Summary: The study found that the switch from CNI to belatacept in HIV-positive kidney allograft recipients may increase long-term kidney allograft survival rates. Results showed that the conversion treatment did not increase the risk of HIV replication after a year, but the importance of patient medication compliance and adverse events should be noted.

Summary: Female kidney transplant recipients (KTRs) had a high prevalence of anal high-risk (hr) human papillomavirus (HPV) (46%) and almost 3-fold higher odds of anal hrHPV than immunocompetent controls. More than 50% of anal hrHPV-positive KTRs were infected with HPV types covered by the nonavalent HPV vaccine.

Summary: In this study, an investigation was conducted on the use of remdesivir in COVID-19 patients who had undergone kidney transplantation, showing that remdesivir performed well in terms of safety, with most patients tolerating the treatment without significant adverse events. However, the mortality rate was significantly higher in patients aged 65 and older, highlighting the need for caution in treating this particular group of patients.

Summary: This cohort study examined cancer incidence among 11,418 Italian recipients of kidney transplantation over a 25-year period. The study found a decreased risk of Kaposi's sarcoma, but no significant changes in the incidence of other cancers over time. Summary: Posttransplant malignancies, particularly recurrent and de novo, are a significant complication in solid organ transplant recipients. Risk factors include older age, viral infections, and pretransplant malignancies. The overall risk of posttransplant malignancies is at least double, and the therapy for cancer in transplant patients poses unique challenges.

Summary: This study evaluated the treatment outcomes of dialysis fistula aneurysms in kidney transplant recipients, comparing surgical revisions in this population to ESRD patients. The results showed significant differences in patency rates between the two groups after surgical corrections of dialysis fistula aneurysms, indicating that reconstructive procedures provide good long-term results with a lower risk of complications.

Summary: Age-related changes in physiology can affect drug metabolism and responsiveness, potentially requiring adjustments in drug dosing for elderly patients. Elderly kidney transplant recipients are at higher risk for complications, especially due to increased susceptibility to immunosuppressant side effects; however, current dosing and monitoring protocols may not account for age differences.

Summary: This study compared the outcomes of plasma exchange (PE), double filtration plasmapheresis (DFPP), and immunoadsorption (IA) in the treatment of late antibody mediated rejection (AMR) in kidney transplantation recipients. The results showed that IA had a higher graft survival rate. Multivariate analysis indicated that baseline estimated glomerular filtration rate, rituximab use, interstitial inflammation, and transplant glomerulopathy were associated with graft loss.

Summary: Living donor liver transplantation (LDLT) in the United States is limited to specific experienced transplant centers. This study analyzed transplant registry data from 2002 to 2019 to assess the impact of recipient and center factors on the likelihood of receiving LDLT. The study found that while the likelihood of receiving LDLT has increased in recent years, there are still disparities in access based on age, gender, race, ethnicity, obesity, and socioeconomic factors.

Summary: The SRTR Living Donor Collective (LDC) is the first attempt to create a lifetime registry for living donor candidates in the United States, requiring transplant programs to register candidates. A survey revealed that funding, electronic data submission options, and elimination of duplicative pre-operative information requirements were identified as enabling factors for LDC participation.

Summary: This article summarizes the outcomes of older adults receiving kidney transplants and discusses strategies to improve their access to transplantation. Older adults are a growing subgroup on the kidney transplant waitlist and have higher mortality and lower graft survival compared to younger adults. Clinicians must balance risk and survival when determining suitability for transplantation in older patients. Access to transplantation for older adults can be increased through interventions targeting frailty and reducing waiting times through optimized organ use. Focused study and educational interventions are needed to improve outcomes for this vulnerable group.

Summary: The accessibility and cost reduction of genetic sequencing techniques have led to an expanded utilization of genetic testing in clinical practice. In the context of living kidney donation, genetic evaluation is increasingly used to identify genetic kidney disease in potential candidates, particularly younger individuals. However, there are numerous challenges and uncertainties associated with genetic testing for asymptomatic living kidney donors. Not all transplant practitioners are aware of the limitations of genetic testing or have access to specialized genetic counseling or expertise in renal genetics.

Summary: This study aimed to explore the attitudes, barriers, and facilitators of living donor liver transplant (LDLT) in US liver transplant centers. The findings showed that there is a strong interest in expanding LDLT, with a high level of agreement that it mitigates donor shortage and should be offered to suitable candidates. However, barriers such as institutional factors and surgical expertise were identified at non-LDLT centers, while waitlist candidate and donor factors were the main barriers at LDLT centers.

Summary: Background. Biliary complications (BCs) remain a significant issue after liver transplantation, impacting patient and graft survival. This study used real-world evidence to estimate the incidence of BCs after liver transplantation and explore the factors associated with their occurrence. The results showed that BCs increased the risk of posttransplant death and graft loss and significantly added to healthcare costs.

Summary: Through the observation of 285 transplant centers in the United States from 2000 to 2016, the association between center volume and the outcomes of high-risk kidney transplants has been determined. Patients with a BMI >= 35 kg/m(2) in low-volume centers have a higher risk of graft loss, and recipients aged >= 70 years in low-volume centers have a higher risk of death. No association between center volume and outcomes was found for donors with high kidney donor profile index, hepatitis C infection, or acute kidney injury.
