Current Status of Cell-Based Therapy in Patients with Critical Limb Ischemia

(1) Background: The treatment of peripheral arterial disease (PAD) is focused on improving perfusion and oxygenation in the affected limb. Standard revascularization methods include bypass surgery, endovascular interventional procedures, or hybrid revascularization. Cell-based therapy can be an alternative strategy for patients with no-option critical limb ischemia who are not eligible for endovascular or surgical procedures. (2) Aims: The aim of this narrative review was to provide an up-to-date critical overview of the knowledge and evidence-based medicine data on the position of cell therapy in the treatment of PAD. The current evidence on the cell-based therapy is summarized and future perspectives outlined, emphasizing the potential of exosomal cell-free approaches in patients with critical limb ischemia. (3) Methods: Cochrane and PubMed databases were searched for keywords critical limb ischemia and cell therapy. In total, 589 papers were identified, 11 of which were reviews and 11 were meta-analyses. These were used as the primary source of information, using cross-referencing for identification of additional papers. (4) Results: Meta-analyses focusing on cell therapy in PAD treatment confirm significantly greater odds of limb salvage in the first year after the cell therapy administration. Reported odds ratio estimates of preventing amputation being mostly in the region 1.6-3, although with a prolonged observation period, it seems that the odds ratio can grow even further. The odds of wound healing were at least two times higher when compared with the standard conservative therapy. Secondary endpoints of the available meta-analyses are also included in this review. Improvement of perfusion and oxygenation parameters in the affected limb, pain regression, and claudication interval prolongation are discussed. (5) Conclusions: The available evidence-based medicine data show that this technique is safe, associated with minimum complications or adverse events, and effective.

Summary: This study evaluated the sensitivity and specificity of VI-RADS in the diagnosis of muscle-invasive bladder cancer and found that it has a high accuracy in discriminating between non-muscle invasive and MIBC. However, despite the valuable information provided by mpMRI in bladder cancer staging, the authors do not consider VI-RADS as a replacement for TURBT.

Mulberry Posterior Inferior Nasal Turbinate Is Associated with a Lower Pharyngeal pH Environment

Summary: This study aimed to investigate the association between Mulberry-like changes of the posterior inferior nasal turbinate (MPINT) and acidic pH. Through questionnaires, video endoscopy, and 24-hour pH monitoring, it was found that MPINT was more common in an acidic environment. These findings suggest a possible link between acidic pH and the development of MPINT.

Comparison of carotid endarterectomy and repeated carotid angioplasty and stenting for in-stent restenosis (CERCAS trial): a randomised study

Summary: This study aimed to compare the safety and effectiveness of carotid endarterectomy with stent removal (CEASR) and repeat percutaneous transluminal angioplasty with or without stenting (rePTA/S) in patients with carotid in-stent restenosis (ISR). The results showed that the incidence of restenosis was significantly lower in the CEASR group compared to the rePTA/S group, although there was no significant difference in the number of patients with restenosis ≥70% within 1 year between the two groups. Therefore, CEASR may be considered as an effective and safe treatment option for carotid ISR.

Summary: Cellular immunity is crucial in the immune response to SARS-CoV-2. We compared the results of two interferon-gamma release tests (Quan-T-Cell SARS-CoV-2 and T-SPOT.COVID) in 90 subjects who had previous COVID-19 infection or vaccination. The evaluation showed similar results for both tests, but Quan-T-Cell appeared to be slightly more sensitive. T-cell-mediated immunity assessment is a more sensitive indicator of immune response than IgG seropositivity assessment.

5th Metatarsal Jones Fracture - To Treat Conservatively, or Surgically Using Headless Double-Threaded Herbert Screw?

Summary: The purpose of this study was to compare the outcomes of osteosynthesis using the Herbert screw with conservative treatment for Jones fractures. The results showed that surgical treatment had better short-term recovery outcomes compared to conservative treatment, which required a longer duration of healing. Therefore, surgical treatment is a superior option for Jones fractures.

Type IV Titanium Hypersensitivity: Rare, or Rarely Detected?

Summary: The presented review aims to summarize the current knowledge of hypersensitivity to titanium and its detection methods. Although allergic reactions to titanium are rare, their actual occurrence can be expected to be higher due to its problematic detection. The unreliable nature of cutaneous patch tests in titanium allergies and the lack of awareness of the Lymphocyte Transformation Test among clinicians present challenges in diagnosis.

Review of patterns in homicides by sharp force: one institution's experience

Summary: This paper offers a retrospective review of patterns in homicides by sharp force in a Czech Republic hospital over a 13-year period. The study examines various aspects of such cases, including the number and location of injuries, defensive wounds, weapon type, cause and place of death, victim and perpetrator profiles, their relationship, and toxicological findings. Additionally, the study analyzes accompanying blunt force trauma as a possible sign of escalating assault. The results of the analysis reveal both consistency and notable differences from existing research.
