Summary: The current TNM staging system for pancreatic ductal adenocarcinoma (PDAC) has shown inconsistent results in validating the revised definitions of T and N categories and stage groups. Thus, a new staging system incorporating various factors is proposed to improve the accuracy of prognosis assessment. This proposed staging system demonstrates superior prognostic performance compared to the TNM staging system, effectively stratifying survival and addressing the limitations of TNM staging in prognostication.

Summary: We investigated the correlation between clinicopathological factors and prognosis in patients with high-risk factors in accordance with the FIGO 2018 staging criteria. Our study analyzed a real-world database of 6,192 patients who underwent radical hysterectomy and identified non-squamous cell carcinoma histology, regional lymph node metastasis, pT2 classification, and ovarian metastasis as independent risk factors for mortality. We also re-classified patients based on pathological findings and found that patients with stage IIIC2p disease had worse prognoses.

Summary: We conducted a clinical trial to compare the efficacy of accelerated hypofractionated radiation therapy (RT) with conventionally fractionated 60-Gy RT in patients with stage III unresectable non-small cell lung cancer (NSCLC). The results showed no significant differences in treatment response, overall survival, progression-free survival, and local tumor control rates between the two groups. This suggests that accelerated hypofractionated RT may not be superior to conventional fractionation in patients with unresectable stage III NSCLC undergoing concurrent chemoradiation therapy.

Summary: Enhanced recovery after surgery (ERAS) programs provide a framework to standardize care processes and improve outcomes by reducing physiological surgical stress. Despite challenges in implementing ERAS in esophageal and gastric cancer resection procedures, systematic reviews have confirmed the benefits of reduced complications and hospital stay. However, there is heterogeneity and lack of standardization in the ERAS components. Despite the growing evidence of effectiveness, adherence to ERAS protocols remains low.

The study found that although adherence to SNB guidelines has increased, the overall SNB rate remains low, with almost two-thirds of eligible cases not receiving the procedure in 2019. This study highlights the crucial need to improve access to SNB for melanoma surgery in the Queensland population.

Summary: The clinical behavior of signet ring cell histology in gastric cancer has been controversial due to the lack of a standardized definition. This meta-analysis aimed to gain a deeper understanding of the prognosis and clinicopathological features of signet ring cell gastric cancer compared to other histologies. The findings revealed that the prognosis of signet ring cell gastric cancer is closely linked to the disease stage and the specific comparison group employed. The lack of standardized classification may contribute to the variability in clinical behavior. Therefore, efforts should be made to establish a uniform classification system for gastric cancer to enhance clarity and coherence in research and clinical practice.
