Excess mortality due to COVID-19 varied widely among municipalities within the three most affected regions in Italy, with higher rates among the elderly and males. Despite common regional health policies, nearby municipalities may have different mortality burdens possibly due to local reinforcements of regional policies. Identifying areas with higher mortality rates and virus spread pathways can help understand reasons for differences and identify vulnerable areas for future epidemic recurrences.
The impact of Coronavirus Disease 2019 (COVID-19) on mortality in Italy has been described at the regional level, while less is known about mortality in municipalities, although the spatial distribution of COVID-19 in its first wave has been uneven. We aimed to describe the excess mortality due to COVID-19 from February 23rd to April 30th, 2020 in the three most affected Italian regions, in age and gender subgroups within each municipality. Excess mortality varied widely among municipalities even within the same region; it was higher among the elderly and higher in males except in the >= 75 age group. Thus, nearby municipalities may show a different mortality burden despite being under common regional health policies, possibly as a result of local reinforcements of regional policies. Identifying the municipalities where mortality was higher and the pathways used by the virus to spread may help to concentrate efforts in understanding the reasons why this happened and to identify the frailest areas in light of recurrences of the epidemic.

Diego Raimondo, Antonio Raffone, Federica Renzulli, Ivano Raimondo, Arianna Raspollini, Giulia Rovero, Camilla Franceschini, Manuela Maletta, Luigi Carbone, Jacopo Lenzi, Antonio Mollo, Renato Seracchioli, Paolo Casadio

Summary: This study investigated the prevalence of uterine artery arteriosclerosis in women undergoing total hysterectomy for benign diseases, and its associations with clinical, laboratory, and ultrasound factors. The results showed a high prevalence of uterine artery arteriosclerosis, which was associated with higher BMI, serum triglycerides, and PSV, as well as lower serum HDL.

Diego Raimondo, Antonio Raffone, Federica Renzulli, Giada Sanna, Arianna Raspollini, Linda Bertoldo, Manuela Maletta, Jacopo Lenzi, Giulia Rovero, Antonio Travaglino, Antonio Mollo, Renato Seracchioli, Paolo Casadio

Summary: Objective of this study was to assess the prevalence of central sensitization (CS) and its association with demographic and clinical factors in patients with endometriosis. The study was conducted as a single-center, observational, cross-sectional study where consecutive patients with endometriosis were enrolled and assessed using the CS Inventory questionnaire. Results showed that the prevalence of CS was 41.4% and it was significantly associated with moderate to severe chronic pelvic pain, posterolateral parametrium involvement, hormonal therapy failure, migraine or tension-type headache, irritable bowel syndrome, and anxiety or panic attacks. Therefore, identifying CS through CS Inventory could be helpful in counseling the patient and choosing multimodal treatment.

Summary: The COVID-19 pandemic has shown the gaps in global health surveillance, disease prevention, and treatment due to issues in financing national health systems, especially in low-income and middle-income countries. This study aims to assess global health spending at the start of the pandemic, determine the amount of development assistance for pandemic preparedness and response, and analyze the expectations for future health spending and investment in pandemic preparedness.

Changes in Artery Diameters and Fetal Growth in Cases of Isolated Single Umbilical Artery

Elena Contro, Laura Larcher, Jacopo Lenzi, Marina Valeriani, Antonio Farina, Eric Jauniaux

Summary: In fetuses with isolated single umbilical artery (SUA), the diameter of the remaining umbilical artery is significantly larger than in control fetuses. A diameter of <3.1 mm in the remaining artery is associated with a higher risk of fetal growth restriction (FGR). This study highlights the importance of measuring the diameter of the remaining artery to identify fetuses at risk of FGR.

Conspiracy beliefs, COVID-19 vaccine uptake and adherence to public health interventions during the pandemic in Europe

Luca Regazzi, Alberto Lontano, Chiara Cadeddu, Pasquale Di Padova, Aldo Rosano

Summary: This study examined the relationship between individual attitudes, socio-demographic factors, conspiracy beliefs, COVID-19 vaccine hesitancy, and preferences about pandemic policies. The findings showed that male gender, middle age, lower levels of education, unemployment, lower levels of trust and satisfaction, and right-wing political orientation were associated with a higher probability of believing in conspiracy theories. Eastern European countries had higher levels of conspiracy beliefs compared to other regions. Individuals who expressed conspiracy beliefs had lower COVID-19 vaccine uptake, were less satisfied with the way health services coped with the pandemic, and less supportive of governmental restrictions.

Summary: This study provides global, regional, and national estimates of the burden of chronic respiratory diseases (CRDs) from 1990 to 2019. The results show that although the age-standardised prevalence, death, and DALY rates of CRDs have decreased, they still cause a substantial burden and deaths worldwide. The highest death and DALY rates are found in low and low-middle SDI countries, emphasizing the urgent need for improved preventive, diagnostic, and therapeutic measures.

Association of Socioeconomic Factors and Physical Activity with Health-Related Quality of Life in Italian Middle School Children: An Exploratory Cross-Sectional Study

Francesco Sanmarchi, Lawrence M. Scheier, Laura Dallolio, Matteo Ricci, Giulia Longo, Andrea Ceciliani, Alice Masini

Summary: Health-related quality of life (HRQoL) is important for predicting future outcomes for children and adolescents. Factors such as socioeconomic status, cardio fitness, and physical activity levels are associated with HRQoL. This study found that younger students, females, having both parents employed, higher family educational status, and higher engagement in routine physical activity were all associated with higher HRQoL. The findings highlight the importance of promoting regular physical activity and addressing socioeconomic disparities to improve children's well-being. Future studies should consider additional measures such as nutrition, cultural factors, and family functioning.

Summary: This study aimed to investigate the impact of gestational diabetes mellitus (GDM) on the mother-infant relationship and maternal mental health. Through a cohort study of 642 women in Bologna, Italy, it was found that women with GDM had significantly lower relationship scores at 15 months postpartum, but not at 6 months. These findings suggest a delayed effect of GDM on the mother-infant relationship.

Summary: This study investigated the pattern and trend of immune-mediated inflammatory diseases (IMIDs) from 1990 to 2019. It found significant regional and global variation in the incidence of IMIDs, with some diseases increasing while others decreasing. Innovative prevention and management strategies are needed to address the increasing incidence of rheumatoid arthritis and other IMIDs.
