Worldwide Esophageal Cancer Collaboration: neoadjuvant pathologic staging data

To address uncertainty of whether pathologic stage groupings after neoadjuvant therapy (ypTNM) for esophageal cancer share prognostic implications with pathologic groupings after esophagectomy alone (pTNM), we report datasimple descriptions of patient characteristics, cancer categories, and non-risk-adjusted survivalfor pathologically staged cancers after neoadjuvant therapy from the Worldwide Esophageal Cancer Collaboration (WECC). Thirty-three institutions from six continents submitted data using variables with standard definitions: demographics, comorbidities, clinical cancer categories, and all-cause mortality from first management decision. Of 7,773 pathologically staged neoadjuvant patients, 2,045 had squamous cell carcinoma, 5,686 adenocarcinoma, 31 adenosquamous carcinoma, and 11 undifferentiated carcinoma. Patients were older (61 years) men (83%) with normal (40%) or overweight (35%) body mass index, 0-1 Eastern Cooperative Oncology Group performance status (96%), and a history of smoking (69%). Cancers were ypT0 (20%), ypT1 (13%), ypT2 (18%), ypT3 (44%), ypN0 (55%), ypM0 (94%), and G2-G3 (72%); most involved the distal esophagus (80%). Non-risk-adjusted survival for yp categories was unequally depressed, more for earlier categories than later, compared with equivalent categories from prior WECC data for esophagectomy-alone patients. Thus, survival of patients with ypT0-2N0M0 cancers was intermediate and similar regardless of ypT; survival for ypN+ cancers was poor. Because prognoses for ypTNM and pTNM categories are dissimilar, prognostication should be based on separate ypTNM categories and groupings. These data will be the basis for the 8th edition cancer staging manuals following risk adjustment for patient, cancer, and treatment characteristics and should direct 9th edition data collection.

Summary: Enhanced recovery after surgery (ERAS) programs provide a framework to standardize care processes and improve outcomes by reducing physiological surgical stress. Despite challenges in implementing ERAS in esophageal and gastric cancer resection procedures, systematic reviews have confirmed the benefits of reduced complications and hospital stay. However, there is heterogeneity and lack of standardization in the ERAS components. Despite the growing evidence of effectiveness, adherence to ERAS protocols remains low.

Summary: This study compared the impact of robot-assisted lobectomy (RAL) and video-assisted lobectomy (VAL) on quality of life in patients with resectable non-small cell lung cancer (NSCLC). The findings showed that both approaches had similar effects on health-related quality of life and postoperative pain up to 48 weeks after surgery, with some minor statistical differences at week 4.

Summary: This study aimed to investigate the impact of the number of lymph nodes dissection (LND) on patients receiving neoadjuvant chemotherapy for esophageal squamous cell carcinoma. The results showed a non-linear relationship between LND and overall survival (OS) and disease-free survival (DFS). Moderate lymphadenectomy was beneficial for patients with nodal metastases, but more extended lymphadenectomy did not improve survival in patients without nodal metastases.

Summary: Since the first successful esophagectomy for cancer in 1913, significant advancements have been made in diagnosis, staging, and therapy. Improved imaging techniques, surgical procedures, perioperative management, and multidisciplinary collaboration have led to therapeutic options for more patients. Future innovations in imaging, molecular biology, genetics, artificial intelligence, machine learning, robotics, and nanotechnology will further enhance personalized treatment plans.

Primary Surgery Not Inferior to Neoadjuvant Chemoradiotherapy for Esophageal Adenocarcinoma

Summary: A retrospective cohort study showed that primary surgery performed through transthoracic approach can yield comparable overall and disease-free survival after 5 years in locally advanced esophageal adenocarcinoma, with potentially fewer postoperative complications and shorter hospital and intensive care unit stay compared with neoadjuvant chemoradiotherapy followed by surgery.

Trends and patterns of care of sentinel node biopsy in cutaneous melanoma: a population-based study in Queensland

Summary: This study evaluated the rate of sentinel node biopsy (SNB) in high-risk melanoma patients and identified factors associated with the procedure not being performed. The study found that although adherence to SNB guidelines has increased, the overall SNB rate remains low, with almost two-thirds of eligible cases not receiving the procedure in 2019. This study highlights the crucial need to improve access to SNB for melanoma surgery in the Queensland population.

Summary: Brain metastases after esophagectomy are rare and diagnostic uncertainty remains. Radiotherapy and advanced stage tumors are associated with the development of brain tumors after esophagectomy. Patients with brain tumors treated with curative intent have a significantly better median overall survival.

Summary: The clinical behavior of signet ring cell histology in gastric cancer has been controversial due to the lack of a standardized definition. This meta-analysis aimed to gain a deeper understanding of the prognosis and clinicopathological features of signet ring cell gastric cancer compared to other histologies. The findings revealed that the prognosis of signet ring cell gastric cancer is closely linked to the disease stage and the specific comparison group employed. The lack of standardized classification may contribute to the variability in clinical behavior. Therefore, efforts should be made to establish a uniform classification system for gastric cancer to enhance clarity and coherence in research and clinical practice.
