P-Tau as prognostic marker in long term follow up for patients with shunted iNPH

This study aimed to assess the correlation between pre-surgical levels of CSF Beta Amyloid protein, Total Tau protein, and Phospho-Tau protein with long-term clinical outcome in iNPH patients. Results showed a significant association between low levels of P-Tau and T-Tau with favorable clinical outcome after surgery. Therefore, these proteins may have potential value in predicting effectiveness in patients post-surgery.
Objective:Diagnosis of idiopathic Normal Pressure Hydrocephalus (iNPH) relies solely on clinical and radiological criteria while, unlike other neurological diseases, the analysis of cerebrospinal fluid markers is not used in clinical practice. Nevertheless, the overlapping of neurodegenerative diseases affects the long-term shunt efficacy and this occurrence should be detected before surgery. Therefore, we performed this study in order to assess the correlation between pre-surgical levels of CSF Beta Amyloid protein, Total Tau protein and Phospho-Tau protein with long-term clinical outcome. Methods:Between March 2012 and May 2016 we prospective evaluated all patients with iNPH according to guidelines criteria and we analysed CSF concentration of these proteins before and during surgery. Two years after surgery we evaluated iNPH score for all patients, grouping them in shunt responders and non-responders. Results:A total of 117 patients were included: Tap Test non-responders were 58 and at two years we had 35 shunt responders and 15 shunt non-responders. We found a significative difference between shunt-responders and shunt non-responders for pre surgical T-Tau (p: 0.02) and for P-Tau (p: 0.01). All the proteins were significantly associated with clinical outcome after surgery with different cut-off values; in particular, having a 'low' value of T-Tau, P-Tau and A beta 1-42 resulted in favourable outcome after surgery. Conclusions:Low level of P-Tau is a useful CSF biochemical prognostic factor for good clinical outcome at least two years after shunt; meanwhile a lower A beta 1-42 might suggest that the pathophysiology of iNPH could have something in common with other neurodegenerative diseases of the elderly.

Treatment strategy for vertebral metastases from anal squamous cell carcinoma: a comprehensive literature review and case report

Summary: This study reported a rare case of multiple vertebral metastases from anal squamous cell carcinoma (SCC) and aimed to define a treatment strategy. Through case presentation and literature review, it was found that there is currently no universally accepted treatment strategy for vertebral metastases from anal SCC. Prompt and aggressive, local and systemic, and multimodal treatment of the vertebral lesions may be crucial for improving patient outcomes.

Leveraging process mining for modeling progression trajectories in amyotrophic lateral sclerosis

Summary: In this study, the evolution trajectories of ALS patients were investigated using Process Mining techniques, revealing differences in disease progression based on patients' characteristics. The study found that most patients start without impairments, and impairment of Walking/Self-care abilities is the most common initial symptom. Spinal onset increases the risk of experiencing the loss of Walking/Self-care ability as the first impairment, and older age at onset corresponds to a faster progression to death. The comparison between the trial population and the clinical cohort showed similarities in the natural progression of the disease, but differences in observed trajectories potentially due to trial scheduling and recruitment criteria.

Summary: Although the key neuropathological hallmarks of Parkinson's disease (PD) are known, the underlying pathogenic mechanisms of the disease still need to be elucidated. This study focused on evaluating the levels and activity of c-Rel protein, which may be involved in PD pathophysiology. The findings suggest that PD is characterized by the loss of NF-κB/c-Rel activity, and the reduction of c-Rel DNA-binding could potentially serve as a novel biomarker for PD.

Exploring depression in Alzheimer's disease: an Italian Delphi Consensus on phenomenology, diagnosis, and management

The study also highlights the strong link between depression and AD, as well as specific features of depression in AD compared to major depressive disorder (MDD).

Summary: This observational study found that choroid plexus volume is significantly increased in patients with frontotemporal lobar degeneration (FTLD) spectrum, and it is associated with disease-specific biomarkers, cognitive/behavioral deficits, and patterns of cortical atrophy. Choroid plexus volume may serve as a potential biomarker for FTLD, especially at the early stage of the disease. Further longitudinal studies are needed to determine its role in disease onset and progression.

Cigarette Smoking and Intracranial Aneurysms: A Pilot Analysis of SNPs in the CYP2A6 Gene in the Italian Population

Summary: This study investigated the associations between specific single-nucleotide polymorphisms (SNPs) in the CYP2A6 gene and the presence and rupture of intracranial aneurysms (IAs) in Italian patients. The results showed that the frequency of aSAH was significantly higher in current smokers, regardless of the pattern of CYP2A6 SNPs. There was a correlation between IA rupture and cigarette smoking in patients with the heterozygous CYP2A6 *1B2 allele.
