Summary: A retrospective study compared the value of positive intradermal dilution testing (IDT) to skin prick tests (SPT) in diagnosing allergies and determining the outcomes of allergy immunotherapy (AIT) in 371 patients. The results showed that IDT could identify patients who would benefit from AIT more accurately than SPT, doubling their chance of successful treatment without increasing the risk of therapeutic failure. Positive IDT following negative SPT offers superior sensitivity in detecting allergens relevant to AIT-responsive atopic disease diagnosis.

Diagnosis of AOM is still based on clinical evaluation, with proposed stricter criteria. Sinusitis diagnosis remains clinical, with imaging reserved for complications. Pain relief is key in AOM management, and watchful waiting or delayed antibiotic prescription are suitable strategies for some patients.

Summary: The prevalence of allergic rhinitis (AR) and the associated socioeconomic burden have increased. Practical guidelines for AR management that consider patients' and physicians' values and preferences are needed. The Korean Academy of Asthma, Allergy and Clinical Immunology revised clinical guidelines for AR and made recommendations for AR pharmacotherapy based on a meta-analysis and review, although the evidence certainty was low. Well-designed randomized controlled trials are needed to measure appropriate outcomes.
