Management outcomes of secondary glaucoma due to retinopathy of prematurity: A 19-year prospective study at a tertiary eye care Institute. The Indian Twin cities ROP Screening (ITCROPS) database report number 8

Purpose To report the clinical presentation and management outcomes of glaucoma in the Indian Twin cities retinopathy of prematurity (ROP) Screening database. Methods All children with diagnosis of ROP and glaucoma between 1997 and 2016 from a prospective database were included. Glaucoma was classified as open when anterior chamber (AC) was deep, closed when AC was shallow or flat and neovascular when there was extensive iris neovascularization. ROP was classified based on International classification of ROP. Results The prevalence of secondary glaucoma in our cohort was 1.36% (82 eyes of 6000 children). Eighty-two eyes of 54 children with secondary glaucoma due to ROP where included in this study. The distribution of glaucoma among the ROP stages included, stage V (58.5%), stage 1V (24.3%), stage III (2.4%) and stage II (1.2%) eyes. Median (interquartile range) duration from birth to glaucoma diagnosis was 7.8 (4.2, 24.9) months. Type of glaucoma was angle closure in 39 (47.6%), open angle in 35 (42.7%) and neovascular in 8 (9.8%) eyes. Retinal interventions included vitreoretinal surgery in 59 (72%), retinal laser in 14 (17%) and intravitreal bevacizumab injection in 19 (23.1%) eyes. The mean (+/- standard deviation) IOP at presentation was 22.6 +/- 11.8 mm Hg. Glaucoma was managed medically in 66 (76%) and surgically in 16 (19.5%) eyes. The mean follow up for the entire cohort was 1.14 +/- 2.24 years. At final visit, 37% eyes with ROP and glaucoma had ambulatory vision with mean IOP of 16.0 +/- 8.1 mm Hg and 56 eyes (68.2%) needed glaucoma medications. Conclusion In this large ROP cohort, 1.36% eyes developed secondary glaucoma. Majority of them had stage V or IV ROP and 1/5 of them needed glaucoma surgery. Around 1/3(rd)of the ROP eyes with glaucoma had ambulatory vision.

Sebaceous gland carcinoma: analysis based on the 8th edition of American Joint Cancer Committee classification

Summary: This study assessed the prognostic value of the T category of the 8th edition AJCC classification in periocular sebaceous gland carcinoma (SGC). The results showed that T4 tumors had higher rates of lymph node metastasis, systemic metastasis, and death compared to T1, T2, and T3 tumors. However, there was no significant association between T category and tumor recurrence.

Clinico-microbiological Features and Treatment Outcomes of Serratia Keratitis and Comparison with Pseudomonas aeruginosa Keratitis

Summary: The purpose of this study was to compare the clinico-microbiological features and outcomes of Serratia keratitis and Pseudomonas aeruginosa keratitis. The results showed that Serratia keratitis usually occurs in eyes with a compromised surface and has good resolution with medical therapy. There were no significant differences in outcomes between Serratia and Pseudomonas aeruginosa keratitis.

Summary: This study aimed to describe and validate a novel inflammatory score (IS) system for managing infectious endophthalmitis. The study included patients with acute post-cataract surgery endophthalmitis and scored the severity of inflammation in four ocular tissues. The results demonstrated that the IS scoring system had good consistency and accuracy, and can be used for decision-making in the treatment and monitoring of infectious endophthalmitis.

Estimation of the full shape of the crystalline lens in-vivo from OCT images using eigenlenses

Summary: In this study, we successfully estimated the full shape of the in vivo crystalline lens using the eigenlenses method, which only utilizes information visible through the pupil. Compared to previous methods, eigenlenses showed improved repeatability, robustness, and computational resource utilization in estimating crystalline lens shape. Eigenlenses can efficiently describe the full shape changes of the crystalline lens with accommodation and refractive error.

Novel En Face OCT-Based Closure Patterns in Idiopathic Macular Holes

Summary: This study aimed to determine the postoperative en face OCT pattern of closure in idiopathic macular holes (MHs). Two different closure patterns, linear and round, were observed on en face OCT. Patients with linear closure had better visual acuity. Further research is needed to determine the functional significance of these closure patterns.

Summary: This study reported the demographic profile and clinical characteristics of retinopathy of prematurity (ROP) in posterior Zone I. The study included 130 eyes of 67 infants with a mean gestational age and birth weight of 29.3 (+/- 2.2) weeks and 1217.3 (+/- 381.9) grams, respectively. ROP in posterior Zone I has distinct features and subtypes different from ROP in other zones.

Clinical presentations, management and factors affecting outcomes in posterior segment open globe injuries - An analysis of 2360 eyes

Summary: This study retrospectively analyzed the clinical presentations, management, and factors affecting outcomes in 2,360 cases of posterior segment open globe injuries. The results showed that posterior open globe injuries generally have a favorable functional outcome in the absence of retinal detachment and orbital fracture. Several factors associated with a favorable functional outcome were identified.

Summary: Quantifying the geometry of human crystalline lens as a function of age and accommodation is crucial for better treatments of cataract and presbyopia. This study validates the use of eigenlenses to estimate the full shape of dis-accommodated lenses in vivo. The results demonstrate that eigenlenses can accurately estimate the shape of dis-accommodated lenses.

Electronic medical records driven big data analytics in retinal diseases, report number 1: non-oncological retinal diseases in children and adolescents in India

Summary: This study aims to describe the spectrum and demographic distribution of non-oncological retinal diseases in children and adolescents in an ophthalmic hospital network in India. It found that 8.44% of new patients aged 0-21 years were diagnosed with non-oncological retinal pathology in at least one eye. The major retinal diseases included retinopathy of prematurity, retinal dystrophy, and retinal detachment.

Ocular surface squamous neoplasia with 360° limbal involvement: a study of 130 patients

Summary: This study compared the risk factors, clinical features, and management outcomes of ocular surface squamous neoplasia (OSSN) with 360 degrees of limbal involvement (360-OSSN) and segmental limbal involvement (SL-OSSN). The results showed that 360-OSSN patients more often had longer duration of symptoms, prior misdiagnosis, and prior intervention. They also had a higher incidence of scleral fixity, corneal/scleral melt, intraocular and orbital tumor extension, and advanced T stage at presentation compared to SL-OSSN patients. Furthermore, lymph node and distant metastasis were more common in the 360-OSSN group during the follow-up period. Aggressive management is required for this advanced form of the disease.
