The study compared three presentation methods and found that overlaying colored lesion candidates on volume-rendered images can help radiologists better detect cerebral aneurysms, although the combined method does not provide additional benefits in terms of diagnostic accuracy.
Purpose The clinical usefulness of computer-aided detection of cerebral aneurysms has been investigated using different methods to present lesion candidates, but suboptimal methods may have limited its usefulness. We compared three presentation methods to determine which can benefit radiologists the most by enabling them to detect more aneurysms. Materials and methods We conducted a multireader multicase observer performance study involving six radiologists and using 470 lesion candidates output by a computer-aided detection program, and compared the following three different presentation methods using the receiver operating characteristic analysis: (1) a lesion candidate is encircled on axial slices, (2) a lesion candidate is overlaid on a volume-rendered image, and (3) combination of (1) and (2). The response time was also compared. Results As compared with axial slices, radiologists showed significantly better detection performance when presented with volume-rendered images. There was no significant difference in response time between the two methods. The combined method was associated with a significantly longer response time, but had no added merit in terms of diagnostic accuracy. Conclusion Even with the aid of computer-aided detection, radiologists overlook many aneurysms if the presentation method is not optimal. Overlaying colored lesion candidates on volume-rendered images can help them detect more aneurysms.

Summary: This study evaluated the diagnostic performance of DWI and dynamic contrast-enhanced MRI in differentiating skull metastasis from osseous venous malformations and skull metastasis of various origins. The results showed that plasma volume and time-to-maximum enhancement were the most statistically significant parameters for differentiating skull metastasis from osseous venous malformations, while normalized mean ADC and peak enhancement values were the most statistically significant parameters for differentiating breast cancer from non-small cell lung cancer.

Summary: Triplet repeat diseases (TRDs) are a group of diseases caused by the expansion of three nucleotide repeats, including polyglutamine disorders, loss-of-function repeat disorders, RNA gain-of-function disorders, and polyalanine disorders. Many TRDs manifest neurologic symptoms, and neuroimaging plays a pivotal role in detection and management of TRDs.

Summary: This study examined the perfusion in patients with steno-occlusive disease using resting-state functional MRI (rsfMRI) and found that the time delay parameter of rsfMRI is more sensitive to hemodynamic changes compared to single-photon emission CT (SPECT) cerebral blood flow (CBF). However, the reliability of rsfMRI is lower than SPECT.

Resting-State fMRI: Emerging Concepts for Future Clinical Application

Shiori Amemiya, Hidemasa Takao, Osamu Abe

Summary: Resting-state functional magnetic resonance imaging (rsfMRI) allows for the investigation of spontaneous neural activity and the identification of resting-state networks (RSNs) in less than 10 minutes. The global rsfMRI signal, traditionally overlooked, is actually a dominant component that provides valuable information about local hemodynamics and can serve as a diagnostic biomarker. Recent spatiotemporal analyses have challenged conventional assumptions and revealed the fundamental association between the global signal and RSN organization.

Resting-state networks representation of the global phenomena

Shiori Amemiya, Hidemasa Takao, Shouhei Hanaoka, Osamu Abe

Summary: This study explored the inter- and intra-subject variations of the time delay and correlation coefficient of signal timeseries in each region using global mean signal as the reference signal, and found that these variations can organize networks equivalent to resting-state networks (RSNs). The findings not only provide further evidence that the local hemodynamic status could be the direct source of RSNs' spatial patterns, but also explain how the regional variations in hemodynamics combined with changes in the global events' power spectrum lead to the observations. These findings pose challenges to interpretations of rsfMRI studies, but also support the view that rsfMRI can offer detailed information related to global neurophysiological phenomena as well as local hemodynamics, which could have great potential as biomarkers.

Rapid MR Angiography Using 3D Gradient-echo Imaging and the Two-point Dixon Method to Evaluate Carotid Plaque

Ryusuke Irie, Shiori Amemiya, Tsuyoshi Ueyama, Yuichi Suzuki, Hidemasa Takao, Osamu Abe

Summary: Liver acquisition with volume acceleration-flex (LAVA-Flex) acquires multiple sequences simultaneously to provide more useful information for the diagnosis of carotid plaque compared to conventional time-of-flight MRA (cTOF MRA). In LAVA MRA, low-intensity plaques are clearly identified in in-phase images, while high-intensity plaques are more clearly identified in water-only or out-of-phase images. cTOF MRA does not offer superior visualization of the plaques.
