Excess mortality in solid organ transplant recipients hospitalized with COVID-19: A large-scale comparison of SOT recipients hospitalized with or without COVID-19

Karola S. Jering, Martina M. McGrath, Finnian R. Mc Causland, Brian Claggett, Jonathan W. Cunningham, Scott D. Solomon

Summary: Solid-organ transplant recipients hospitalized with COVID-19 have higher odds of complications and in-hospital mortality compared to admissions for other causes, including non-COVID-19 pneumonia. The odds of in-hospital death are 2.5 times higher for SOT recipients with COVID-19 compared to those with non-COVID-19 pneumonia, regardless of the time of hospitalization.

Ischaemic and bleeding risk in atrial fibrillation with and without peripheral artery disease and efficacy and safety of full- and half-dose edoxaban vs. warfarin: insights from ENGAGE AF-TIMI 48

Summary: In patients with atrial fibrillation and peripheral artery disease, those with PAD have higher risk of stroke and cardiovascular events. While both higher dose edoxaban and warfarin have similar efficacy and safety in stroke and bleeding prevention in AF patients, lower dose edoxaban is inferior in stroke prevention for patients with PAD.

Eilken, Alice McElhinney, Andrea Derix, Steven J. Atlas, Patrick T. Ellinor, Anthony A. Philippakis, Christopher D. Anderson, Jennifer E. Ho, Puneet Batra, Steven A. Lubitz

Summary: This study used electronic health record datasets and employed sampling and natural language processing to recover missing data, resulting in a community-based research cohort. The results indicated that NLP can reduce missing vital signs, and the recovered vital signs were highly correlated with values derived from structured fields. Compared to convenience samples, C3PO showed lower incidence of atrial fibrillation and myocardial infarction/stroke, and the risk models were better calibrated.

Cunningham, Shaan Khurshid, Carolina Roselli, Honghuang Lin, Satoshi Koyama, Kaoru Ito, Yoichiro Kamatani, Issei Komuro, Sean J. Jurgens, Emelia J. Benjamin, Puneet Batra, Pradeep Natarajan, Kenney Ng, Udo Hoffmann, Steven A. Lubitz, Jennifer E. Ho, Mark E. Lindsay, Anthony A. Philippakiss, Patrick T. Ellinor

Summary: This study utilized deep learning models to recognize and measure right heart structures and function. Genome-wide association studies identified several loci associated with right heart measurements, some of which were previously linked with congenital heart disease. Additionally, a polygenic predictor was associated with right ventricular ejection fraction and risk of dilated cardiomyopathy.

Coronary Artery Disease and Heart Failure With Preserved Ejection Fraction: The ARIC Study

Summary: Coronary artery disease (CAD) is a significant risk factor for heart failure with preserved ejection fraction (HFpEF). The risk of developing HFpEF and heart failure with reduced ejection fraction (HFrEF) is highest within the first year after CAD develops. After one year post-CAD, the risk of developing HFrEF and HFpEF is similar. Echocardiographic measurements, particularly left ventricular diastolic function, partially account for the relationship between CAD and incident HFpEF.

Summary: In this study, a machine learning model was developed to measure native myocardial T1 time, a marker of myocardial fibrosis, and identify the biological pathways underlying fibrosis in the human heart. The model was applied to a large sample of UK Biobank participants and revealed associations between greater T1 time and various cardiovascular diseases and conditions. Genome-wide association analysis further identified 11 independent loci associated with T1 time, implicating genes involved in important biological processes such as glucose transport, iron homeostasis, tissue repair, oxidative stress, cardiac hypertrophy, and calcium signaling. This research provides insights into the pathways involved in myocardial fibrosis and myofibroblast cell state acquisition, and highlights the potential of machine learning in large-scale quantification of cardiac fibrosis using imaging techniques.
