Background Exclusive breastfeeding rates in many high-income countries are considerably lower than the World Health Organization recommendations. Younger mothers are less likely than older mothers to exclusively breastfeed or to exclusively breastfeed for a long duration. This systematic review explores interventions to increase the rate of exclusive breastfeeding among young mothers in high-income countries. Methods A systematic search of the following databases was completed in August 2020: CINAHL, PubMed, MEDLINE, ProQuest, PsychInfo, Web of Science, Cochrane, Scopus and Embase. A manual search of the reference lists of all the included studies and published systematic reviews was also performed. The Cochrane Collaboration Risk of Bias Tool was used to assess the quality of the included studies. A random effects model meta-analyses was applied. Heterogeneity of outcomes between the studies was assessed using both the chi(2) test and the I-2 statistic. Results Of 955 records identified in the search, 392 duplicates were removed, and nine studies met the inclusion criteria. Seven studies were randomised controlled trial (RCTs) and two were quasi-experimental in design. Eight were conducted in the United States. The interventions included peer counselling, telephone support, massage, gift packs, financial incentive and antenatal education. Most studies included a combination of strategies, peer counselling being the most common. A meta-analysis of four of nine included studies did not detect a difference in rate of exclusive breastfeeding to 3 months postpartum (RR 1.44; 95% CI 0.82, 2.55; p = 0.204). This review is limited by the relatively few studies which met the inclusion criteria and the small sample sizes of most included studies. High rates of attrition and formula supplementation among the participants made it difficult to detect a statistically significant effect. Consistency in follow up times would enable more studies to be included in a meta-analysis. Conclusions Peer counselling was the most promising strategy associated with higher rates of exclusive breastfeeding. However, further studies are needed to understand the breastfeeding experiences of young mothers. Young mothers should be targeted specifically in intervention studies.

Dietary patterns associated with obesity outcomes in adults: an umbrella review of systematic reviews

Canaan Negash Seifu, Paul P. Fahey, Tewodros G. Hailemariam, Steven A. Frost, Evan Atlantis

Summary: The umbrella review showed that the Mediterranean-type diet can reduce the risk of obesity in adults, while the Korean diet pattern may increase obesity prevalence. Diet quality is inversely related to weight gain or BMI, and increased intake of fruits and vegetables can help reduce obesity risk.

Unhealthy Diet Pattern Mediates the Disproportionate Prevalence of Obesity among Adults with Socio-Economic Disadvantage: An Australian Representative Cross-Sectional Study

Canaan Negash Seifu, Paul Patrick Fahey, Evan Atlantis

Summary: This study found an association between socio-economic disadvantage and obesity, with unhealthy dietary patterns mediating this relationship, especially in communities with high socio-economic disadvantage. Therefore, public health interventions should promote healthy dietary patterns, such as the Mediterranean diet, to reduce obesity.

Summary: The study evaluated the clinical usefulness of a new screening tool based on EOSS for activating weight management discussions in general practice. Thematic analysis of GP and patient views revealed five themes, including GP recognition of obesity as a health priority, obesity stigma, patient health literacy, patient motivation for self-management, and applicability and scalability.

Zao Ren An Shen capsule for insomnia: a double-blind, randomized, placebo-controlled trial

Yoann Birling, Xiaoshu Zhu, Nicole Avard, Caterina Tannous, Paul P. Fahey, Jerome Sarris, Alan Bensoussan

Summary: This study aimed to evaluate the efficacy and safety of Zao Ren An Shen (ZRAS) capsule, a Chinese herbal medicine, for the treatment of insomnia. The results showed that ZRAS capsule was not significantly different from placebo in terms of insomnia severity and adverse events. However, the dose of the product used in this study may have been lower than what is commonly used clinically, which may explain the lack of efficacy.

The efficacy of imagery in the rehabilitation of people with Parkinson's disease: protocol for a systematic review and meta-analysis

Tabitha Singer, Paul Fahey, Karen P. Y. Liu

Summary: This study aims to systematically assess the impact of imagery on rehabilitation outcomes for Parkinson's disease patients. By screening clinical trials and conducting meta-analysis, the effectiveness of imagery in activities, body structure and function, and participation outcomes will be evaluated. The findings of this study will provide guidance and recommendations for clinical practice and future research.

Summary: This study evaluated the effects of cognitive remediation on the performance of instrumental activities of daily living in older adults with mild cognitive impairment and early-stage dementia. The results showed that cognitive remediation improved the performance of these activities in individuals with cognitive impairment and dementia. Summary: Mal de debarquement syndrome (MdDS) is a neurological condition characterized by a constant sensation of self-motion. This study found that patients with MdDS may also experience visual disorders, particularly those with the non-motion-triggered subtype. Further research is needed to investigate the differing pathophysiological mechanisms between subtypes.
