Summary: The study aimed to evaluate the efficacy and safety of liraglutide 1.8 mg in participants undergoing laparoscopic adjustable gastric banding (LAGB) for type 2 diabetes mellitus (T2DM) and obesity. The results showed that there was no additional improvement in glycemic control or body weight with liraglutide therapy. However, the study was underpowered to detect significant changes in the primary and secondary outcomes.

Summary: Black females with T2DM show widespread downregulation of genes across aerobic metabolism pathways before RYGB, which resolves 1 year post-RYGB and is related to improvements in clinical markers. These data support the efficacy of RYGB for improving skeletal muscle health, especially in patients with T2DM.

Summary: This study compared the glucagon response to insulin-induced postprandial hypoglycemia between post-bariatric surgery individuals and non-surgical control individuals. The results showed that the glucagon response was significantly lower in the post-bariatric surgery group, regardless of the surgical modality, compared to the non-surgical control group. There were no significant differences between patients with post-bariatric hypoglycemia and surgical control individuals, suggesting that impaired counter-regulation is not the root cause of post-bariatric hypoglycemia.

Summary: This study investigated the role of ghrelin in weight reduction and glycemic and lipid control after Roux-en-Y gastric bypass surgery. The results showed that gastric and circulating ghrelin levels significantly increased after the surgery. However, lowering ghrelin levels did not affect the metabolic benefits induced by the surgery.

Summary: This study assessed the impact of bariatric surgery on remission and relapse of type 2 diabetes mellitus (T2DM) and analyzed predictive factors. The results showed that bariatric surgery significantly improved BMI, fasting glucose, and haemoglobin A1c (HbA1c). The remission rates at 1 and 10 years post-surgery were 74% and 53% respectively. T2DM duration, baseline HbA1c, and ensuing insulin therapy were identified as the strongest predictors of remission. Short T2DM duration and good glycemic control before surgery were essential for long-lasting remission, while weight loss did not impact the long-term relapse of T2DM.

Summary: This study compared the short- and medium-term outcomes of SG and RYGB after failure of primary GB. In propensity score matching analysis, SG group had significantly shorter operative time and lower post-operative complications. Weight loss and comorbidities at 2 years follow-up were similar between the two groups, but SG had a significantly lower rate of adverse events.

Summary: We identified biomarkers for disease progression in type 2 diabetes cohorts, including metabolites, lipids, and proteins. Various substances such as homocitrulline, isoleucine, 2-aminoadipic acid, triacylglycerol species, and sphingomyelin 42:2;2 levels were found to predict faster progression towards insulin requirement. Proteins like GDF15/MIC-1, IL-18Ra, CRELD1, NogoR, FAS, and ENPP7 were associated with faster progression, while others like SMAC/DIABLO, SPOCK1, and HEMK2 predicted slower progression rates. The findings also suggested possible disease mechanisms and potential therapeutic avenues to slow diabetes progression.

Summary: The study investigated the relationship between preoperative beta-cell function and diabetes remission in individuals undergoing gastric bypass surgery for severe obesity and type 2 diabetes. The results showed that presurgery beta-cell glucose sensitivity was negatively correlated with diabetes duration, HbA(1c), insulin use, and diabetes remission scores. However, adding preoperative beta-cell function to established clinical models did not improve the predictive ability for diabetes remission after surgery.
