Efficacy of Serum Angiotensin II Levels in Prognosis of Patients With Coronavirus Disease 2019

The study aimed to determine serum angiotensin II levels in patients with COVID-19 and investigate its effect on disease prognosis. Results showed significantly lower angiotensin II levels in COVID-19 patients compared to healthy controls, with lower levels observed in patients with acute respiratory distress syndrome and those requiring ICU admission. Although low angiotensin II levels were linked to lung damage, they did not significantly correlate with mortality.
OBJECTIVES: We aimed to determine serum angiotensin II levels in patients with coronavirus disease 2019 infection and to investigate the effect of these levels on the prognosis of the disease. DESIGN: The study was planned prospectively and observationally. SETTING: The study was conducted in a tertiary university hospital. PATIENTS: Coronavirus disease 2019 patients older than 18 years old, polymerase chain reaction test positive, with signs of pneumonia on tomography, and hospitalized were included in the study. ICU need, development of acute respiratory distress syndrome, and in-hospital mortality were considered as primary endpoints. INTERVENTIONS: Blood samples were taken from patients three times for angiotensin II levels. MEASUREMENTS AND MAIN RESULTS: Angiotensin II levels were studied by enzyme-linked immunosorbent assay method. The SPSS 24.0 program (Statistics Program for Social Scientists, SPSS, Chicago, IL) was used to analyze the data. A total of 112 patients were included in the study, of which 63.4% of the patients were men. The serum angiotensin II levels were statistically significantly lower in the patients with coronavirus disease 2019 compared with the healthy control group (p < 0.001). There was no statistical significance between the serum angiotensin II levels measured at three different times (p > 0.05). The serum angiotensin II levels of the patients with acute respiratory distress syndrome were found to be statistically significantly lower than those without acute respiratory distress syndrome in three samples collected at different clinical periods (p < 0.05). The angiotensin II levels of the patients who required admission to the ICU at all three times of blood sample collection were found to be statistically significantly lower than those who did not (p < 0.05). Although the serum angiotensin II levels of the patients who died were low, there was no statistically significant difference in mortality at all three times (p > 0.05). CONCLUSIONS: The serum angiotensin II levels decrease significantly in patients with coronavirus disease 2019, and this decrease is correlated with lung damage.

Summary: Despite high vaccination rates, increasing cases continue to be reported, and the durability of vaccine-induced immune response remains a hot topic. This study investigated the changes in antibodies after the second dose of the CoronaVac vaccine at different time points. The results showed that while antibody positivity remained above 90% in the 6th month after vaccination, the median antibody titers decreased rapidly, especially in individuals with no prior history of COVID-19 infection.

Summary: Oral antiviral agents can achieve similar complete virological responses in the treatment of HBeAg-negative chronic hepatitis B. Younger age and low baseline viral load are the main factors for virological response. However, drug resistance and virological breakthrough in the lamivudine group, as well as side effects in the telbivudine group, were observed during long-term follow-up. The effectiveness of oral antiviral agents in achieving HBsAg seroclearance is low.

Summary: This study aimed to evaluate the immunogenicity of heterologous boosts by BioNTech against homologous boosts by CoronaVac in two groups of health care workers over a one-year period. The study found that there was a significant difference in antibody levels between individuals with and without prior COVID-19 infection at three months after receiving two doses of CoronaVac, but this difference was not observed in subsequent follow-ups. Those who received BioNTech as a third dose had higher antibody levels and neutralizing antibody responses against Wuhan and Omicron compared to those who received CoronaVac. The study suggests that continuing heterologous CoronaVac/BioNTech vaccination with at least a six-month interval booster could be an effective and safe strategy for protection against COVID-19, particularly in health care workers.

Summary: This study investigated the effectiveness of nasal irrigation with saline, povidone-iodine (PVP-I) 1%, and a combination of hypertonic alkaline and 1% PVP-I against SARS-CoV-2. The results showed that the use of the combination of hypertonic alkaline and 1% PVP-I solution was more effective in reducing the nasal viral load.

The relations of circulating agouti-related peptide and leptin with altered sleep architecture in patients with active Cushing's disease: a pilot study

Summary: This study aimed to evaluate the sleep architecture of patients with Cushing's disease (CD) and investigate the role of agouti-related peptide (AgRP) and/or leptin in sleep alterations in patients with active CD. Polysomnography was performed on 26 patients with active CD and compared with age and sex-matched control subjects. The results showed that patients with CD had reduced sleep efficiency and increased wake after sleep onset, and their serum levels of AgRP and leptin were elevated. These findings suggest that CD is associated with impaired sleep efficiency and continuity, which may be influenced by elevated levels of AgRP and leptin.

Skeletal Muscle Injury during COVID-19: A Cohort of 873 Patients

Summary: The aim of this study was to determine the prevalence of skeletal muscle injury in COVID-19 patients using creatine kinase (CK) levels. Medical records of COVID-19 patients were retrospectively analyzed and divided into high CK levels and normal CK levels groups. Results showed that the use of favipiravir and hospitalization in the intensive care unit were risk factors for high CK levels. CK levels were positively correlated with age, hospitalization duration, intensive care duration, and levels of liver function tests, serum urea, D-dimer, and ferritin levels, and negatively correlated with oxygen saturation and thrombocyte levels.

Summary: The aim of this study was to compare the effectiveness of two drugs in preventing infectious complications after prostate biopsy. The results showed that, when used for prophylaxis, patients receiving FT had significantly lower risks of developing lower urinary tract symptoms, having positive urine cultures, and requiring hospitalization for antibiotic treatment compared to patients receiving CIP.

Summary: This study compares neutralizing antibody levels and side effects in different vaccination schemes. The results show that administration of booster doses with mRNA vaccines leads to higher antibody levels and fewer side effects. Therefore, for those at risk of contracting severe COVID-19 and healthcare workers caring for this population, administration of booster doses with mRNA vaccines is the most accurate strategy.

Summary: This study aimed to investigate the prevalence of microalbuminuria and associated factors in patients with Graves' Disease (GD). The results showed that the prevalence of microalbuminuria in GD patients was 12.1%. There was a significant correlation between microalbuminuria and the level of thyroid-stimulating hormone receptor antibody (TRAb) in GD patients, suggesting that autoimmunity may be one of the underlying mechanisms of microalbuminuria in GD patients.

Summary: This study evaluated the effectiveness of COVID-19 vaccines among healthcare workers, finding that receiving two doses of CoronaVac and two doses of BNT162b2 showed the highest effectiveness, while receiving two doses of CoronaVac alone had the lowest effectiveness. The effectiveness of two doses of CoronaVac was not significant, but increased significantly with a booster dose of CoronaVac or BNT162b2. The results suggest the need for booster doses, with heterologous boosting using BNT162b2 potentially offering higher effectiveness for individuals who received two doses of CoronaVac.

Geriatric mortality risk factors in emergency department for non-traumatic abdominal pain

Summary: This study aimed to determine the factors affecting the mortality of geriatric patients presenting to the emergency department with non-traumatic abdominal pain, as well as the associations of these factors with mortality. The results showed that neurological diseases, a history of surgery, and anemia were associated with increased risk of mortality.
