Prognostic Markers and Long-Term Outcomes After Aortic Valve Replacement in Patients With Chronic Aortic Regurgitation

Background The objectives of the present study were (1) to evaluate the echocardiographic prognostic factors associated with improved left ventricular (LV) systolic function after aortic valve replacement, and (2) to compare the long-term outcomes after aortic valve replacement in chronic aortic regurgitation (AR) patients with or without LV dysfunction. Methods and Results A total of 280 patients who underwent aortic valve replacement because of chronic aortic regurgitation were studied. Patients with reduced LV systolic function (LV ejection fraction [LVEF] <50%; group reduced LVEF [rEF]; N=80) were compared with those with preserved LV systolic function (LVEF >= 50%; group preserved LVEF; N=200). Postoperative clinical outcomes, overall survival, and freedom from cardiac death were compared. Postoperative echocardiographic examinations were reviewed, and changes in echocardiographic parameters were analyzed. The parameters related to LVEF improvement or normalization were evaluated, and risk factors affecting long-term survival were identified. Follow-up was complete in 100% of patients, with a median follow-up of 104.8 months. Overall and cardiac mortality-free survival rates at postoperative 10 years were 80.1% and 92.9% and 87.3% and 97.2% in groups rEF and preserved LVEF, respectively (P=0.036 and P=0.058, respectively). LVEF tended to decrease in the early postoperative period but improved thereafter in both groups. Preoperative early diastolic transmitral flow velocity/mitral annular tissue velocity ratio was a parameter of postoperative improvement or normalization of LVEF in all patients (area under the curve, 0.719; P=0.003) and in group rEF patients (area under the curve, 0.726; P=0.011) with a cutoff value of 12.73. Preoperative early diastolic transmitral flow velocity/mitral annular tissue velocity ratio also was the parameter of overall survival in all patients (hazard ratio [HR], 1.08; P=0.001) and in group rEF patients (HR, 1.08; P=0.005). Conclusions Long-term outcomes and survival after aortic valve replacement were related to preoperative LV function in patients with chronic aortic regurgitation. Preoperative early diastolic transmitral flow velocity/mitral annular tissue velocity ratio was correlated with the postoperative improvement or normalization of LVEF and long-term survival, especially in group rEF patients.

The long-term fate of ascending aorta aneurysm after wrapping versus replacement

Summary: This study aimed to examine the long-term fate of aortic diameter expansion at 4 cardiac regions after wrapping or replacement during aortic valve surgery. The results showed that aortic wrapping and replacement may be long-term durable treatment options for patients with a moderately enlarged ascending aorta. However, careful evaluation of redilation in the proximal arch after an aorta wrapping procedure is recommended.

This study found that transient elastography (TE) and the enhanced liver fibrosis (ELF) test were unable to predict the degree of liver fibrosis in Fontan patients. Elevated N-terminal pro-brain natriuretic peptide levels and ventricular end-diastolic pressure may be associated with advanced liver fibrosis.

Sirolimus-Embedded Silk Microneedle Wrap to Prevent Neointimal Hyperplasia in Vein Graft Model

Summary: We investigated the effectiveness of a sirolimus-embedded silk microneedle wrap as an external vascular device for drug delivery, inhibition of neointimal hyperplasia, and vascular remodeling. In a vein graft model using dogs, the sirolimus-embedded silk MN wrap successfully delivered the drug to the intimal layer and prevented vein graft dilatation. It also inhibited neointimal hyperplasia, resulting in decreased collagen density in the intima layer.

Effect of Preoperative Administration of Intravenous Ferric Carboxymaltose in Patients with Iron Deficiency Anemia after Off-Pump Coronary Artery Bypass Grafting: A Randomized Controlled Trial

Summary: Patients with iron deficiency anemia (IDA) who were scheduled for off-pump coronary artery bypass grafting (OPCAB) were randomly assigned to receive either intravenous ferric carboxymaltose (IVFC) or placebo treatment. The IVFC treatment significantly reduced the need for red blood cell and platelet transfusions, and patients in the IVFC group had higher levels of hematologic parameters and iron concentrations after surgery. Preoperative IVFC treatment improved the values of hematologic parameters and iron bioavailability in patients with IDA undergoing OPCAB.

Hemodynamic management during off-pump coronary artery bypass surgery: a narrative review of proper targets for safe execution and troubleshooting

Summary: Off-pump coronary surgery requires careful management of cardiac displacement and hemodynamics to avoid complications and poor outcomes. Objective hemodynamic targets, such as maintaining mean arterial pressure and venous oxygen saturation within specific ranges, must be considered before the surgery. Transesophageal echocardiography should be used to assess for any potential complications and ensure that the patient can tolerate the procedure. Careful inspection of the ascending aorta is also necessary to prevent embolism.
