highly proliferative and clinically aggressive neuroendocrine neoplasms, but recent literature data suggested that NECs could be further subdivided into two prognostic distinct categories based on the Ki67 labeling index (LI) cut-off of 55%. However, no clear indication on the clinical management and the specific treatment protocol of NECs with a low Ki67 LI are available. We performed a systematic review of the literature to explore the clinicopathological features and the treatment response according to Ki67 LI cut-off in NECs, which is a hot topic in neuroendocrinology. Using evidence from 8 studies, for a total of 268 NEC affected patients, the systematic review showed that NECs with a low Ki67 LI had a better prognosis than the subgroup with higher Ki67 LI but worse than G3 neuroendocrine tumors suggesting that NECs are a heterogeneous disease for the pathology findings, the clinical behavior and the treatment response. Background: Highly proliferative (G3) neuroendocrine neoplasms are divided into well differentiated tumors (NETs) and poorly differentiated carcinomas (NECs), based on the morphological appearance. This systematic review aims to evaluate the clinicopathological features and the treatment response of the NEC subgroup with a Ki67 labeling index (LI) < 55%. Methods: A literature search was performed using MEDLINE, Cochrane Library, and Scopus between December 2019 and April 2020, last update in October 2020. We included studies reporting data on the clinicopathological characteristics, survival, and/or therapy efficacy of patients with NECs, in which the Ki67 LI was specified. Results: 8 papers were included, on a total of 268 NEC affected patients. NECs with a Ki67 LI < 55% have been reported in patients of both sexes, mainly of sixth decade, pancreatic origin, and large-cell morphology. The prevalent treatment choice was chemotherapy, followed by surgery and, in only one study, peptide receptor radionuclide therapy. The subgroup of patients with NEC with a Ki67 LI < 55% showed longer overall survival and progression free survival and higher response rates than the subgroup of patients with a tumor with higher Ki67 LI (>= 55%). Conclusions: NECs are heterogeneous tumors. The subgroup with a Ki67 LI < 55% has a better prognosis and should be treated and monitored differently from NECs with a Ki67 LI >= 55%.

Summary: Breast cancer is the most common cancer in women and a leading cause of cancer death. An artificial intelligence approach using a deep learning model based on neural networks is being studied to predict the risk of distant metastasis in breast cancer patients using MRI images. The performance of the model was evaluated and showed moderate accuracy in predicting distant metastasis.

Adenosine as adjunctive therapy in acute coronary syndrome: a meta-analysis of randomized controlled trials

Summary: The aim of this study was to analyze the effects of adenosine administration in patients with ACS undergoing myocardial revascularization. The results showed that adenosine did not provide any clinical benefit in terms of major adverse cardiac events, death, myocardial infarction, and heart failure. However, it was associated with an increased risk of advanced atrioventricular blocks and adenosine-triggered ventricular arrhythmias in patients with long ischemic time.

Summary: The study aimed to evaluate glucose control and insulin regimen modifications during pregnancy in women with T1D, comparing them to non-diabetic, normal-weight pregnant women. The findings showed that women with T1D need to modify their insulin regimen and dietary habits to control weight gain and promote optimal birth outcomes. However, due to the increase in insulin regimen, women with T1D gave birth to babies with higher birth weights than non-diabetic pregnant women.

Summary: This study aimed to assess the role of radiomics analysis in pre-operatively predicting molecular or clinicopathological prognostic factors in patients with endometrial carcinoma. The results showed that MRI has high sensitivity and specificity in predicting high-grade endometrial carcinoma, deep myometrial invasion, lymphovascular space invasion, and nodal metastasis.

Carcinoid Syndrome: Preclinical Models and Future Therapeutic Strategies

Summary: Carcinoid syndrome is a debilitating paraneoplastic disease occurring in 10-40% of patients with neuroendocrine tumors. Its main symptoms include flushing, diarrhea, hypotension, tachycardia, bronchoconstriction, venous telangiectasia, dyspnea, and fibrotic complications. Although there are drugs available, lack of therapeutic response and resistance remain challenges. Preclinical models serve as crucial tools for studying tumor progression and developing new therapies. This paper provides an overview of in vitro and in vivo models in NETs with carcinoid syndrome and discusses future developments and therapeutic approaches.

Summary: Gastric well-differentiated neuroendocrine tumors (g-NETs) account for 5-15% of neuroendocrine neoplasms in the gastrointestinal tract. Type 3 g-NETs are the most aggressive subcategory, with high rates of lymph-vascular infiltration and metastases. This systematic review found that size, grading, and gastric wall infiltration are important factors in the management of type 3 g-NETs.

Summary: The study aimed to investigate the influence of diabetic microvascular complications (DMC) on coronary collateral (CC) vessel development in patients with coronary chronic total occlusion (CTO). Results showed that patients with DMC had a higher prevalence of CC and high-grade CC compared with those without DMC. Furthermore, there was a positive association between the number of DMC in each patient and the prevalence of high-grade CC.

Summary: This study aimed to evaluate the magnesium content in human cirrhotic liver and its correlation with serum AST levels, expression of hepatocellular injury, and MELDNa prognostic score. The results showed that patients with cirrhosis had lower hepatic magnesium content and higher percentage of TRPM7-positive hepatocytes. The increase in hepatic magnesium content and TRPM7 expression were associated with worsening of MELDNa score. These findings suggest that severity of hepatocyte injury and prognosis in cirrhosis are closely related to magnesium depletion and overexpression of TRPM7.
