Summary: Although testosterone replacement therapy is commonly used for men with classical hypogonadism, its role in age-related decline of testosterone remains unclear due to limited evidence from long-term clinical trials. However, men aged 50 years and above with comorbidities and higher body mass index often exhibit symptoms of androgen deficiency and low testosterone levels. Clinicians face a challenging dilemma in initiating testosterone therapy and must carefully assess the benefits and risks based on limited clinical trial data. In this article, a practical approach to the assessment and management of such men is presented using a case scenario.

Research on MH is limited to North America and predominantly white participants, highlighting the need for a broader understanding to improve patient information delivery. A patient-centered approach to interactions and a multidisciplinary approach to address symptoms beyond MH and TRT may be beneficial.

Summary: Diagnosis of functional hypogonadism should lead to a thorough evaluation and optimization of general health, followed by a consideration of testosterone replacement therapy. Testosterone treatment can improve libido, but may not be as effective for erectile dysfunction. It can also have positive effects on insulin resistance, bone strength, physical strength, and mild depressive symptoms, although the long-term clinical significance of these improvements remains uncertain.

In 2019, the ACR/AF released guidelines for the treatment of hand osteoarthritis.
