Summary: Intracranial germinomas are rare tumors that mostly affect male pediatric patients, usually developing in the pineal and suprasellar regions. They can cause endocrinological disturbances, visual deficits, and increased intracranial pressure. Diagnosis is based on MRI, serum and CSF markers, and tumor biopsy. Germinomas are highly radiosensitive, but resistant cases may benefit from personalized management and targeted therapies. Genetic and immunological testing play a crucial role in identifying potential targets for therapy. PD-1 and PD-L1 immune markers show promise as potential targets for immune checkpoint inhibitors in germinomas.

Summary: This cross-sectional study found substantial variation in the use of targeted therapies for EGFR- and ALK-altered NSCLC across Medicaid programs, with evidence of underuse in 30 of 33 states. The observed variation was associated with Medicaid policies, oncologist density, and state gross domestic product per capita. This study suggests that targeted therapies are underused in many state Medicaid programs, limiting access to efficacious treatments; attention is needed for state policies and characteristics that may limit access to these useful drugs.
