Management of connective tissue diseases associated interstitial lung disease: a review of the published literature

Purpose of review Interstitial lung disease (ILD), though a common and often a severe manifestation of many connective tissue diseases (CTD), is challenging to manage because of its variable presentation and the relative lack of guidelines to assist the clinician. In this review, we discuss the approach to diagnosis, treatment, and monitoring patients with CTD-associated ILD, with a focus on systemic sclerosis (SSc), rheumatoid arthritis (RA), and idiopathic inflammatory myopathy (IIM). Recent findings High-resolution computed tomography scan and pulmonary function testing can be reliably used to diagnose ILD and monitor progression, and often to determine its likely histologic subtype and severity. In SSc-ILD, randomized controlled trials show ILD stabilization with cyclophosphamide treatment; preliminary data from another randomized controlled trial demonstrates similar findings with mycophenolate. There are no robust clinical trials supporting specific treatments for RA-ILD or IIM-ILD, but rituximab in RA-ILD, and cyclophosphamide, mycophenolate and calcineurin inhibitors in IIM-ILD show promise. Though ILD contributes substantially to morbidity and mortality in patients with CTD, there are minimal data to guide its management except in SSc-ILD.

Summary: This study in a national cohort of veterans examined the association between the use of anti-TNF or corticosteroids and all-cause mortality in patients with IBD. Results suggest that anti-TNF therapy may be linked to reduced mortality compared with prolonged corticosteroid use, particularly among patients with Crohn disease.

Summary: High fibromyalgianess is associated with persistent glucocorticoid use independent of inflammatory activity. Participants with high/very high baseline fibromyalgianess were more likely to be taking prednisone at follow-up relative to those with low fibromyalgianess.

Predictors of Glucocorticoid Use for Acute Optic Neuritis in the United States, 2005-2019

Summary: This retrospective cohort study analyzed insured ON patients in the United States and found that factors such as comorbidities, clinician type, visit number, and region were associated with glucocorticoid use. The study suggests the need for further exploration of the reasons for care variation in ON and patient/clinician preferences to optimize treatment.

Summary: This study aimed to examine the correlation between the self-administered Fibromyalgia Survey Questionnaire (FSQ) and quantitative sensory testing (QST) in patients with rheumatoid arthritis (RA). The results showed a weak correlation between FSQ and QST, indicating that the two measures cannot be used interchangeably.

Summary: Although traditional methods show an association between NSAID and IBD exacerbations, further analysis suggests that this may be secondary to residual bias. These findings may reassure patients and clinicians considering NSAID as a nonopioid pain management option.

Summary: Although patients with rheumatic and musculoskeletal diseases are at higher risk of adverse outcomes from COVID-19, their willingness to be vaccinated is low. Concerns about vaccine side effects and worsening of symptoms contribute to vaccine hesitancy. This study aimed to understand the decision-making process of COVID-19 vaccination among patients with rheumatic and musculoskeletal diseases through qualitative analysis of their comments.

Characteristics of Facilities With Early and Rapid Ustekinumab Adoption for Patients With Inflammatory Bowel Disease

Summary: This study examined the association between facility characteristics and the adoption of ustekinumab for inflammatory bowel disease. The results showed that ustekinumab adoption increased over time and was higher in urban facilities with more teamwork. Early adopters were more likely to be high-volume facilities. These findings suggest the importance of targeted dissemination strategies to improve medication uptake and enhance inflammatory bowel disease care.

Prevalence and prescribing patterns of oral corticosteroids in the United States, Taiwan, and Denmark, 2009-2018

Summary: This study investigated and compared the prescription patterns of oral corticosteroids (OCS) in the general populations of the United States, Taiwan, and Denmark from 2009 to 2018. The findings showed a steady increase in OCS use, particularly short-term prescribing to healthy adults, in all three countries.
