Noninvasive biomarkers in heart transplant: 2020-2021 year in review

Summary: This review provides an update on noninvasive blood-based methods for rejection surveillance and diagnosis in heart transplantation, highlighting the good test performance of donor-derived cell-free DNA (dd-cfDNA) and microRNAs (miR) in detecting acute rejection. These biomarkers have the potential to address the limitations of invasive endomyocardial biopsy and provide valuable information for rejection monitoring and targeted therapy.

Summary: In a US cross-sectional study, the estimated prevalence of familial dilated cardiomyopathy (DCM) among probands was high, and there were differences in the risk of DCM among first-degree relatives across different races and ethnicities, with higher risk observed in first-degree relatives of non-Hispanic Black probands.

Higher levels of allograft injury in black patients early after heart transplantation

Summary: Black patients who undergo heart transplantation have higher rates of antibody-mediated rejection and worse long-term graft survival. Donor-derived cell free DNA (ddcfDNA) is released into the blood after allograft injury. This study found that immediately after transplant, Black patients had higher levels of %ddcfDNA. In the first week post-transplant, the rate of decay in %ddcfDNA was similar for both Black and non-Black patients. Black patients were also more likely to receive a race mismatched organ, which may explain the higher levels of early allograft injury.

The report also emphasizes the importance of reducing readmissions after LVAD implantation in improving patient outcomes.

Transapical Cannulation With a Dual Lumen Cannula for Mechanical Circulatory Support in Cardiogenic Shock

Summary: This study analyzed the gene sequences of 729 patients with idiopathic DCM and found rare TTR variants in 2 patients, but without features of hTTR. Both patients had other plausible genetic causes of DCM. Additionally, the frequency of TTR Va1142Ile in non-Hispanic Black patients was comparable to the control group.

Clinical myocardial recovery in advanced heart failure with long term left ventricular assist device support

Summary: Left ventricular assist-device (LVAD) implantation is a life-saving therapy for patients with advanced heart failure. However, the translation of reverse remodeling and cardiac recovery after LVAD support into meaningful outcomes, such as explantation, is challenging. In selected patients, significant functional improvement and long-term freedom from recurrent heart failure events have been demonstrated. Predictors of myocardial recovery suggest the potential use of less invasive VADs in early-stage heart failure. It is important to develop tools to predict HF reversibility, optimize recovery with medical therapy, and monitor myocardial improvement before LVAD implantation.

Summary: The number of heart transplants performed annually in the United States and worldwide continues to increase, but there has been little change in graft longevity and patient survival over the past 2 decades. The use of circulating biomarkers and molecular diagnostics has shown predictive value in rejection monitoring and diagnosing posttransplant complications. The understanding of immunogenetics has enhanced the knowledge of antibody-mediated rejection and the evaluation of donor human leukocyte antigens.
