Summary: This review discusses the evolution of endovascular treatment for acute ischemic stroke, its current state, and the challenges it faces in the next decade. Endovascular thrombectomy (EVT) has rapidly become a standard of care for stroke patients, thanks to scientific advancements in patient selection, stroke assessment, technical developments, and imaging technologies. While EVT has shown overwhelmingly positive results in randomized controlled trials, it is only available for a small portion of stroke patients, and not all successful recanalized EVT patients achieve excellent functional outcomes. The major challenges ahead include early detection of stroke symptoms, accurate diagnosis, effective recanalization, and personalized post-EVT stroke care.

Summary: In this study, the influence of small vessel disease and cortical atrophy on clinical outcome after acute ischemic stroke was assessed using native cranial computed tomography (NCCT). The results showed that pre-existing brain deterioration was significantly correlated with clinical outcome. Both age-related white matter changes (ARWMC) and cortical atrophy were independent predictors of clinical outcome after endovascular treatment (EVT).

Summary: This study aimed to evaluate the impact of the hyperdense middle cerebral artery sign (HMCAS) on the outcome and technical challenges in patients undergoing endovascular thrombectomy (EVT). The results showed that HMCAS did not predict a poor outcome in patients treated with EVT. However, patients with HMCAS required longer procedure times and a higher number of thrombus passes.

Summary: In this study, mechanical properties of thrombi retrieved with EVT in acute stroke patients were analyzed, revealing a strong association between fibrin/platelet content and thrombus stiffness, as well as impacts of erythrocytes and platelets on thrombus stiffness. These findings provide a basis for future research and development of next-generation EVT devices tailored to thrombus composition.

The coronary balloon-mounted stent serves as a valuable rescue treatment for intracranial stent occlusion.

Summary: It is unclear whether WMH on MRI provides relevant cerebrovascular prognostic information beyond traditional risk factors in hypertensive patients. A post hoc analysis of the SPRINT-MIND study showed that the addition of WMH to prediction models improved the prognostic ability above vascular risk and demographics alone, with 26.3% of the effect on the primary outcome mediated through WMH.

Summary: This study found that portable magnetic resonance imaging (pMRI) could successfully identify white matter hyperintensity (WMH) in the brain and be used in unconventional settings. The results suggest that pMRI may have a potential role in reducing disparities in neuroimaging.

Summary: This study found a significant association between blood pressure variability after endovascular thrombectomy (EVT) and mortality and disability. This association was independent of mean blood pressure levels within the first 24 hours post EVT. Therefore, blood pressure variability in the first 24 hours may be a new therapeutic target to improve outcomes after EVT for acute ischemic stroke.

Summary: This study evaluated the outcomes of mechanical thrombectomy in LVO patients with ASPECTS 0-2 and found that age, National Institutes of Health Stroke Scale on admission, and successful recanalization were independently associated with favorable outcomes.

Summary: This study demonstrates how time-varying covariates and selection bias influence the association estimates between anticoagulation and health-related quality of life (HRQoL) in patients with atrial fibrillation. The results show that the association between warfarin use and better HRQoL strengthens when accounting for time-varying confounding and selection bias.
